Is there anything good about menopause? Yep, here are 4 things to look forward to

<p><em><a href="https://theconversation.com/profiles/yvonne-middlewick-1395795">Yvonne Middlewick</a>, <a href="https://theconversation.com/institutions/edith-cowan-university-720">Edith Cowan University</a></em></p> <p>Menopause is having a bit of <a href="https://journals.sagepub.com/doi/full/10.1177/13675494231159562">a moment</a>, with less stigma and more awareness about the changes it can bring.</p> <p>A recent senate inquiry <a href="https://www.aph.gov.au/Parliamentary_Business/Committees/Senate/Community_Affairs/Menopause/Report">recommended</a> public education about perimenopause and menopause, more affordable treatments and flexible work arrangements.</p> <p>But like many things in life the experiences of menopause are on a continuum. While some women find it challenging and require support, others experience some physical and emotional benefits. These are rarely reported – but we can learn from the research available and, importantly, from people’s lived experiences.</p> <p>Here are four changes to look forward to once you reach menopause.</p> <h2>1. No more periods or related issues</h2> <p><a href="https://theconversation.com/what-are-the-most-common-symptoms-of-menopause-and-which-can-hormone-therapy-treat-225174">Menopause</a> is considered “complete” 12 months after the final period of a woman (or person assigned female at birth) who previously menstruated.</p> <p>Perhaps unsurprisingly, the benefit at the top of the list is no more periods (unless you are taking hormone therapy and still have your womb). This can be particularly beneficial for women who have had to manage <a href="https://www.mdpi.com/1660-4601/18/4/1951">erratic, unpredictable and heavy bleeding</a>.</p> <p>At last, you don’t need to keep sanitary protection in every bag “just in case”. No more planning where the bathroom is or having to take extra clothes. And you’ll save money by not purchasing sanitary products.</p> <p>There is also good news for women who have had heavy bleeding due to uterine fibroids – common benign gynaecological tumours that affect up to 80% of women. The evidence <a href="https://journals.lww.com/menopausejournal/abstract/2020/02000/uterine_fibroids_in_menopause_and_perimenopause.17.aspx">suggests</a> hormonal changes (for women not taking hormone therapy) can lead to a reduction in the size of fibroids and relieve symptoms.</p> <p>Women who suffer from menstrual migraine may experience an improvement in migraines post-menopause as their hormonal fluctuations begin to settle – but the timeframe for this <a href="https://www.maturitas.org/article/S0378-5122(20)30329-7/abstract">remains unclear</a>.</p> <p>For some women, no more periods also means more participation in social activities from which they may have been excluded due to periods. For example, <a href="https://www.tandfonline.com/doi/full/10.1080/08952841.2014.954502">religious activities or food preparation</a> in some cultures.</p> <h2>2. Getting your body and your groove back</h2> <p>Throughout their reproductive lives, women in heterosexual relationships are usually the ones expected to be <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6115298/">proactive about preventing pregnancy</a>.</p> <p>Some post-menopausal women <a href="https://www.tandfonline.com/doi/full/10.1080/08952841.2018.1396783">describe</a> a re-emergence of their sexuality and a sense of sexual freedom that they had not previously experienced (despite contraceptive availability) as there is no longer a risk of pregnancy.</p> <p>A participant in my research <a href="https://eprints.soton.ac.uk/451254/">into women’s experiences of menopause</a> described the joy of no longer being child-bearing age:</p> <blockquote> <p>I’ve got a body back for me, you know, coz I can’t get pregnant, not that I haven’t enjoyed having [children] and things like that and it was a decision to get pregnant but I feel like, ooh my body isn’t for anybody now but me, people, you know?</p> </blockquote> <p>For women who have chosen to be child-free there may also be a sense of freedom from social expectations. People will likely stop asking them when they are planning to have children.</p> <h2>3. A new chapter and a time to focus on yourself</h2> <p>Another participant <a href="https://eprints.soton.ac.uk/451254/">described</a> menopause as an unexpected “acceleration point” for change.</p> <p>Women told us they were more accepting of themselves and their needs rather than being focused on the needs of other people. Researchers have previously <a href="https://sk.sagepub.com/books/individualization">tracked this shift</a> from “living for others” to “a life of one’s own”.</p> <p>Some women find the strength of emotions at this time a challenge, whereas others find their potency can <a href="https://doi.org/10.1145/3290605.3300710">facilitate liberation</a> – enabling them to speak their minds or be more assertive than at any other time in their lives.</p> <h2>4. Increased self-confidence</h2> <p>A new sense of liberation can fuel increased self-confidence at menopause. This has been <a href="https://doi.org/10.1145/3290605.3300710">reported</a> in studies based on in-depth <a href="https://doi.org/10.1080/0167482X.2016.1270937">interviews with women</a>.</p> <p>Confidence boosts can coincide with <a href="https://eprints.soton.ac.uk/451254/">changes in career and sometimes in relationships</a> as priorities and self-advocacy transform.</p> <h2>Life on the other side</h2> <p>It can be hard to think about what is good about menopause, particularly if you are having challenges during perimenopause – but these can get better with time.</p> <p>In <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6910086/">cultures where women are valued as they become older</a>, women describe themselves as positively contributing to the community. They find they gain power and respect as they age.</p> <p>We need to work towards more positive societal attitudes on this front. Thinking of trying a new diet? 4 questions to ask yourself before you do

Messages about <a href="https://doi.org/10.2196/38245">dieting and weight loss</a> are amplified on social media, with a never-ending cycle of weight loss fads and diet trends.</p> <p>Amid often conflicting messages and misinformation, if you’re looking for diet advice online, it’s easy to become confused and overwhelmed.</p> <p>So before diving into the latest weight loss trend or extreme diet, consider these four questions to help you make a more informed decision.</p> <h2>1. Is the diet realistic?</h2> <p>Have you considered the financial cost of maintaining the diet or lifestyle, and the time and resources that would be required? For example, do you need to purchase specific products, supplements, or follow a rigid meal plan?</p> <p>If the diet is coming from someone who is trying to sell you something – such as a particular weight-loss product you need in order to follow the diet – this could be a particular red flag.</p> <p>Many extreme diet recommendations come from a place of privilege and overlook food access, affordability, cooking skills, where you live, or even your culture and ethics.</p> <p>If the diet has these sorts of issues it can lead to <a href="https://doi.org/10.1017/S1368980024000132">frustration, stress, stigmatisation</a> and feelings of failure for the person trying to adhere to the diet. But the problem may be with the diet itself – not with you.</p> <h2>2. Is there evidence to support this diet?</h2> <p>Self-proclaimed “experts” online will often make claims focused on specific groups, known as <a href="https://doi.org/10.4103%2F0972-6748.77642">target populations</a>. This might be 30- to 50-year-old men with diabetes, for example.</p> <p>In some cases, evidence for claims made may come from animal studies, which might not be applicable to humans at all.</p> <p>So be aware that if research findings are for a group that doesn’t match your profile, then the results might not be relevant to you.</p> <p>It takes time and a lot of high-quality studies to tell us a “diet” is safe and effective, not just one study. Ask yourself, is it supported by multiple studies in humans? Be critical and question the claims before you accept them.</p> <p>For accurate information look for government websites, or ask your GP or dietitian.</p> <h2>3. How will this diet affect my life?</h2> <p>Food is much more than calories and nutrients. It plays many roles in our lives, and likewise diets can influence our lives in ways we often overlook.</p> <p>Socially and culturally, food can be a point of <a href="https://doi.org/10.1017/S1368980024000132">connection and celebration</a>. It can be a source of enjoyment, a <a href="https://doi.org/10.1016/j.jbusres.2018.12.024">source of</a> comfort, or even a way to explore new parts of the world.</p> <p>So when you’re considering a new diet, think about how it might affect meaningful moments for you. For example, if you’re going travelling, will your diet influence the food choices you make? Will you feel that you can’t sample the local cuisine? Or would you be deterred from going out for dinner with friends because of their choice of restaurant?</p> <h2>4. Will this diet make me feel guilty or affect my mental health?</h2> <p>What is your favourite meal? Does this diet “allow” you to eat it? Imagine visiting your mum who has prepared your favourite childhood meal. How will the diet affect your feelings about these special foods? Will it cause you to feel stressed or guilty about enjoying a birthday cake or a meal cooked by a loved one?</p> <p>Studies have shown that dieting can negatively impact our <a href="https://doi.org/10.1007%2FBF03405201">mental health</a>, and skipping meals can increase symptoms of <a href="https://doi.org/10.1016/j.jagp.2020.01.160">depression and anxiety</a>.</p> <p>Many diets fail to consider the psychological aspects of eating, even though our mental health is just as important as physical health. Eating should not make you feel stressed, anxious, or guilty.</p> <p>So before starting another diet, consider how it might affect your mental health.</p> <h2>Moving away from a dieting mindset</h2> <p>We’re frequently told that weight loss is the path to better health. Whereas, we can prioritise our health without focusing on our weight. Constant messages about the need to lose weight can also be harmful to mental health, and not necessarily helpful for <a href="https://doi.org/10.1152/ajpregu.00755.2010">physical health</a>.</p> <p>Our research has found eating in a way that prioritises health over weight loss is linked to a range of <a href="https://doi.org/10.1016/j.appet.2024.107361">positive outcomes</a> for our health and wellbeing. These include a more <a href="https://books.google.com.au/books?hl=en&amp;lr=&amp;id=IuZzDgAAQBAJ&amp;oi=fnd&amp;pg=PT9&amp;dq=%22relationship+with+food%22&amp;ots=e-NxLzUIFF&amp;sig=s7p6_SijfgZE6Odx7ztV4xXPGp8&amp;redir_esc=y#v=onepage&amp;q=%22relationship%20with%20food%22&amp;f=false">positive relationship</a> with food, and less guilt and stress.</p> <p>Our research also indicates <a href="https://doi.org/10.1016/j.appet.2024.107361">mindful and intuitive eating</a> practices – which focus on internal cues, body trust, and being present and mindful when eating – are related to lower levels of depression and stress, and greater body image and self-compassion.</p> <p>But like anything, it takes practice and time to build a positive relationship with food. Be kind to yourself, seek out <a href="https://www.sizeinclusivehealth.org.au/find-a-provider">weight-inclusive</a> health-care professionals, and the changes will come. Strength training has a range of benefits for women. Here are 4 ways to get into weights

<p><em><a href="https://theconversation.com/profiles/erin-kelly-1497598">Erin Kelly</a>, <a href="https://theconversation.com/institutions/university-of-canberra-865">University of Canberra</a></em></p> <p>Picture a gym ten years ago: the weights room was largely a male-dominated space, with women mostly doing cardio exercise. Fast-forward to today and you’re likely to see women of all ages and backgrounds confidently navigating weights equipment.</p> <p>This is more than just anecdotal. According to data from the <a href="https://app.powerbi.com/view?r=eyJrIjoiZGU1YWFhZDgtMmRhZi00YTgyLThhMzItYjc2ODk5NTg0MTg1IiwidCI6IjhkMmUwZjRjLTU1ZjItNGNiMS04ZWU3LWRhNWRkM2ZmMzYwMCJ9">Australian Sports Commission</a>, the number of women <a href="https://www.clearinghouseforsport.gov.au/research/ausplay/results#portal">participating in weightlifting</a> (either competitively or not) grew nearly five-fold between 2016 and 2022.</p> <p>Women are discovering what research has long shown: strength training offers benefits beyond sculpted muscles.</p> <h2>Health benefits</h2> <p><a href="https://www.womenshealth.gov/a-z-topics/osteoporosis">Osteoporosis</a>, a disease in which the bones become weak and brittle, affects more women than men. Strength training increases <a href="https://www.mayoclinic.org/healthy-lifestyle/fitness/in-depth/strength-training/art-20046670">bone density</a>, a crucial factor for <a href="https://bmcmedicine.biomedcentral.com/articles/10.1186/1741-7015-8-47">preventing osteoporosis</a>, especially for women negotiating menopause.</p> <p>Strength training also improves <a href="http://link.springer.com/10.1007/s40279-015-0379-7">insulin sensitivity</a>, which means your body gets better at using insulin to manage blood sugar levels, reducing the risk of type 2 diabetes. Regular strength training contributes to better <a href="https://doi.org/10.1139/apnm-2020-0245">heart health</a> too.</p> <p>There’s a mental health boost as well. Strength training has been linked to reduced symptoms of <a href="https://doi.org/10.1001/jamapsychiatry.2018.0572">depression</a> and <a href="http://link.springer.com/10.1007/s40279-017-0769-0">anxiety</a>.</p> <h2>Improved confidence and body image</h2> <p>Unlike some forms of exercise where progress can feel elusive, strength training offers clear and tangible measures of success. Each time you add more weight to a bar, you are reminded of your ability to meet your goals and <a href="https://www.tandfonline.com/doi/full/10.1080/2159676X.2019.1634127">conquer challenges</a>.</p> <p>This sense of achievement doesn’t just stay in the gym – it can change how women see themselves. A <a href="https://linkinghub.elsevier.com/retrieve/pii/S1755296623000194">recent study</a> found women who regularly lift weights often feel more empowered to make positive changes in their lives and feel ready to face life’s challenges outside the gym.</p> <p>Strength training also has the potential to positively impact <a href="https://journals.lww.com/nsca-jscr/abstract/2002/11000/Relations_of_Strength_Training_to_Body_Image_Among.25.aspx">body image</a>. In a world where women are often judged on appearance, lifting weights can shift the focus <a href="http://journals.sagepub.com/doi/10.1177/0193732502238256">to function</a>.</p> <p>Instead of worrying about the number on the scale or fitting into a certain dress size, women often come to appreciate their bodies for what they can do. “Am I lifting more than I could last month?” and “can I carry all my groceries in a single trip?” may become new measures of physical success.</p> <p>Lifting weights can also be about challenging outdated ideas of how women “should” be. Qualitative <a href="https://journals.humankinetics.com/view/journals/wspaj/aop/article-10.1123-wspaj.2022-0088/article-10.1123-wspaj.2022-0088.xml">research</a> I conducted with colleagues found that, for many women, strength training becomes a powerful form of rebellion against unrealistic beauty standards. As one participant told us:</p> <blockquote> <p>I wanted something that would allow me to train that just didn’t have anything to do with how I looked.</p> </blockquote> <p>Society has long told women to be small, quiet and not take up space. But when a woman steps up to a barbell, she’s pushing back against these outdated rules. One woman in our study said:</p> <blockquote> <p>We don’t have to […] look a certain way, or […] be scared that we can lift heavier weights than some men. Why should we?</p> </blockquote> <p>This shift in mindset helps women see themselves differently. Instead of worrying about being objects for others to look at, they begin to see their bodies as capable and strong. Another participant explained:</p> <blockquote> <p>Powerlifting changed my life. It made me see myself, or my body. My body wasn’t my value, it was the vehicle that I was in to execute whatever it was that I was executing in life.</p> </blockquote> <p>This newfound confidence often spills over into other areas of life. As one woman said:</p> <blockquote> <p>I love being a strong woman. It’s like going against the grain, and it empowers me. When I’m physically strong, everything in the world seems lighter.</p> </blockquote> <h2>Feeling inspired? Here’s how to get started</h2> <p><strong>1. Take things slow</strong></p> <p>Begin with bodyweight exercises like squats, lunges and push-ups to build a foundation of strength. Once you’re comfortable, add external weights, but keep them light at first. Focus on mastering <a href="https://theconversation.com/what-are-compound-exercises-and-why-are-they-good-for-you-228385">compound movements</a>, such as deadlifts, squats and overhead presses. These exercises engage multiple joints and muscle groups simultaneously, making your workouts more efficient.</p> <p><strong>2. Prioritise proper form</strong></p> <p>Always prioritise proper form over lifting heavier weights. Poor technique can lead to injuries, so learning the correct way to perform each exercise is crucial. To help with this, consider working with an exercise professional who can provide personalised guidance and ensure you’re performing exercises correctly, at least initially.</p> <p><strong>3. Consistency is key</strong></p> <p>Like any fitness regimen, consistency is key. Two to three sessions a week are plenty for most women to see benefits. And don’t be afraid to occupy space in the weights room – remember you belong there just as much as anyone else.</p> <p><strong>4. Find a community</strong></p> <p>Finally, join a community. There’s nothing like being surrounded by a group of strong women to inspire and motivate you. Engaging with a supportive community can make your strength-training journey more enjoyable and rewarding, whether it’s an in-person class or an online forum.</p> <h2>Are there any downsides?</h2> <p>Gym memberships can be expensive, especially for specialist weightlifting gyms. 4 ways to cut down on meat when dining out – and still make healthy choices

And one way to do this is by reducing the amount of meat we eat.</p> <p>That doesn’t mean you need to become a <a href="https://theconversation.com/why-you-should-eat-a-plant-based-diet-but-that-doesnt-mean-being-a-vegetarian-78470">vegan or vegetarian</a>. Our <a href="https://jn.nutrition.org/article/S0022-3166(24)00333-X/fulltext">recent research</a> shows even small changes to cut down on meat consumption could help improve health and wellbeing.</p> <p>But not all plant-based options are created equal and some are <a href="https://pubmed.ncbi.nlm.nih.gov/30744710/">ultra-processed</a>. Navigating what’s available when eating out – including options like tofu and fake meats – can be a challenge.</p> <p>So what are your best options at a cafe or restaurant? Here are some guiding principles to keep in mind when cutting down on meat.</p> <h2>Health benefits to cutting down</h2> <p>Small amounts of lean meat can be part of a healthy, balanced diet. But the majority of Australians <a href="https://cancer.org.au/about-us/policy-and-advocacy/prevention/obesity/related-resources/meat-and-cancer#consumption">still eat more meat</a> than recommended.</p> <p>Only a small percentage of Australians (10%) are vegetarian or vegan. But an <a href="https://www.foodfrontier.org/wp-content/uploads/2019/10/Food-Frontier-Hungry-For-Plant-Based-Australian-Consumer-Insights.pdf">increasing</a> number opt for a <a href="https://theconversation.com/love-meat-too-much-to-be-vegetarian-go-flexitarian-73741">flexitarian</a> diet. <a href="https://theconversation.com/whats-the-difference-between-vegan-and-vegetarian-225275">Flexitarians</a> eat a diet rich in fruits and vegetables, while still enjoying small amounts of meat, dairy, eggs and fish.</p> <p>Our <a href="https://jn.nutrition.org/article/S0022-3166(24)00333-X/fulltext">recent research</a> looked at whether the average Australian diet would improve if we swapped meat and dairy for plant-based alternatives, and the results were promising.</p> <p>The study found health benefits when people halved the amount of meat and dairy they ate and replaced them with healthy plant-based foods, like tofu or <a href="https://www.heartfoundation.org.au/blog/why-you-need-legumes-in-your-life">legumes</a>. On average, their dietary fibre intake – which helps with feeling fuller for longer and digestive health – went up. Saturated fats – which increase our blood cholesterol levels, a risk factor for heart disease – went down.</p> <p>Including more fibre and less saturated fat helps reduce the risk of <a href="https://www.heartfoundation.org.au/healthy-living/healthy-eating/healthy-eating-to-protect-your-heart">heart disease</a>.</p> <p>Achieving these health benefits may be as simple as swapping ham for baked beans in a toastie for lunch, or substituting half of the mince in your bolognese for lentils at dinner.</p> <h2>How it’s made matters</h2> <p>For a long time we’ve known processed meats – such as ham, bacon and sausages – are bad for your health. Eating high amounts of these foods is associated with poor <a href="https://www.heartfoundation.org.au/healthy-living/healthy-eating/protein-and-heart-health">heart health</a> and some forms of <a href="https://cancer.org.au/cancer-information/causes-and-prevention/diet-and-exercise/meat-and-cancer-risk">cancer</a>.</p> <p>But the same can be true of many processed meat alternatives.</p> <p>Plant-based alternatives designed to mimic meat, such as sausages and burgers, have become readily available in supermarkets, cafes and restaurants. These products are ultra-processed and can be <a href="https://theconversation.com/we-looked-at-700-plant-based-foods-to-see-how-healthy-they-really-are-heres-what-we-found-222991">high in salt and saturated fat</a>.</p> <p>Our study found when people replaced meat and dairy with ultra-processed meat alternatives – such as plant-based burgers or sausages – they ate more salt and less calcium, compared to eating meat or healthy plant-based options.</p> <p>So if you’re cutting down on meat for health reasons, it’s important to think about what you’re replacing it with. The <a href="https://www.eatforhealth.gov.au/sites/default/files/2022-09/n55g_adult_brochure.pdf">Australian Dietary Guidelines</a> recommend eggs, legumes/beans, tofu, nuts and seeds.</p> <p>Tofu can be a great option. But we recommend flavouring plain tofu with herbs and spices yourself, as pre-marinated products are often ultra-processed and can be high in salt.</p> <h2>What about when dining out?</h2> <p>When you’re making your own food, it’s easier to adapt recipes or reduce the amount of meat. But when faced with a menu, it can be difficult to work out what is the best option.</p> <p>Here are our four ways to make healthy choices when you eat out:</p> <p><strong>1. Fill half your plate with vegetables</strong></p> <p>When cutting down on meat, aim for half your plate to be vegetables. Try to also eat <a href="https://theconversation.com/were-told-to-eat-a-rainbow-of-fruit-and-vegetables-heres-what-each-colour-does-in-our-body-191337">a variety of colours</a>, such as leafy green spinach, red capsicum and pumpkin.</p> <p>When you’re out, this might look like choosing a vegetable-based entree, a stir-fry or ordering a side salad to have with your meal.</p> <p><strong>2. Avoid the deep fryer</strong></p> <p>The Australian Dietary Guidelines <a href="https://www.eatforhealth.gov.au/sites/default/files/files/the_guidelines/n55a_australian_dietary_guidelines_summary_130530.pdf">recommend limiting</a> deep fried foods to once a week or less. When dining out, choose plant-based options that are sautéed, grilled, baked, steamed, boiled or poached – instead of those that are crumbed or battered before deep frying.</p> <p>This could mean choosing vegetarian dumplings that are steamed not fried, or poached eggs at brunch instead of fried. Ordering a side of roast vegetables instead of hot chips is also a great option.</p> <p><strong>3. Pick wholegrains</strong></p> <p>Scan the menu for wholegrain options such as brown rice, wholemeal pizza or pasta, barley, quinoa or wholemeal burger buns. Not only are they good sources of protein, but they also provide more <a href="https://www.eatforhealth.gov.au/food-essentials/five-food-groups/grain-cereal-foods-mostly-wholegrain-and-or-high-cereal-fibre">dietary fibre</a> than refined grains, which help keep you fuller for longer.</p> <p><strong>4. If you do pick meat – choose less processed kinds</strong></p> <p>You may not always want, or be able, to make a vegetarian choice when eating out and with other people. If you do opt for meat, it’s better to steer clear of processed options like bacon or sausages.</p> <p>If sharing dishes with other people, you could try adding unprocessed plant-based options into the mix. For example, a curry with lentils or chickpeas, or a vegetable-based pizza instead of one with ham or salami. 4 things ancient Greeks and Romans got right about mental health

<p><em><a href="https://theconversation.com/profiles/konstantine-panegyres-1528527">Konstantine Panegyres</a>, <a href="https://theconversation.com/institutions/the-university-of-melbourne-722">The University of Melbourne</a></em></p> <p>According to the World Health Organization, about <a href="https://www.who.int/news-room/fact-sheets/detail/depression">280 million people</a> worldwide have depression and about <a href="https://www.who.int/health-topics/mental-health#tab=tab_2">one billion</a> have a mental health problem of any kind.</p> <p>People living in the ancient world also had mental health problems. So, how did they deal with them?</p> <p>As we’ll see, some of their insights about mental health are still relevant today, even though we might question some of their methods.</p> <h2>1. Our mental state is important</h2> <p>Mental health problems such as depression were familiar to people in the ancient world. Homer, the poet famous for the Iliad and Odyssey who lived around the eighth century BC, apparently <a href="https://www.loebclassics.com/view/LCL496/2003/volume.xml">died</a> after wasting away from depression.</p> <p>Already in the late fifth century BC, ancient Greek doctors recognised that our health partly depends on the state of our thoughts.</p> <p>In the Epidemics, a medical text written in around 400BC, an anonymous doctor <a href="https://www.loebclassics.com/view/LCL477/1994/volume.xml">wrote</a> that our habits about our thinking (as well as our lifestyle, clothing and housing, physical activity and sex) are the main determinants of our health.</p> <h2>2. Mental health problems can make us ill</h2> <p>Also writing in the Epidemics, an anonymous doctor <a href="https://www.loebclassics.com/view/LCL477/1994/volume.xml">described</a> one of his patients, Parmeniscus, whose mental state became so bad he grew delirious, and eventually could not speak. He stayed in bed for 14 days before he was cured. We’re not told how.</p> <p>Later, the famous doctor <a href="https://www.britannica.com/biography/Galen">Galen of Pergamum</a> (129-216AD) <a href="https://dfg-viewer.de/show?tx_dlf%5Bdouble%5D=0&amp;tx_dlf%5Bid%5D=http%3A%2F%2Fcmg.bbaw.de%2Fepubl%2Fonline%2Fmets%2Fsuppl_or_05_03.xml&amp;tx_dlf%5Bpage%5D=1390&amp;cHash=a0a715a587fa4e89a8839ccb310b0734">observed</a> that people often become sick because of a bad mental state:</p> <blockquote> <p>It may be that under certain circumstances ‘thinking’ is one of the causes that bring about health or disease because people who get angry about everything and become confused, distressed and frightened for the slightest reason often fall ill for this reason and have a hard time getting over these illnesses.</p> </blockquote> <p>Galen also described some of his patients who suffered with their mental health, including some who became seriously ill and died. <a href="https://dfg-viewer.de/show?tx_dlf%5Bdouble%5D=0&amp;tx_dlf%5Bid%5D=http%3A%2F%2Fcmg.bbaw.de%2Fepubl%2Fonline%2Fmets%2Fsuppl_or_05_03.xml&amp;tx_dlf%5Bpage%5D=1392&amp;cHash=c93bf32c31975103b100e918bee893d9">One man</a> had lost money:</p> <blockquote> <p>He developed a fever that stayed with him for a long time. In his sleep he scolded himself for his loss, regretted it and was agitated until he woke up. While he was awake he continued to waste away from grief. He then became delirious and developed brain fever. He finally fell into a delirium that was obvious from what he said, and he remained in this state until he died.</p> </blockquote> <h2>3. Mental illness can be prevented and treated</h2> <p>In the ancient world, people had many different ways to prevent or treat mental illness.</p> <p>The philosopher Aristippus, who lived in the fifth century BC, used to advise people <a href="https://www.loebclassics.com/view/LCL486/1997/volume.xml">to focus on the present</a> to avoid mental disturbance:</p> <blockquote> <p>concentrate one’s mind on the day, and indeed on that part of the day in which one is acting or thinking. Only the present belongs to us, not the past nor what is anticipated. The former has ceased to exist, and it is uncertain if the latter will exist.</p> </blockquote> <p>The philosopher Clinias, who lived in the fourth century BC, <a href="https://www.loebclassics.com/view/LCL486/1997/volume.xml">said</a> that whenever he realised he was becoming angry, he would go and play music on his lyre to calm himself.</p> <p>Doctors had their own approaches to dealing with mental health problems. Many <a href="https://books.google.com.au/books?id=KPHaAAAAMAAJ&amp;q=caelius+aurelianus+drabkin+on+acute+diseases&amp;dq=caelius+aurelianus+drabkin+on+acute+diseases&amp;hl=en&amp;newbks=1&amp;newbks_redir=0&amp;sa=X&amp;ved=2ahUKEwjpqszXwuSGAxUjamwGHS1yCuoQ6AF6BAgHEAI">recommended</a> patients change their lifestyles to adjust their mental states. They advised people to take up a new regime of exercise, adopt a different diet, go travelling by sea, listen to the lectures of philosophers, play games (such as draughts/checkers), and do mental exercises equivalent to the modern crossword or sudoku.</p> <p>For instance, the physician Caelius Aurelianus (fifth century AD) <a href="https://books.google.com.au/books?id=KPHaAAAAMAAJ&amp;q=caelius+aurelianus+drabkin+on+acute+diseases&amp;dq=caelius+aurelianus+drabkin+on+acute+diseases&amp;hl=en&amp;newbks=1&amp;newbks_redir=0&amp;sa=X&amp;ved=2ahUKEwjpqszXwuSGAxUjamwGHS1yCuoQ6AF6BAgHEAI">thought</a> patients suffering from insanity could benefit from a varied diet including fruit and mild wine.</p> <p>Doctors also advised people to take plant-based medications. For example, the herb <a href="https://www.psychiatriki-journal.gr/documents/psychiatry/30.1-EN-2019-58.pdf">hellebore</a> was given to people suffering from paranoia. However, ancient doctors recognised that hellebore could be dangerous as it sometimes induced toxic spasms, killing patients.</p> <p>Other doctors, such as Galen, had a slightly different view. He believed mental problems were caused by some idea that had taken hold of the mind. He believed mental problems could be cured if this idea was removed from the mind and <a href="https://dfg-viewer.de/show?tx_dlf%5Bdouble%5D=0&amp;tx_dlf%5Bid%5D=http%3A%2F%2Fcmg.bbaw.de%2Fepubl%2Fonline%2Fmets%2Fsuppl_or_05_03.xml&amp;tx_dlf%5Bpage%5D=1396&amp;cHash=1697e4b73dd653092cd8398749f1989f">wrote</a>:</p> <blockquote> <p>a person whose illness is caused by thinking is only cured by taking care of the false idea that has taken over his mind, not by foods, drinks, [clothing, housing], baths, walking and other such (measures).</p> </blockquote> <p>Galen <a href="https://dfg-viewer.de/show?id=9&amp;tx_dlf%5Bid%5D=http%3A%2F%2Fcmg.bbaw.de%2Fepubl%2Fonline%2Fmets%2Fsuppl_or_05_03.xml&amp;tx_dlf%5Bpage%5D=1418">thought</a> it was best to deflect his patients’ thoughts away from these false ideas by putting new ideas and emotions in their minds:</p> <blockquote> <p>I put fear of losing money, political intrigue, drinking poison or other such things in the hearts of others to deflect their thoughts to these things […] In others one should arouse indignation about an injustice, love of rivalry, and the desire to beat others depending on each person’s interest.</p> </blockquote> <h2>4. Addressing mental health needs effort</h2> <p>Generally speaking, the ancients believed keeping our mental state healthy required effort. If we were anxious or angry or despondent, then we needed to do something that brought us the opposite of those emotions.</p> <p>This can be achieved, they thought, by doing some activity that directly countered the emotions we are experiencing.</p> <p>For example, Caelius Aurelianus <a href="https://books.google.com.au/books?id=KPHaAAAAMAAJ&amp;q=caelius+aurelianus+drabkin+on+acute+diseases&amp;dq=caelius+aurelianus+drabkin+on+acute+diseases&amp;hl=en&amp;newbks=1&amp;newbks_redir=0&amp;sa=X&amp;ved=2ahUKEwjpqszXwuSGAxUjamwGHS1yCuoQ6AF6BAgHEAI">said</a> people suffering from depression should do activities that caused them to laugh and be happy, such as going to see a comedy at the theatre.</p> <p>However, the ancients did not believe any single activity was enough to make our mental state become healthy. The important thing was to make a wholesale change to one’s way of living and thinking.</p> <p>When it comes to experiencing mental health problems, we clearly have a lot in common with our ancient ancestors. Ovarian cancer is hard to detect. Focusing on these 4 symptoms can help with diagnosis

<p><em><a href="https://theconversation.com/profiles/jenny-doust-12412">Jenny Doust</a>, <a href="https://theconversation.com/institutions/the-university-of-queensland-805">The University of Queensland</a></em></p> <p>Ovarian cancers are often found when they are already advanced and hard to treat.</p> <p>Researchers <a href="https://ascopubs.org/doi/10.1200/JCO.2010.32.2164">have long believed</a> this was because women first experienced symptoms when ovarian cancer was already well-established. Symptoms can also be <a href="https://cancer.org.au/cancer-information/causes-and-prevention/early-detection-and-screening/early-detection-of-ovarian-cancer">hard to identify</a> as they’re vague and similar to other conditions.</p> <p>But <a href="https://ijgc.bmj.com/content/early/2024/07/24/ijgc-2024-005371">a new study</a> shows promising signs ovarian cancer can be detected in its early stages. The study targeted women with four specific symptoms – bloating, abdominal pain, needing to pee frequently, and feeling full quickly – and put them on a fast track to see a specialist.</p> <p>As a result, even the most aggressive forms of ovarian cancer could be detected in their early stages.</p> <p>So what did the study find? And what could it mean for detecting – and treating – ovarian cancer more quickly?</p> <h2>Why is ovarian cancer hard to detect early?</h2> <p>Ovarian cancer <a href="https://pubmed.ncbi.nlm.nih.gov/24979449/">cannot be detected</a> via cervical cancer screening (which used to be called a pap smear) and pelvic exams <a href="https://pubmed.ncbi.nlm.nih.gov/24979449/">aren’t useful</a> as a screening test.</p> <p>Current <a href="https://www.canceraustralia.gov.au/cancer-types/gynaecological-cancers/clinicians-hub/ovarian-cancer-guidelines">Australian guidelines</a> recommend women get tested for ovarian cancer if they have symptoms for <a href="https://www.canceraustralia.gov.au/sites/default/files/publications/assessment-symptoms-may-be-ovarian-cancer-guide-gps/pdf/ocg_assessment_of_symptoms_gp_card_0.pdf">more than a month</a>. But many of the <a href="https://cancer.org.au/cancer-information/types-of-cancer/ovarian-cancer">symptoms</a> – such as tiredness, constipation and changes in menstruation – are vague and overlap with other common illnesses.</p> <p>This makes early detection a challenge. But it is crucial – a woman’s <a href="https://seer.cancer.gov/statfacts/html/ovary.html">chances of surviving ovarian cancer</a> are associated with how advanced the cancer is when she is diagnosed.</p> <p>If the cancer is still confined to the original site with no spread, the five-year survival rate is 92%. But over half of women diagnosed with ovarian cancer first present when the cancer has <a href="https://seer.cancer.gov/statfacts/html/ovary.html">already metastatised</a>, meaning it has spread to other parts of the body.</p> <p>If the cancer has spread to nearby lymph nodes, the survival rate is reduced to 72%. If the cancer has already metastasised and spread to distant sites at the time of diagnosis, the rate is only 31%.</p> <p>There are mixed findings on whether detecting ovarian cancer earlier leads to better survival rates. For example, a trial in the UK that screened more than 200,000 women <a href="https://pubmed.ncbi.nlm.nih.gov/37183782/">failed to reduce deaths</a>.</p> <p>That study screened the general public, rather than relying on self-reported symptoms. The new study suggests asking women to look for specific symptoms can lead to earlier diagnosis, meaning treatment can start more quickly.</p> <h2>What did the new study look at?</h2> <p>Between June 2015 and July 2022, the researchers recruited 2,596 women aged between 16 and 90 from 24 hospitals across the UK.</p> <p>They were asked to monitor for these four symptoms:</p> <ul> <li>persistent abdominal distension (women often refer to this as bloating)</li> <li>feeling full shortly after starting to eat and/or loss of appetite</li> <li>pelvic or abdominal pain (which can feel like indigestion)</li> <li>needing to urinate urgently or more often.</li> </ul> <p>Women who reported at least one of four symptoms persistently or frequently were put on a <a href="https://pubmed.ncbi.nlm.nih.gov/22479719/">fast-track pathway</a>. That means they were sent to see a gynaecologist within two weeks. The fast track pathway has been used in the UK since 2011, but is not specifically part of Australia’s guidelines.</p> <p>Some 1,741 participants were put on this fast track. First, they did a blood test that measured the cancer antigen 125 (CA125). If a woman’s CA125 level was abnormal, she was sent to do a internal vaginal ultrasound.</p> <h2>What did they find?</h2> <p>The study indicates this process is better at detecting ovarian cancer than general screening of people who don’t have symptoms. Some 12% of women on the fast-track pathway were diagnosed with some kind of ovarian cancer.</p> <p>A total of 6.8% of fast-tracked patients were diagnosed with high-grade serous ovarian cancer. It is the most aggressive form of cancer and responsible for 90% of ovarian cancer deaths.</p> <p>Out of those women with the most aggressive form, one in four were diagnosed when the cancer was still in its early stages. That is important because it allowed treatment of the most lethal cancer before it had spread significantly through the body.</p> <p>There were some promising signs in treating those with this aggressive form. The majority (95%) had surgery and three quarters (77%) had chemotherapy. Complete cytoreduction – meaning all of the cancer appears to have been removed – was achieved in six women out of ten (61%).</p> <p>It’s a promising sign that there may be ways to “catch” and target ovarian cancer before it is well-established in the body.</p> <h2>What does this mean for detection?</h2> <p>The study’s findings suggest this method of early testing and referral for the symptoms leads to earlier detection of ovarian cancer. This may also improve outcomes, although the study did not track survival rates.</p> <p>It also points to the importance of public awareness about symptoms.</p> <p>Clinicians should be able to recognise all of the ways ovarian cancer can present, including vague symptoms like general fatigue.</p> <p>But empowering members of the general public to recognise a narrower set of four symptoms can help trigger testing, detection and treatment of ovarian cancer earlier than we thought.</p> <p>This could also save GPs advising every woman who has general tiredness or constipation to undergo an ovarian cancer test, making testing and treatment more targeted and efficient.</p> <p>Many women remain <a href="https://www.google.com/url?q=https://pubmed.ncbi.nlm.nih.gov/24612526/&amp;sa=D&amp;source=docs&amp;ust=1723610085733806&amp;usg=AOvVaw3J5o5DwRFiDzFaQcD7VTQn">unaware of the symptoms</a> of ovarian cancer. We reviewed the health habits of centenarians. These 4 things could lead to a longer life

<p><em><a href="https://theconversation.com/profiles/zhaoli-dai-keller-1547476">Zhaoli Dai-Keller</a>, <a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a> and <a href="https://theconversation.com/profiles/perminder-sachdev-46869">Perminder Sachdev</a>, <a href="https://theconversation.com/institutions/unsw-sydney-1414">UNSW Sydney</a></em></p> <p>The number of centenarians around the world rose from <a href="https://population.un.org/wpp/Download/Standard/Population/">151,000 in 2000</a> to 573,000 in 2021. People are living longer, and we can expect to see more people reach 100 in the years to come.</p> <p>Centenarians <a href="https://academic.oup.com/biomedgerontology/article/67A/4/395/623695">exemplify successful ageing</a>, often experiencing <a href="https://agmr.hapres.com/htmls/AGMR_1264_Detail.html">fewer chronic diseases</a> and maintaining independence in daily life well into their 90s. While <a href="https://link.springer.com/article/10.1186/s12979-016-0066-z">genetics contribute</a> to longevity, modifiable factors account for more than 60% <a href="https://pubmed.ncbi.nlm.nih.gov/14624729/">of successful aging</a>.</p> <p>But what sort of factors specifically contribute to living to 100? To find out, we reviewed lifestyle and health habits of centenarians and near-centenarians (those aged 95–99) worldwide.</p> <p>Our <a href="https://link.springer.com/article/10.1007/s11357-024-01247-4">recent review</a> included 34 observational studies published since 2000. Here are four key factors we found contribute to extreme longevity.</p> <h2>1. A diverse diet with controlled salt intake</h2> <p>Centenarians and near-centenarians typically had a balanced and diverse diet. We found, on average, they consumed between 57% and 65% of their energy intake from carbohydrates, 12% to 32% from protein, and 27% to 31% from fat. Their diets included staple foods (such as rice and wheat), fruits, vegetables, and protein-rich foods like poultry, fish and legumes, with moderate red meat consumption.</p> <p>This dietary pattern, similar to the <a href="https://pubmed.ncbi.nlm.nih.gov/29852087/">Mediterranean diet</a>, is linked to lower risks of <a href="https://doi.org/10.3390/ijerph14111364">physical function impairment</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/31685303/">death</a>.</p> <p>Most centenarians also preferred a low-salt diet. While only one study in our review measured the mean daily sodium intake, finding 1.6 grams, this was within the <a href="https://www.who.int/data/gho/indicator-metadata-registry/imr-details/3082#:%7E:text=A%20salt%20intake%20of%20less,much%20more%20salt%20than%20recommended">World Health Organization’s</a> recommendation of less than 2g of sodium per day (equivalent to about 5g of salt). The <a href="https://pubmed.ncbi.nlm.nih.gov/20234038/">traditional Okinawan diet</a>, known for its consumption by Japanese centenarians on Okinawa Island, contains an estimated 1.1g of sodium.</p> <p>We found higher salt intake (those who preferred salty food or added extra salt to meals) had a 3.6-fold increased risk of physical function impairment compared to those without a preference for salt.</p> <p>Practically, these findings suggest we should include plenty of wholegrains, root vegetables, beans, legumes, fruits and vegetables in our diet, minimise red meat consumption and opt for lean poultry, fish, and plant-based protein, and monitor salt in our food.</p> <h2>2. Lower medication use</h2> <p>Centenarians are not free from chronic conditions but typically develop them much later than average adults. More than half of the people in our review experienced common issues such as hypertension (high blood pressure), dementia, or cognitive impairment.</p> <p>We found people in our review took an average of 4.6 medications. The most frequently used medications included blood pressure medications and drugs for heart disease. This is similar to the results of <a href="https://doi.org/10.3390/jcm9051563">a large health register-based study</a> in Spain, which found centenarians took an average of 4.9 medications. Non-centenarians in this study took 6.7 medications on average.</p> <p>The fact centenarians appear to take fewer medications may indicate better health with fewer medical conditions. However, data on medication use is often self-reported and so may not be entirely accurate, especially among those with cognitive impairment.</p> <p><a href="https://www.thelancet.com/journals/lanhl/article/PIIS2666-7568(24)00007-2/fulltext">Polypharmacy</a> is often defined as taking five or more medications simultaneously, and is common in older adults. Inappropriate polypharmacy is associated with <a href="https://www.nia.nih.gov/news/dangers-polypharmacy-and-case-deprescribing-older-adults">increased risks</a> of adverse events such as falls, cognitive impairment and hospitalisation, due to harmful drug interactions.</p> <p>While the type or number of prescribed medications may not be within a patient’s control, it’s important for doctors to <a href="https://link.springer.com/article/10.1186/s12875-017-0642-0">prescribe medications</a> only when necessary, fully inform patients about benefits and risks, and regularly review treatment plans.</p> <h2>3. Getting good sleep</h2> <p>Sleep quality and quantity <a href="https://newsinhealth.nih.gov/2013/04/benefits-slumber">affect</a> the immune system, stress hormones, and cardiometabolic functions such as obesity, high blood pressure, and diabetes. Good <a href="https://academic.oup.com/biomedgerontology/article/74/2/204/4837199">sleep</a> is associated with extended years of good health and reduced risks of chronic diseases.</p> <p>In our review, 68% of the centenarians were satisfied with their sleep quality. In a survey of adults’ sleep satisfaction in 13 countries in 2020, sleep satisfaction ranged from <a href="https://www.statista.com/statistics/1233775/adults-worldwide-satisfied-sleep-country/">29% to 67%</a>.</p> <p>The <a href="https://academic.oup.com/biomedgerontology/article/74/2/204/4837199">optimal sleep duration</a> is between seven and eight hours per night. Tips to <a href="https://www.mayoclinic.org/healthy-lifestyle/adult-health/in-depth/sleep/art-20048379">achieving better sleep</a> include keeping a regular sleep routine, creating a restful environment, exercising regularly and managing stress.</p> <h2>4. Living environment</h2> <p>More than 75% of the centenarians and near-centenarians in our review lived in rural areas. This is a pattern reflected in “<a href="https://www.bluezones.com/">blue zones</a>”, areas known for high concentrations of centenarians, such as Okinawa in Japan, Sardinia in Italy, the Nicoya Peninsula in Costa Rica, and Ikaria in Greece.</p> <p>This may be partly related to the connection between nature and health and wellbeing. For example, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6562165/">exposure to green space</a> has been associated with lower stress, depression, blood pressure, type 2 diabetes and heart disease, potentially increasing <a href="https://doi.org/10.1016/j.envint.2023.107785">life expectancy</a>.</p> <h2>Other important factors</h2> <p>We didn’t look at all lifestyle factors associated with longevity in our review. Research also shows <a href="https://www.sciencedirect.com/science/article/abs/pii/S053155650700143X">not smoking</a>, <a href="https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2802963">avoiding alcohol</a> or drinking moderately, staying <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3618983/">physically active</a> and maintaining <a href="https://doi.org/10.1098/rspb.2019.1991">social connections</a> are important for boosting a person’s chances of living to 100.</p> <p>Of course, adopting the lifestyle changes discussed in this article won’t guarantee you’ll reach the ripe old age of 100. And on the flip side, some centenarians have shared <a href="https://theconversation.com/why-you-should-never-take-nutrition-advice-from-a-centenarian-229159">questionable</a> health habits.</p> <p>But many older adults are seeking to adopt <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9092121/">healthier lifestyles</a> to prevent and manage chronic conditions, while health-care professionals are similarly recognising the value of <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5638636/">lifestyle medicine</a>.</p> <p>The earlier you can adopt positive lifestyle changes and healthier habits, the better placed you’ll be to achieve a long and healthy life. The skin around our eyes and on our eyelids is the most sensitive and least elastic on our face and the most vulnerable... so keep your fingers away.</p> <p><strong>Skimping on sunglasses</strong></p> <p>As well as being a fashionable accessory, sunglasses also do wonders to minimize lines around your eyes. Shading your eyes from the sun’s glare prevents squinting and crow's feet wrinkles, of course, but it also shields delicate skin from the destructive onslaught of UV rays. Make sure you opt for a pair with UV protection.</p> <p><strong>Neglecting your neck, chest and hands</strong></p> <p>The delicate skin of these areas lack the oil glands of other areas of skin, which results in dryness and accelerated aging. Plus, these areas are often fraught with sunscreen neglect. 4 things you’re likely doing that are damaging your hearing

<p>Your hearing is a precious gift, so it’s important to take good care of your ears. We’ve got some advice on the things to avoid, and what you can do to protect your hearing.</p> <p><strong>Using cotton tips</strong></p> <p>Though they’re commonly used for the job, cotton tips should never be used to clean out your ears. In fact, no solid object should be put inside your ears. Cotton tips account for around four per cent of all ruptured eardrums. </p> <p>These innocuous-looking objects can also cause bleeding, scratch your ear canal, or trigger an infection. So put them down and step away. Instead, use a commercial ear cleaner, or even just a few drops of mineral oil, baby oil, or glycerine to soften the earwax.</p> <p><strong>Don’t wait and see</strong></p> <p>If your hearing in one or both ears suddenly disappears without cause, you need to go and see your doctor as soon as you can. It could be inflammation, infection, or a decrease in blood supply to the area. </p> <p>If you need treatment, you’ll likely need it immediately to have any hope of restoring your hearing.</p> <p><strong>Using eardrops without advice</strong></p> <p>There are many over-the-counter eardrops available to help with things like swimmer’s ear. But in extreme cases, these products can cause deafness. Before you use anything, including a home remedy, get your doctor to check that you don’t have a ruptured eardrum. </p> <p>Some people may be born that way, or have had surgery as a child, or suffered an injury. If the ingredients in these drops make their way inside your eardrum, it can cause a lot of pain, and permanent deafness.</p> <p><strong>Always protect your ears</strong></p> <p>It’s incredibly important to protect your ears from permanent damage. Tiny hairs inside your ears act as hearing receptors, and these can be broken by extremely loud noises. Once they’re gone, they don’t come back. </p> <p>So things like loud music, fireworks, machinery, and artillery are all risky to be around. The best thing you can do is cover up with earmuffs whenever you’re around these things – especially if it’s on a regular basis. Want to reduce your dementia risk? Eat these 4 foods, says new study

<p>If you are what you eat, this might make you hungrier for foods that are bright in every sense. Research has shown that living, vibrant foods can slow down aging at a cellular level; while fruit and vegetables in particular have been associated with lower incidence of cognitive decline as individuals age.</p> <p>However, research has been relatively lacking on just how much of these brain-healthy foods you really need and which fruit and vegetables are best for the job.</p> <p>In collaboration with public health experts at Harvard University, medical researchers at China’s Zhejiang University School of Medicine conducted a meta-analysis that’s slated to be published in the June 2024 issue of <em>The Journal of Nutrition, Health, and Aging</em>. They combined data from two large-scale population-representative studies that analysed the diets and cognitive function of more than 10,000 participants ages 55 and older from China and the US.</p> <h2>What daily diets revealed</h2> <p>The data included diet questionnaires that honed in on the average of participants’ total daily intake of several different types of foods, including fruit and vegetables, and also broke them down into sub-types like green leafy vegetables and berries. Over a period of five years, the participants also took part in activities designed to assess their cognitive function and the average rate of cognitive decline.</p> <p>Overall, participants who included the most fruit and vegetables in their daily diets performed best on the brain tests and maintained those results over time. This suggested that both fruit and vegetables had protective elements that slowed cognitive decline.</p> <h2>Vegetables that help protect cognition</h2> <p>Interestingly, certain types of vegetables appeared to be more beneficial than others—say the researchers: “Our findings support the potential beneficial roles of VF, especially cruciferous vegetables, green leafy vegetables, and red and yellow vegetables, in maintaining cognitive function and slowing cognitive decline in middle-aged and older adults.”</p> <p>The researchers pointed to several reasons these particular vegetables might have shown a substantial impact, including anti-inflammatory and antioxidation nutrients like flavonoids and various vitamins or even gut improvements that have been shown to help improve or protect cognition.</p> <p>While beans didn’t figure prominently in both studies, they showed a protective element in the US study, so they are also worth keeping on your plate. (Beans are also thought to be one of the top foods for longevity.)</p> <h2>Fruit that pack a punch</h2> <p>As for fruit, while some didn’t show as much of a protective effect across the board, berries and apples are two examples of fruit that experts have previously said provide major polyphenol and antioxidant effect.</p> <p>Participants whose brains maintained performance were shown to have eaten three or more servings of vegetables and two or more servings of fruit per day. 4 ways to avoid foot pain when travelling

<p>Whether it’s caused by a hectic day of sightseeing or a mad rush through the airports, there’s nothing quite as annoying as foot pain when you’re on holidays. And when you consider how easy it is to avoid (so long as you take the correct preventative measures) you’ll feeling like kicking yourself for putting up with it for all these years.</p> <p>Here are four ways to avoid foot pain when travelling.</p> <p><strong>1. Choosing the right pair of shoes  </strong></p> <p>Out of all the fashion statements, shoes are probably responsible for more chronic foot pain than anything else. So make sure you choose the right pair of shoes for your trip. For example, if you’re going to be walking around all day sightseeing it might be an idea to ditch the stiletto heels for a pair of joggers (even if they’re not quite so aesthetically pleasing).</p> <p>Dr Robert Mathews from Cremorne Medical in NSW says, “I recommend wearing supportive shoe such as running shoes. If you want to wear something more stylish then consider buying some gel insoles to slip in your shoes, you can get a wide variety of these from your local chemist.“</p> <p><strong>2. Manage your feet on flights</strong></p> <p>Foot swelling can become quite a big problem on long haul flight, so managing your feet becomes crucial. Simple, preventative measures anyone can take, like wearing support stocks, standing up every so often to move around or even just flexing your feet and wriggling your toes, can make a big difference and greatly reduce the chance of swelling.</p> <p><strong>3. Slip, slop and slap</strong></p> <p>So many island holidays have been soured by the blistering pain of sunburnt feet. If you’re staying at a resort or near a beach and your feet are exposed, don’t forget to apply sunscreen everywhere. Otherwise you’re going to want to have some aloe vera gel handy!</p> <p><strong>4. Take time to rest</strong></p> <p>While you’re probably in a mad rush to see everything, fear of missing out can put significant strain on your feet. So make sure you set aside plenty of time every day to put your feet up and rest. It also might be worth considering some extra pampering, like a foot bath or even a half hour massage. 4 foods that ease heartburn

<p>Heartburn, acid reflux, GORD… whatever you want to call it, it’s not a pleasant thing to experience. Yet hundreds of thousands of us around the country suffer from heartburn each year. And while medication may be the solution for some, it’s not always the most effective option.</p> <p>So, you’ll be happy to learn that what you eat may ease your symptoms. Here are four of the best foods for heartburn.</p> <p><strong>1. Papaya</strong></p> <p>Papain, an enzyme found in papaya, has been proven to aid in digestion. In addition, the fibre content and proteolytic enzymes are a great source of short-chain fatty acids, which a recent study described as “the most important product of fermentation”, and an essential part of improving gut health.</p> <p><strong>2. Aloe vera juice</strong></p> <p>Aloe vera does much more than just ease sunburn or skin irritations – it’s been used to help ease constipation and treat type 2 diabetes and gastrointestinal disorders for centuries. “Its anti-inflammatory properties have been suggested to ease inflammation in the oesophagus caused by reflux,” Maria Bella, author of <em>The Complete Idiot’s Guide to the Acid Reflux Diet</em>, tells <a href="http://www.goodhousekeeping.com/health/diet-nutrition/g4536/best-foods-for-acid-reflux/" target="_blank" rel="noopener"><strong><span style="text-decoration: underline;">Good Housekeeping</span></strong></a>.</p> <p><strong>3. Banana</strong></p> <p>Because of its low acidity, bananas are great for gastrointestinal discomfort, but they’re particularly good for heartburn as they can stick to the irritated oesophageal lining. “It forms a protective film that coats, protects and soothes,” digestive health expert Dr Gerard E. Mullin explains to <a href="http://www.prevention.com/food/foods-soothe-heartburn" target="_blank" rel="noopener"><strong><span style="text-decoration: underline;">Prevention</span></strong></a>.</p> <p><strong>4. Cinnamon gum</strong></p> <p>By producing saliva, gum helps neutralise stomach acid. “It also leads to more frequent swallowing, which can move the irritating acid down the oesophagus more quickly,” Maria Bella says. Yes, adults can develop food allergies. Here are 4 types you need to know about

<p><em><a href="https://theconversation.com/profiles/clare-collins-7316">Clare Collins</a>, <a href="https://theconversation.com/institutions/university-of-newcastle-1060">University of Newcastle</a></em></p> <p>If you didn’t have food allergies as a child, is it possible to develop them as an adult? The short answer is yes. But the reasons why are much more complicated.</p> <p>Preschoolers are about <a href="https://pubmed.ncbi.nlm.nih.gov/25316115/">four times more likely to have a food allergy</a> than adults and are more likely to grow out of it as they get older.</p> <p>It’s hard to get accurate figures on adult food allergy prevalence. The Australian National Allergy Council reports <a href="https://nationalallergycouncil.org.au/about-us/our-strategy">one in 50 adults</a> have food allergies. But a US survey suggested as many as <a href="https://pubmed.ncbi.nlm.nih.gov/30646188/">one in ten adults</a> were allergic to at least one food, with some developing allergies in adulthood.</p> <h2>What is a food allergy</h2> <p><a href="https://pubmed.ncbi.nlm.nih.gov/36509408/">Food allergies</a> are immune reactions involving <a href="https://www.aaaai.org/tools-for-the-public/allergy,-asthma-immunology-glossary/immunoglobulin-e-(ige)-defined">immunoglobulin E (IgE)</a> – an antibody that’s central to triggering allergic responses. These are known as “IgE-mediated food allergies”.</p> <p>Food allergy symptoms that are <em>not</em> mediated by IgE are usually delayed reactions and called <a href="https://pubmed.ncbi.nlm.nih.gov/25316115/">food intolerances or hypersensitivity</a>.</p> <p>Food allergy symptoms can include hives, swelling, difficulty swallowing, vomiting, throat or chest tightening, trouble breathing, chest pain, rapid heart rate, dizziness, low blood pressure or <a href="https://www.allergy.org.au/hp/papers/acute-management-of-anaphylaxis-guidelines?highlight=WyJhbmFwaHlsYXhpcyJd">anaphylaxis</a>.</p> <p>IgE-mediated food allergies can be life threatening, so all adults need an <a href="https://allergyfacts.org.au/allergy-management/newly-diagnosed/action-plan-essentials">action management plan</a> developed in consultation with their medical team.</p> <p>Here are four IgE-mediated food allergies that can occur in adults – from relatively common ones to rare allergies you’ve probably never heard of.</p> <h2>1. Single food allergies</h2> <p>The most <a href="https://pubmed.ncbi.nlm.nih.gov/30646188/">common IgE-mediated food allergies</a> in adults in a US survey were to:</p> <ul> <li>shellfish (2.9%)</li> <li>cow’s milk (1.9%)</li> <li>peanut (1.8%)</li> <li>tree nuts (1.2%)</li> <li>fin fish (0.9%) like barramundi, snapper, salmon, cod and perch.</li> </ul> <p>In these adults, about 45% reported reacting to multiple foods.</p> <p>This compares to <a href="https://pubmed.ncbi.nlm.nih.gov/25316115/">most common childhood food allergies</a>: cow’s milk, egg, peanut and soy.</p> <p>Overall, adult food allergy prevalence appears to be increasing. Compared to <a href="https://pubmed.ncbi.nlm.nih.gov/14657884/">older surveys published in 2003</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/15241360/">2004</a>, peanut allergy prevalence has increased about three-fold (from 0.6%), while tree nuts and fin fish roughly doubled (from 0.5% each), with shellfish similar (2.5%).</p> <p>While new <a href="https://pubmed.ncbi.nlm.nih.gov/38214821/">adult-onset food allergies are increasing</a>, childhood-onset food allergies are also more likely to be retained into adulthood. Possible reasons for both <a href="https://pubmed.ncbi.nlm.nih.gov/38214821/">include</a> low vitamin D status, lack of immune system challenges due to being overly “clean”, heightened sensitisation due to allergen avoidance, and more frequent antibiotic use.</p> <h2>2. Tick-meat allergy</h2> <p>Tick-meat allergy, also called α-Gal syndrome or mammalian meat allergy, is an allergic reaction to galactose-alpha-1,3-galactose, or α-Gal for short.</p> <p><a href="https://pubmed.ncbi.nlm.nih.gov/33529984/">Australian immunologists first reported</a> links between α-Gal syndrome and tick bites in 2009, with cases also reported in the United States, Japan, Europe and South Africa. The <a href="https://pubmed.ncbi.nlm.nih.gov/38318181/">US Centers for Disease Control estimates</a> about 450,000 Americans <a href="https://www.cdc.gov/mmwr/volumes/72/wr/mm7230a2.htm">could be affected</a>.</p> <p>The α-Gal contains a carbohydrate molecule that is bound to a <a href="https://pubmed.ncbi.nlm.nih.gov/38318181/">protein</a> molecule in <a href="https://alphagalinformation.org/what-is-a-mammal/">mammals</a>.</p> <p>The IgE-mediated allergy is triggered after repeated bites from ticks or <a href="https://www.insectshield.com/pages/chiggers">chigger mites</a> that have bitten those mammals. When tick saliva crosses into your body through the bite, antibodies to α-Gal are produced.</p> <p>When you subsequently eat foods that contain α-Gal, the allergy is triggered. These triggering foods include meat (lamb, beef, pork, rabbit, kangaroo), dairy products (yoghurt, cheese, ice-cream, cream), <a href="https://en.wikipedia.org/wiki/Gelatin">animal-origin gelatin</a> added to gummy foods (jelly, lollies, marshmallow), prescription medications and over-the counter supplements containing gelatin (<a href="https://www.drugs.com/inactive/gelatin-57.html">some antibiotics, vitamins and other supplements</a>).</p> <p>Tick-meat allergy reactions can be hard to recognise because they’re usually delayed, and they can be severe and include anaphylaxis. Allergy <a href="https://www.allergy.org.au/patients/food-allergy/mammalian-meat-tick-faq">organisations produce management guidelines</a>, so always discuss management with your doctor.</p> <h2>3. Fruit-pollen allergy</h2> <p>Fruit-pollen allergy, called pollen food allergy syndrome, is an <a href="https://pubmed.ncbi.nlm.nih.gov/38002141/">IgE-mediated allergic reaction</a>.</p> <p>In susceptible adults, pollen in the air provokes the production of IgE antibodies to antigens in the pollen, but these antigens are similar to ones found in some fruits, vegetables and herbs. The problem is that <a href="https://pubmed.ncbi.nlm.nih.gov/38002141/">eating those plants</a> triggers an allergic reaction.</p> <p>The <a href="https://pubmed.ncbi.nlm.nih.gov/38002141/">most allergenic tree pollens</a> are from birch, cypress, Japanese cedar, <a href="https://www.betterhealth.vic.gov.au/health/conditionsandtreatments/latex-allergy">latex</a>, grass, and ragweed. Their pollen can cross-react with <a href="https://pubmed.ncbi.nlm.nih.gov/38002141/">fruit and vegetables</a>, including kiwi, banana, mango, avocado, grapes, celery, carrot and potato, and some herbs such as caraway, coriander, fennel, pepper and paprika.</p> <p>Fruit-pollen allergy is not common. Prevalence <a href="https://pubmed.ncbi.nlm.nih.gov/38002141/">estimates are between 0.03% and 8%</a> depending on the country, but it can be life-threatening. Reactions range from itching or tingling of lips, mouth, tongue and throat, called <a href="https://pubmed.ncbi.nlm.nih.gov/20306812/">oral allergy syndrome</a>, to mild <a href="https://www.allergy.org.au/patients/skin-allergy/urticaria-hives">hives</a>, to anaphylaxis.</p> <h2>4. Food-dependent, exercise-induced food allergy</h2> <p>During heavy exercise, the stomach produces less acid than usual and gut permeability increases, meaning that small molecules in your gut are more likely to escape across the membrane into your blood. These include food molecules that trigger an IgE reaction.</p> <p>If the person already has IgE antibodies to the foods eaten before exercise, then the risk of triggering food allergy reactions is increased. This allergy is called <a href="https://pubmed.ncbi.nlm.nih.gov/37893663/">food-dependent exercise-induced allergy</a>, with symptoms ranging from hives and swelling, to difficulty breathing and anaphylaxis.</p> <p><a href="https://pubmed.ncbi.nlm.nih.gov/30601082/">Common trigger foods include</a> wheat, seafood, meat, poultry, egg, milk, nuts, grapes, celery and other foods, which could have been eaten many hours before exercising.</p> <p>To complicate things even further, allergic <a href="https://pubmed.ncbi.nlm.nih.gov/33181008/">reactions can</a> occur at lower levels of trigger-food exposure, and be more severe if the person is simultaneously taking non-steroidal inflammatory medications like aspirin, drinking alcohol or is sleep-deprived.</p> <p>Food-dependent exercise-induced allergy is extremely rare. Surveys have estimated prevalence as between <a href="https://www.sciencedirect.com/science/article/pii/S1555415517300259">one to 17 cases per 1,000 people worldwide</a> with the highest prevalence between the teenage years to age 35. Those affected often have other allergic conditions such as hay fever, asthma, allergic conjunctivitis and dermatitis.</p> <h2>Allergies are a growing burden</h2> <p>The <a href="https://pubmed.ncbi.nlm.nih.gov/36509408/">burden on physical health, psychological health</a> and health costs due to food allergy is increasing. In the US, this <a href="https://pubmed.ncbi.nlm.nih.gov/38393624/">financial burden was estimated as $24 billion per year</a>.</p> <p>Adult food allergy needs to be taken seriously and those with severe symptoms should wear a medical information bracelet or chain and carry an <a href="https://www.healthdirect.gov.au/how-to-use-an-adrenaline-autoinjector-epipen-anapen">adrenaline auto-injector pen</a>. Concerningly, surveys suggest only <a href="https://pubmed.ncbi.nlm.nih.gov/30646188/">about one in four adults</a> with food allergy have an adrenaline pen.</p> <p>If you have an IgE-mediated food allergy, discuss your management plan with your doctor. Here’s why having chocolate can make you feel great or a bit sick – plus 4 tips for better eating

<p><em><a href="https://theconversation.com/profiles/saman-khalesi-366871">Saman Khalesi</a>, <a href="https://theconversation.com/institutions/cquniversity-australia-2140">CQUniversity Australia</a></em></p> <p>Australians are <a href="https://www.retail.org.au/media/sweet-spending-boon-predicted-for-easter-retail">predicted</a> to spend around A$1.7 billion on chocolates, hot cross buns and other special foods this Easter season.</p> <p>Chocolate has a long history of production and consumption. It is made from cacao beans that go through processes including fermentation, drying, roasting and grounding. What is left is a rich and fatty liquor that is pressed to remove the fat (cocoa butter) and the cacao (or “cocoa”) powder which will then be mixed with different ingredients to produce dark, milk, white and other types of chocolates.</p> <p>There are several health benefits and potential problems that come in these sweet chocolatey packages.</p> <h2>The good news</h2> <p>Cacao beans contain <a href="https://foodstruct.com/food/cocoa-bean">minerals</a> like iron, potassium, magnesium, zinc and phosphorus and some vitamins. They are also rich in beneficial chemicals called <a href="https://pubmed.ncbi.nlm.nih.gov/23150750/">polyphenols</a>.</p> <p>These are great antioxidants, with the potential to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5465250/">improve heart health</a>, increase <a href="https://pubmed.ncbi.nlm.nih.gov/25164923/">nitric oxide</a> (which dilates blood vessels) and <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3488419/">reduce blood pressure</a>, provide food for gut microbiota and <a href="https://www.mdpi.com/2072-6643/12/7/1908">promote gut health</a>, boost the <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5465250/">immune system</a> and reduce inflammation.</p> <p>However, the concentration of polyphenols in the chocolate we eat depends largely on the cocoa solid amounts used in the final product.</p> <p>In general terms, the darker the chocolate, the more cocoa solids, minerals and polyphenols it has. For example, dark chocolates may have around <a href="https://www.tandfonline.com/doi/full/10.1080/10942912.2011.614984">seven times more polyphenols</a> compared to white chocolates and <a href="https://www.tandfonline.com/doi/full/10.1080/10942912.2011.614984">three times more polyphenols</a> compared to milk chocolates.</p> <h2>But also some bad news</h2> <p>Unfortunately, the <a href="https://theconversation.com/treat-or-treatment-chocolate-is-good-but-cocoa-is-better-for-your-heart-3084">health benefits of cocoa solids</a> are easily offset by the high sugar and fat content of modern-day chocolates. For example, milk and white chocolate eggs are on average 50% sugar, 40% fat (mostly saturated fats) – which means a lot of added kilojoules (calories).</p> <p>Also, there may be some side effects that come with ingesting chocolate.</p> <p>Cocoa beans include a compound called theobromine. While it has the anti-inflammatory properties responsible for some of the health benefits of chocolate, it is also a mild brain stimulant that acts in a similar way to caffeine. The mood boost it offers may also be partly responsible for how much we <a href="https://www.frontiersin.org/articles/10.3389/fphar.2015.00030/full?crsi=662496658&amp;cicada_org_src=healthwebmagazine.com&amp;cicada_org_mdm=direct">like chocolate</a>. Dark chocolate has higher theobromine compared to milk and white chocolate.</p> <p>But accordingly, overindulging in chocolate (and therefore theobromine) may lead to feeling restless, <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3672386/">headaches</a> and nausea.</p> <h2>What else is in your chocolate?</h2> <p>Milk and dairy-based chocolates may also cause stomach upset, abdominal pain and bloating in people with <a href="https://dietitiansaustralia.org.au/health-advice/lactose-intolerance">lactose intolerance</a>. This happens when we don’t produce enough lactase enzymes to digest milk sugar (lactose).</p> <p>People with lactose intolerance can usually tolerate up to 6 grams of lactose without showing symptoms. Milk chocolate can have around <a href="https://www.ncbi.nlm.nih.gov/books/NBK310258/">3 grams of lactose</a> per 40 grams (the size of a standard chocolate bar). So two chocolate bars (or the equivalent in milk chocolate eggs or bunnies) may be enough to cause symptoms.</p> <p>It’s worth noting that lactase enzyme activity dramatically declines as we age, with the highest activity in newborns and children. So lactose sensitivity or intolerance may not be such an issue for your kids and your symptoms may increase over time. Genetics also plays a major role in how sensitive people are to lactose.</p> <p><a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6815241/">Allergic reactions</a> to chocolate are usually due to the added ingredients or cross-contamination with potential allergens such as nuts, milk, soy, and some sweeteners used in the production of chocolate.</p> <p>Symptoms can be mild (acne, rashes and stomach pain) or more severe (swelling of the throat and tongue and shortness of breath).</p> <p>If you or your family members have known allergic reactions, make sure you read the label before indulging – especially in a whole block or basket of the stuff. And if you or your family members do experience symptoms of an allergic reaction after eating chocolate, <a href="https://www.betterhealth.vic.gov.au/health/conditionsandtreatments/allergic-reactions-emergency-first-aid">seek medical attention</a> immediately.</p> <h2>4 take home tips</h2> <p>So, if you are like me and have a weakness for chocolate there are a few things you can do to make the experience a good one.<img style="border: none !important; box-shadow: none !important; margin: 0 !important; max-height: 1px !important; max-width: 1px !important; min-height: 1px !important; min-width: 1px !important; opacity: 0 !important; outline: none !important; padding: 0 !important;" src="https://counter.theconversation.com/content/202848/count.gif?distributor=republish-lightbox-basic" alt="The Conversation" width="1" height="1" /></p> <ol> <li>keep an eye out for the darker chocolate varieties with higher cocoa solids. You may notice a percentage on labelling, which refers to how much of its weight is from cocoa beans. In general, the higher this percentage, the lower the sugar. White chocolate has almost no cocoa solid, and mostly cocoa butter, sugar and other ingredients. Dark chocolate has 50–100% cocoa beans, and less sugar. Aim for at least 70% cocoa</li> <li>read the fine print for additives and possible cross-contamination, especially if allergies might be an issue</li> <li>the ingredients list and nutrition information panel should tell you all about the chocolate you choosing. Go for varieties with lower sugar and less saturated fat. How do I handle it if my parent is refusing aged care? 4 things to consider

<p><em><a href="https://theconversation.com/profiles/lee-fay-low-98311">Lee-Fay Low</a>, <a href="https://theconversation.com/institutions/university-of-sydney-841">University of Sydney</a></em></p> <p>It’s a shock when we realise our parents aren’t managing well at home.</p> <p>Perhaps the house and garden are looking more chaotic, and Mum or Dad are relying more on snacks than nutritious meals. Maybe their grooming or hygiene has declined markedly, they are socially isolated or not doing the things they used to enjoy. They may be losing weight, have had a fall, aren’t managing their medications correctly, and are at risk of getting scammed.</p> <p>You’re worried and you want them to be safe and healthy. You’ve tried to talk to them about aged care but been met with swift refusal and an indignant declaration “I don’t need help – everything is fine!” Now what?</p> <p>Here are four things to consider.</p> <h2>1. Start with more help at home</h2> <p>Getting help and support at home can help keep Mum or Dad well and comfortable without them needing to move.</p> <p>Consider drawing up a roster of family and friends visiting to help with shopping, cleaning and outings. You can also use home aged care services – or a combination of both.</p> <p>Government subsidised home care services provide from one to 13 hours of care a week. You can get more help if you are a veteran or are able to pay privately. You can take advantage of things like rehabilitation, fall risk-reduction programs, personal alarms, stove automatic switch-offs and other technology aimed at increasing safety.</p> <p>Call <a href="https://www.myagedcare.gov.au/">My Aged Care</a> to discuss your options.</p> <h2>2. Be prepared for multiple conversations</h2> <p>Getting Mum or Dad to accept paid help can be tricky. Many families often have multiple conversations around aged care before a decision is made.</p> <p>Ideally, the older person feels supported rather than attacked during these conversations.</p> <p>Some families have a meeting, so everyone is coming together to help. In other families, certain family members or friends might be better placed to have these conversations – perhaps the daughter with the health background, or the auntie or GP who Mum trusts more to provide good advice.</p> <p>Mum or Dad’s main emotional support person should try to maintain their relationship. It’s OK to get someone else (like the GP, the hospital or an adult child) to play “bad cop”, while a different person (such as the older person’s spouse, or a different adult child) plays “good cop”.</p> <h2>3. Understand the options when help at home isn’t enough</h2> <p>If you have maximised home support and it’s not enough, or if the hospital won’t discharge Mum or Dad without extensive supports, then you may be <a href="https://academic.oup.com/gerontologist/article/60/8/1504/5863160">considering a nursing home</a> (also known as residential aged care in Australia).</p> <p>Every person has a legal right to <a href="https://humanrights.gov.au/our-work/9-your-right-choose-where-you-live">choose where we live</a> (unless they have lost capacity to make that decision).</p> <p>This means families can’t put Mum or Dad into residential aged care against their will. Every person also has the right to choose to take risks. People can choose to continue to live at home, even if it means they might not get help immediately if they fall, or eat poorly. We should respect Mum or Dad’s decisions, even if we disagree with them. Researchers call this “dignity of risk”.</p> <p>It’s important to understand Mum or Dad’s point of view. Listen to them. Try to figure out what they are feeling, and what they are worried might happen (which might not be rational).</p> <p>Try to understand what’s really important to their quality of life. Is it the dog, having privacy in their safe space, seeing grandchildren and friends, or something else?</p> <p>Older people are often understandably concerned about losing independence, losing control, and having strangers in their personal space.</p> <p>Sometimes families prioritise physical health over psychological wellbeing. But we need to consider both when considering nursing home admission.</p> <p><a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC9826495/">Research</a> suggests going into a nursing home temporarily increases loneliness, risk of depression and anxiety, and sense of losing control.</p> <p>Mum and Dad should be involved in the decision-making process about where they live, and when they might move.</p> <p>Some families start looking “just in case” as it often takes some time to <a href="https://www.abc.net.au/everyday/questions-to-ask-when-choosing-an-aged-care-home-for-a-loved-one/10302590">find the right nursing home</a> and there can be a wait.</p> <p>After you have your top two or three choices, take Mum or Dad to visit them. If this is not possible, take pictures of the rooms, the public areas in the nursing home, the menu and the activities schedule.</p> <p>We should give Mum or Dad information about their options and risks so they can make informed (and hopefully better) decisions.</p> <p>For instance, if they visit a nursing home and the manager says they can go on outings whenever they want, this might dispel a belief they are “locked up”.</p> <p>Having one or two weeks “respite” in a home may let them try it out before making the big decision about staying permanently. And if they find the place unacceptable, they can try another nursing home instead.</p> <h2>4. Understand the options if a parent has lost capacity to make decisions</h2> <p>If Mum or Dad have lost capacity to choose where they live, family may be able to make that decision in their best interests.</p> <p>If it’s not clear whether a person has capacity to make a particular decision, a medical practitioner can assess for that capacity.</p> <p>Mum or Dad may have appointed an <a href="https://www.tag.nsw.gov.au/wills/appoint-enduring-guardian/what-enduring-guardian">enduring guardian</a> to make decisions about their health and lifestyle decisions when they are not able to.</p> <p>An enduring guardian can make the decision that the person should live in residential aged care, if the person no longer has the capacity to make that decision themselves.</p> <p>If Mum or Dad didn’t appoint an enduring guardian, and have lost capacity, then a court or tribunal can <a href="https://www.tag.nsw.gov.au/guardianship/information-about-guardianship">appoint</a> that person a private guardian (usually a family member, close friend or unpaid carer).</p> <p>If no such person is available to act as private guardian, a public official may be appointed as public guardian.</p> <h2>Deal with your own feelings</h2> <p>Families often feel <a href="https://link.springer.com/article/10.1007/s12144-023-04538-9">guilt and grief</a> during the decision-making and transition process.</p> <p>Families need to act in the best interest of Mum or Dad, but also balance other caring responsibilities, financial priorities and their own wellbeing.<img style="border: none !important; box-shadow: none !important; margin: 0 !important; max-height: 1px !important; max-width: 1px !important; min-height: 1px !important; min-width: 1px !important; opacity: 0 !important; outline: none !important; padding: 0 !important;" src="https://counter.theconversation.com/content/221210/count.gif?distributor=republish-lightbox-basic" alt="The Conversation" width="1" height="1" /></p> <p><a href="https://theconversation.com/profiles/lee-fay-low-98311"><em>Lee-Fay Low</em></a><em>, Professor in Ageing and Health, <a href="https://theconversation.com/institutions/university-of-sydney-841">University of Sydney</a></em></p> <p><em>Image credits: Getty Images </em></p> <p><em>This article is republished from <a href="https://theconversation.com">The Conversation</a> under a Creative Commons license. 4 strategies to keep you from overspending this holiday season

<p><em><a href="https://theconversation.com/profiles/johanna-peetz-1494248">Johanna Peetz</a>, <a href="https://theconversation.com/institutions/carleton-university-900">Carleton University</a></em></p> <p>The urge to spend money is present all year round, but during the gift-giving season, the temptation to splurge on loved ones can be particularly strong. For many, the desire to be generous during the holidays clashes with the need to conserve funds for essential expenses.</p> <p>This year, money is tighter than ever, with <a href="https://www150.statcan.gc.ca/n1/daily-quotidien/231121/dq231121a-eng.htm?indid=3665-1&amp;indgeo=0">high prices for groceries, housing and entertainment</a> leaving shoppers with reduced funds as the holiday season descends upon us.</p> <p>A growing number of individuals are feeling the financial squeeze, with 40 per cent of Canadians <a href="https://www.bnnbloomberg.ca/financial-stress-is-impacting-the-mental-health-of-canadians-survey-1.1933491">citing money as their main source of stress</a>. <a href="https://newsroom.bmo.com/2023-11-08-78-Per-Cent-of-Canadians-Plan-to-Cut-Back-on-Holiday-Spending,-but-a-Third-Will-Still-Give-Back-to-Charitable-Causes-BMO-Survey">Seventy-eight per cent of Canadians</a> plan on buying fewer gifts this holiday season and 37 per cent are worried they won’t be able to afford all the items on their holiday shopping lists.</p> <p>Given that <a href="https://doi.org/10.3389/fpsyg.2022.790434">pricier gifts are not necessarily more appreciated by the person receiving the gift</a>, what are some ways shoppers can resist the temptation of appealing, yet expensive, gift options that might strain their finances?</p> <p>As a social psychologist who studies personal spending, I think it is worthwhile to remind ourselves of self-control strategies that can help us manage financial decisions during the holiday season.</p> <h2>Strategies for resisting temptation</h2> <p><a href="https://doi.org/10.1177/1745691615623">Self-control is not just suppressing temptation</a>; it also involves setting yourself up for success by creating situations that make resisting temptations easier.</p> <p><strong>1. Avoid temptations</strong></p> <p>Perhaps the most obvious strategy is to avoid shopping temptations. This may include steering clear of places — both physical and online — that are out of your budget range. While this is easier said than done during gift shopping, it’s an effective way to manage temptations: People who report having an easier time with self-control <a href="https://doi.org/10.1016/j.paid.2014.09.031">tend to avoid rather than resist temptations</a>.</p> <p><strong>2. Make a budget</strong></p> <p>If you haven’t sat down to make a holiday budget yet, it’s never too late to make one. Considering one-quarter of Canadians are <a href="https://globalnews.ca/news/10087745/canadian-holiday-spending-debt/">still paying off last year’s holiday debts</a>, being as fiscally responsible as possible is a wise choice this year.</p> <p>Setting spending limits ahead of time makes your financial goals clear and explicit. When setting budgets for gifts <a href="https://doi.org/10.1093/jcr/ucad011">people tend to spend the entirety of the estimated amount (unlike budgets for personal purchases where they try to come in under the budget)</a>. It’s good to be realistic, rather than optimistic, when setting budgets.</p> <p><strong>3. Implementation intentions</strong></p> <p>Anticipate any potential shopping temptations you are likely to encounter so you can develop strategies to resist them. One effective approach is <a href="https://kops.uni-konstanz.de/server/api/core/bitstreams/14cc2a36-5f01-4dc1-b9ca-f2d0ca0c8930/content">forming intentions</a> about how you will act once you encounter a temptation.</p> <p>For example, you might consider what you will do when you see a gadget your friend would enjoy when you have already bought them something and have reached the limit of your budget. Instead of purchasing it and exceeding your budget, you could write down the gadget for next year’s gift.</p> <p><strong>4. Write a list</strong></p> <p>Finally, thinking ahead to the gifts you plan to buy and writing a shopping list rather than relying on being inspired in the store might help with sticking to a budget. <a href="http://dx.doi.org/10.1007/s11747-019-00670-w">Consumers spend thousands each year on impulse purchases</a>. Writing shopping lists, <a href="https://doi.org/10.1002/cb.1812">even for online shopping</a>, can reduce overall spending and shopping regret.</p> <h2>The best strategy is the one that works</h2> <p>The holidays should be about joy, not financial stress. Maintaining self-control allows you to celebrate without compromising your financial well-being.</p> <p>There are of course many strategies beyond the four strategies listed here that can help create situations where resisting temptations is easier. The most effective strategies for maintaining financial self-control <a href="https://doi.org/10.1016/j.jesp.2021.104189">are the ones you are already using</a>, and the ones <em>you</em> find most effective. If you want to avoid giving in to shopping temptations, take a moment to think about the financial strategies you are already using and think about how you might use them in your holiday shopping.</p> <p>If you haven’t yet found a strategy that works for you, now is a great opportunity for you to try some out and see which ones are effective. Using strategies to manage the cost of holiday spending can prevent gift-giving from becoming a financial stressor in an already stressful time.</p> <p>Finally, while adhering to a budget is important, it shouldn’t be the sole or primary focus during holiday shopping. Keep in mind that the true spirit of the season is spending quality time with loved ones. 1 in 4 adults think smacking is necessary to ‘properly raise’ kids. But attitudes are changing

<p><a href="https://theconversation.com/profiles/divna-haslam-893417">D<em>ivna Haslam</em></a><em>, <a href="https://theconversation.com/institutions/queensland-university-of-technology-847">Queensland University of Technology</a></em></p> <p>“Do you want a smack?!” This has been a common refrain from many parents across history. Right along with “just wait till your father gets home”. Somehow parents thought this threat of violence would magically improve their child’s behaviour.</p> <p>The United Nations <a href="https://www.right-to-education.org/sites/right-to-education.org/files/resource-attachments/CRC_1989.pdf">Convention on the Rights of the Child</a> considers smacking and all types of physical punishment, however mild, a violation of child rights. It’s banned in <a href="https://endcorporalpunishment.org/countdown/">65 countries</a>.</p> <p>Yet it remains <a href="https://aifs.gov.au/resources/resource-sheets/physical-punishment-legislation#:%7E:text=Physical%20punishment%20by%20a%20parent%20towards%20a%20child%20remains%20lawful,'">legal</a> in Australia for parents to use “reasonable force” for discipline. Children are the only group of people it remains legal to hit.</p> <p>Our <a href="https://onlinelibrary.wiley.com/doi/epdf/10.1002/ajs4.301">new research</a> found one in four Australians still think physical punishment is necessary to “properly raise” children. And half of parents (across all age groups) reported smacking their children.</p> <p>But attitudes are slowly changing, with newer generations of parents less likely to smack their kids than previous ones.</p> <h2>What is physical punishment?</h2> <p><a href="https://www.jstor.org/stable/j.ctt1njkrb">Physical</a> or “corporal” punishment is the use of physical force to cause pain, but not injury, to discipline a child for misbehaviour. It’s distinct from physical abuse which is more extreme and not used to correct behaviour.</p> <p>Physical punishment is <a href="https://onlinelibrary.wiley.com/doi/10.1002/ajs4.276#:%7E:text=Corporal%20punishment%20(CP)%20is%20the,and%20Christian%20missionaries%20during%20colonisation.">the most common type</a> of violence against children. It usually involves smacking, but also includes things like pinching, slapping, or using an implement such as wooden spoon, cane or belt.</p> <p>Smacking doesn’t actually work and makes behaviour <a href="https://journals.sagepub.com/doi/abs/10.1177/0956797617729816?casa_token=YHpEf1m4GiwAAAAA%3A8VRH5_z9fufHJiFGpWVYAk0kuTZCCRB-zneATDatqfLomERAhcyyIES30hMPdIIQ-E-IHOTekiC0Zg&amp;journalCode=pssa">worse over time</a>. And it’s <a href="https://psycnet.apa.org/doiLanding?doi=10.1037%2Ffam0000191">associated with</a> children internalising problems, increased child aggression, poor parent-child relationships, poorer metal heath and more.</p> <p>In contrast, there are a lot of non-violent parenting strategies that <a href="https://theconversation.com/research-shows-its-harmful-to-smack-your-child-so-what-should-parents-do-instead-186739">do work</a>.</p> <h2>Assessing the state of smacking in Australia</h2> <p>We conducted the first <a href="https://onlinelibrary.wiley.com/doi/epdf/10.1002/ajs4.301">study</a> to comprehensively assess the state of smacking and physical punishment in Australia. We wanted to determine if smacking was still common and how many Australians believed we need to smack our kids.</p> <p>We interviewed more than 8,500 Australians aged 16 to 65 years. Our sample was representative of the national population so we can be confident the findings represent the thoughts and experiences of Australians as a nation.</p> <p>Using such a large age range allowed us to compare people across different age groups to determine if changes are occurring.</p> <h2>What we found</h2> <p>Overall, six in ten (62.5%) Australians between 16–65 years had experienced four or more instances of smacking or physical punishment in childhood. Men were slightly more likely to be physically punished than women (66.3% v 59.1%).</p> <p>Young people, aged 16–24, reported slightly lower rates (58.4%) than older people suggesting a slight decline over time. But these rates remain unacceptably high.</p> <p>Overall, one in two (53.7%) Australian parents reported using some type of physical punishment, mostly about once a month.</p> <p>However, older parents reported on this retrospectively (what they did while raising children) and there were clear age differences:</p> <ul> <li>64.2% of parents aged over 65 years had used physical punishment</li> <li>32.8% of parents 25–34 years had used it</li> <li>14.4% of parents under 24 had used it.</li> </ul> <p>So younger generations of parents are substantially less likely to use physical punishment.</p> <p><iframe id="3dcJw" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/3dcJw/2/" width="100%" height="400px" frameborder="0"></iframe></p> <p>Concerningly, one-quarter (26.4%) of all Australians still believe physical punishment is necessary to properly raise children. But the vast majority (73.6%) do not.</p> <p>And generational change is occurring. Some 37.9% of Australians older than 65 believe physical punishment is necessary compared to 22.9% of those aged 35–44 years, and only 14.8% of people under age 24.</p> <p><iframe id="NT51y" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/NT51y/3/" width="100%" height="400px" frameborder="0"></iframe></p> <p>Socioeconomically disadvantaged people are 2.3 times more likely to believe physical punishment is necessary than those with no disadvantage.</p> <p>Parents who had been physically disciplined when they were children were both more likely to believe it is needed and more likely to use it with their own children. This indicates this form of violence is transmitted across generations.</p> <h2>Time for change</h2> <p>Law reform works best when changes in community attitudes and behaviours are already occurring. So it’s encouraging that younger people are much less likely to believe physical punishment is necessary and are much less likely to use it. This suggests Australians may be open to prohibiting this common form of violence.</p> <p>All states and territories should immediately enact legal reform to prohibit corporal punishment and protect the rights of Australian children. This should be paired with public health and education campaigns about what parents can do instead.</p> <p>If you are a parent looking for effective non-violent parenting strategies the <a href="https://www.health.gov.au/ministers/the-hon-greg-hunt-mp/media/406-million-to-support-the-mental-health-and-wellbeing-of-aussie-kids">government</a> has also made the <a href="https://www.triplep-parenting.net.au/qld-en/free-parenting-courses/triple-p-online-under-12/?gad_source=1&amp;gclid=Cj0KCQiAgqGrBhDtARIsAM5s0_mmMmbY3khwvp306pGOijqntKzYh6dDI5lQYszLgl6_BOGnuk8HMeEaAn_vEALw_wcB">Triple P Positive Parenting Program</a> available for free. This online program provides practical strategies parents can use to encourage positive behaviour and calm, alternative discipline techniques that can be used to instead of smacking.</p> <p>A number of other evidence-based programs, such as <a href="https://tuningintokids.org.au/">Tuning Into Kids</a>, Parents Under Pressure and <a href="https://www.pcit.org/pcit-in-australia.html">Parent Child Interaction Therapy</a>, are also available.</p> <p>Australia has an opportunity to capitalise on naturally occurring societal changes. The 4 biggest gift-giving mistakes, according to a consumer psychologist

<p><em><a href="https://theconversation.com/profiles/julian-givi-1395671">Julian Givi</a>, <a href="https://theconversation.com/institutions/west-virginia-university-1375">West Virginia University</a></em></p> <p>A good gift can elicit a surge of happiness and gratitude in the recipient. It also feels great to give, <a href="https://theconversation.com/whats-the-point-of-holiday-gifts-173306">with psychologists finding</a> that the joy of giving a gift is more pronounced than the pleasure of receiving one.</p> <p>Unfortunately, there are times when you receive a gift and you have to force a smile and fake your gratitude.</p> <p>I’m a consumer psychologist <a href="https://scholar.google.com/citations?user=wjAq_TcAAAAJ&amp;hl=en">who specializes in gift-giving research</a> – in particular, gift-giving mistakes.</p> <p>Here are four of the most common ones.</p> <h2>1. Prioritizing the big reveal</h2> <p>One way givers can err is by focusing too much on <a href="https://doi.org/10.1177/0963721416656937">the moment the recipient will open the gift</a>.</p> <p>Givers want their gift to be <a href="https://doi.org/10.1086/675737">desirable</a>. They hope <a href="https://doi.org/10.1016/j.jesp.2011.03.015">to surprise</a> the recipient and <a href="http://dx.doi.org/10.2139/ssrn.2733341">put a smile</a> on their face.</p> <p>A chocolate fondue fountain might meet these criteria – it’s quirky and sure to elicit curiosity and smiles from onlookers.</p> <p>However, when people receive a gift, they care less about the moment the bow comes off, and instead think about the weeks and months ahead.</p> <p>People want gifts that are <a href="https://www.acrwebsite.org/volumes/1023703/volumes/v45/NA-45">useful</a> and <a href="https://doi.org/10.1086/675737">reliable</a> and <a href="https://doi.org/10.1016/j.jesp.2011.03.015">meet their needs</a>.</p> <p>How often would a chocolate fondue fountain realistically be used?</p> <p>Compare that to a new coffee maker, which could see action every day. Sure, it isn’t a novelty – and probably won’t elicit “oohs” and “ahhs” on Christmas Day – but the recipient will be quite happy to have it on hand when their alarm rings each morning.</p> <h2>2. Unique and new are overrated</h2> <p>Another factor that can lead givers to go wrong involves unwritten rules for what constitutes good gift-giving practices.</p> <p>Givers often focus on these rules more than they should. For example, they may <a href="https://doi.org/10.1016/j.jbusres.2020.05.023">avoid giving the same gift</a> to someone in back-to-back years because this goes against the norm of giving a unique gift each year. Givers also often <a href="https://doi.org/10.1348/014466604x23428">refrain from giving used products</a> as gifts because this violates the unspoken rule that a gift should be brand new.</p> <p>In contrast, recipients are quite open to gifts that violate these norms.</p> <p>If someone loves a certain type of wine, they’re <a href="https://doi.org/10.1016/j.jbusres.2020.05.023">more than happy to receive it</a> in subsequent years. And if one digital camera is lightly used but possesses several innovative features, while another is new but has fewer features, people <a href="https://doi.org/10.1348/014466604x23428">are happy to receive the used one</a>.</p> <h2>3. Being risk-averse</h2> <p>Givers can make missteps when they avoid gifts that they see as too risky.</p> <p>Consider sentimental gifts, like a scrapbook or a nostalgic memento.</p> <p>Studies have shown that recipients <a href="https://doi.org/10.1016/j.jcps.2017.06.002">love these gifts</a>; they <a href="https://doi.org/10.1037/pspa0000036">elicit happiness for extended periods of time</a>.</p> <p>Givers, however, tend to <a href="https://doi.org/10.1016/j.jcps.2017.06.002">shy away from sentimental gifts</a> because they see them risky – sure, they could be a home run, but they could also whiff. Doubts can creep into shoppers’ heads as they consider sentimental gifts: What if it comes across as sappy? What if the recipient thinks I’m being cheap?</p> <p>And so people tend to opt for <a href="https://www.sciencedirect.com/science/article/abs/pii/S105774081730044X">safer, superficial gifts</a> that they assume will be at least somewhat well-liked. Or, to continue with the baseball analogy, givers are happy to take the sure single.</p> <p>As another example, consider material goods versus experiences.</p> <p>When giving gifts, people often opt for <a href="https://doi.org/10.1093/jcr/ucy010">tangible objects over experiences</a> because material goods are on the safer side – almost everyone could use a new appliance or a new shirt. Experiences are trickier; they require a bit more of an understanding of who the recipient truly is – not everyone loves going to see the symphony.</p> <p>Yet recipients tend to be <a href="https://doi.org/10.1002/jcpy.1281">more open to experiences than givers anticipate</a> – and these gifts <a href="https://www.acrwebsite.org/volumes/1017575/volumes/v42/NA-42">are actually more likely to make people happier</a> than material goods.</p> <h2>4. Does the thought really count?</h2> <p>Givers can also err by wanting their gift to appear especially thoughtful.</p> <p>Of course, recipients appreciate thoughtfulness – but not when it comes at the expense of receiving something that’s actually useful.</p> <p>This plays out when givers are shopping for multiple people. They’ll often <a href="https://doi.org/10.1086/674199">choose unique gifts for each recipient</a>, rather than give the same gift to everyone, because a distinct gift for each person will make them feel as though they put more time and effort into gift selection. People do this even if they realize that some recipients will be receiving less desirable gifts.</p> <p>You’ll also see this happen with <a href="https://www.acrwebsite.org/volumes/1020146/volumes/v43/NA-43">gift cards</a>. Givers often choose specific gift cards – to a particular clothing store or restaurant, for example – that reflect the interests or tastes of the recipient.</p> <p>But recipients are more open to gift cards that give them more flexibility and freedom – think an Amazon or Visa gift card. 4 tips to help your loved one with dementia enjoy the festive season

But as you make these plans, it’s important to consider how you can best include any friends or loved ones living with dementia.</p> <p>While no two people experience dementia in the exact same way, <a href="https://www.dementia.org.au/about-dementia/what-is-dementia">dementia</a> often affects the way people process and respond to their environment. Too much stimulation – like a lot of noise and activity at a Christmas party – can be overwhelming and may cause confusion or agitation.</p> <p>Finding ways to create a <a href="https://www.healthdirect.gov.au/creating-a-calming-home-for-people-with-dementia#:%7E:text=The%20physical%20environment%20is%20important,in%20and%20enjoy%20everyday%20activities.">safe environment</a> at home for your loved one with dementia will help maximise the chances everyone has a good time.</p> <h2>1. Plan ahead, but be flexible</h2> <p>Planning celebrations can be overwhelming for everyone involved, and having excessive expectations can raise stress levels. Try to keep <a href="https://www.dementia.org.au/information/about-you/i-am-a-carer-family-member-or-friend">expectations realistic</a> and in line with the current needs of your friend or loved one living with dementia.</p> <p>For example, people with dementia may experience <a href="https://www.dementia.org.au/information/about-you/i-am-a-carer-family-member-or-friend/personal-care/eating">changes in their appetite</a> or food preferences, or difficulties chewing and swallowing. These changes might make some of the things on your festive menu unappetising or difficult to eat. Be guided by the needs and preferences of the person with dementia and keep options limited to one or two special foods if larger banquets are likely to be overwhelming.</p> <p>Things can change quickly for people living with dementia and their abilities will likely <a href="https://www.dementia.org.au/about-dementia/what-is-dementia/progression-of-dementia#:%7E:text=A%20person%20with%20dementia's%20abilities,over%20a%20number%20of%20years.">vary from day to day</a>. Try to be flexible and have a backup plan in place. For special events, plan to record speeches or ceremonies to share when things are quieter.</p> <p>If you’re planning a large event, consider having a smaller gathering with your loved one with dementia and just a few special people.</p> <h2>2. Stick to the familiar</h2> <p>The sudden appearance of lots of decorations <a href="https://www.dementia.org.au/sites/default/files/helpsheets/Helpsheet-TipsToAssistSocialEngagement03-TipsForHolidayCheer_english.pdf">may be overwhelming</a> for a person with dementia and trigger a negative sensory reaction or distress. Ensure decorations are safe and familiar and put them up slowly over a period of a few days.</p> <p>Try to also stick to familiar traditions and routines. <a href="https://www.alz.org/help-support/caregiving/daily-care/daily-care-plan">Daily routines</a> are an important way of supporting people with dementia and sudden changes <a href="https://memory.ucsf.edu/caregiving-support/behavior-personality-changes#I-Investigate-Possible-Causes">may result in agitation and distress</a>. Stick to routine eating, bathing, and rest times where possible throughout the holiday period.</p> <p>For many people with dementia, long-term memories are less affected than <a href="https://www.alzheimers.org.uk/about-dementia/symptoms-and-diagnosis/symptoms/memory-loss">more recent memories</a>. Familiar family traditions can therefore be a good way to <a href="https://www.dementia.org.au/national/support-and-services/carers/therapies-and-communication-approaches">reminisce</a>. Family keepsakes or <a href="https://www.dementia.org.au/sites/default/files/20110303-NSW-LifeHistoryBook.pdf">memory books</a> can also help connect with stories from past celebrations.</p> <h2>3. Have a quiet space</h2> <p>Try to have <a href="https://www.scie.org.uk/dementia/supporting-people-with-dementia/dementia-friendly-environments/noise.asp">a quiet place</a> where the person living with dementia can go if things become overwhelming. Designating a support person who can stay with them throughout the day and take them to a separate room or area away from the action can help to keep things calm.</p> <p>Having some familiar objects or <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5432607/#:%7E:text=Passive%20music%20listening%20is%20reported,et%20al.%2C%202013">quiet music</a> in the space can be a good way to block out the noise of activities and reduce agitation.</p> <h2>4. Make sure everyone has a part</h2> <p>Everyone wants to feel a part of the activities on a special day, including people living with dementia. Ensuring everyone has a role to play may mean modifying tasks to suit the abilities of the person with dementia.</p> <p>For example, if you’re hosting an event at home, try to get your friend or relative with dementia involved in the kitchen by tossing the salad or helping to set the table.</p> <p>People with dementia are still the same person, even if their abilities have changed or they can no longer communicate their needs and feelings like they used to. It’s important to treat everyone with dignity and try to include your friends and loved ones with dementia in celebrations whenever possible.</p> <h2>Sometimes, you can’t be together</h2> <p>Despite the best laid plans, sometimes it won’t be possible to share in festive celebrations with your loved one living with dementia. More advanced dementia, <a href="https://www.health.nsw.gov.au/Infectious/covid-19/Pages/racf-latest-advice.aspx">aged care visitor restrictions</a> or even just distance can keep many of us apart from our loved ones.</p> <p>Be prepared for this separation to bring up your own feelings of <a href="https://www.alz.org/help-support/caregiving/caregiver-health/grief-loss-as-alzheimers-progresses">grief or sadness</a>. Look after your mental health as well as the person with dementia.</p> <p>Caring responsibilities still largely <a href="https://www.pmc.gov.au/sites/default/files/resource/download/national-strategy-gender-equality-discussion-paper_0.pdf">fall to women</a> and it’s important to share the load. Should we still be using RATs to test for COVID? 4 key questions answered

However the threat COVID poses to us is significantly less than it has ever been, thanks to immunity we’ve acquired through <a href="https://www.thelancet.com/journals/laninf/article/PIIS1473-3099(22)00801-5/fulltext">a combination</a> of prior infection and vaccination.</p> <p>That said, COVID is by no means behind us. The threat of severe illness remains higher for many people, and we’re all potentially at risk of developing <a href="https://www.health.gov.au/topics/covid-19/long-covid">long COVID</a>.</p> <p>While many people appear to be doing fewer rapid antigen tests (RATs) than they used to – if any at all – with rising cases, and as we head towards the festive season, testing continues to be important.</p> <p>So what do you need to know about testing in this wave? Here are four key questions answered.</p> <h2>1. When should I do a RAT?</h2> <p>There are a few situations where determining your COVID status is important to inform your actions, particularly during an uptick in infections. With more circulating virus, your index of suspicion that you have COVID if you’re experiencing cold-like symptoms should be higher.</p> <p>RATs work best when they’re used to confirm whether you have COVID when you <a href="https://www.tga.gov.au/products/covid-19/covid-19-tests/how-testing-works-covid-19">have respiratory symptoms</a> and are infectious. So the primary use of RATs should be to determine your COVID status when you’re sick. A positive test should prompt you to isolate, and if you’re eligible, to seek antivirals.</p> <p>Testing might also be worthwhile if you’ve come into contact with someone with COVID but you haven’t developed symptoms. If you find you have in fact contracted the virus, you can take steps to avoid spreading it to other people (you can infect others even <a href="https://www.healthline.com/health/what-is-asymptomatic-covid#prevalence">when you’re asymptomatic</a>). This is especially important if you’re going to be socialising in large groups or in contact with people who are vulnerable.</p> <p>Another situation in which to consider testing, particularly at this time of year, is before attending large social gatherings. While the reliability of a RAT is never perfect, do the test as close to the event as possible, because your disease status <a href="https://www.businessinsider.com/omicron-rapid-test-hour-before-party-not-day-before-expert-2021-12">can change quickly</a>.</p> <h2>2. Should I test multiple times?</h2> <p>Yes. RATs are not as sensitive as PCR tests, which is the trade-off we make for being able to do this test at home and <a href="https://www.wsj.com/articles/public-health-officials-pursue-covid-19-tests-that-trade-precision-for-speed-11599562800">getting a rapid result</a>.</p> <p>This means that while if you test positive with a RAT you can be very confident you have COVID, if you test negative, you cannot be as confident that you don’t have COVID. That is, the test may give you a false negative result.</p> <p>Although RATs from different manufacturers have different accuracies, all RATs approved by Australia’s Therapeutic Goods Administration must have a sensitivity of <a href="https://www.tga.gov.au/products/covid-19/covid-19-tests/covid-19-rapid-antigen-self-tests-home-use/covid-19-rapid-antigen-self-tests-are-approved-australia#:%7E:text=For%20rapid%20antigen%20tests%2C%20this,specificity%20of%20at%20least%2098%25.">at least 80%</a>.</p> <p>The way to increase your confidence in a negative result is to do multiple RATs serially – each negative test increases the confidence you can have that you don’t have COVID. If you have symptoms and have tested negative after your first RAT, <a href="https://www.fda.gov/medical-devices/safety-communications/home-covid-19-antigen-tests-take-steps-reduce-your-risk-false-negative-results-fda-safety">the advice</a> is to repeat the test after 48 hours, and potentially a third time after another 48 hours if the second test is also negative.</p> <h2>3. Do RATs detect the latest variants?</h2> <p>Since RATs <a href="https://www.healthdirect.gov.au/covid-19/testing#:%7E:text=Rapid%20antigen%20tests%2C%20or%20RATs,of%20proteins%20of%20the%20virus.">detect particular surface proteins</a> on SARS-CoV-2 (the virus that causes COVID), it’s theoretically possible that as the virus evolves, the reliability of these tests may be affected.</p> <p>However, RATs were designed to detect a part of the virus that is not as likely to mutate, so the hope is these tests <a href="https://www.health.com/do-covid-tests-work-new-variants-7967102">will continue to hold up</a> as SARS-CoV-2 evolves.</p> <p>The performance of RATs is continually being assessed by manufacturers. So far, there’s been no change reported in the ability of these tests to <a href="https://www.ama.com.au/articles/tga-updated-advice-rats-nearing-expiry-and-rats-efficacy-current-strains#:%7E:text=The%20TGA%20has%20received%20evidence,19%20RAT%20post%2Dmarket%20review.">detect the latest variants</a>.</p> <h2>4. Can I rely on expired RATs?</h2> <p>At this point in the pandemic, you might have a few expired tests at the back of your cupboard.</p> <p>Technically the most appropriate advice is to say you should never use a diagnostic test <a href="https://www.tga.gov.au/products/covid-19/covid-19-tests/covid-19-rapid-antigen-self-tests-home-use/covid-19-rapid-antigen-self-tests-are-approved-australia">past its expiry date</a>. As a general principle the performance of a test cannot be guaranteed beyond this date. The risk is that over time the components of the RAT degrade and if you use a test that’s not working optimally, it’s more likely to indicate <a href="https://www.health.com/can-you-use-expired-covid-test-6827970">you don’t have COVID</a> when you actually do, which may have consequences.</p> <p>However, as for all things COVID, the answer is not so black and white. Since these tests were new when they were introduced earlier in the pandemic, manufacturers didn’t have specific data on their performance over time, and so the expiry dates given were necessarily conservative.</p> <p>It’s likely these tests will work beyond the expiry dates on the packet, but just how long and how well they work is a bit of an unknown, so we need to be cautious.</p> <p>The other thing to consider is ensuring you store RATs correctly. Storage instructions should be found on the packet, but the key issue is making sure they’re not exposed to extreme temperatures. In particular, <a href="https://7news.com.au/lifestyle/health-wellbeing/how-to-properly-store-your-at-home-covid-19-rapid-antigen-tests-c-5465412">high temperatures</a> may damage the chemicals in the test which may reduce its sensitivity.</p> <h2>The path from here</h2> <p>Regular upticks in COVID cases are something we’re going to have to get used to. At these times, we should all be a bit more cautious about looking after ourselves and others as we go about our lives.
