How long does back pain last? And how can learning about pain increase the chance of recovery?

One in thirteen people have it right now and worldwide a staggering 619 million people will <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7186678/">have it this year</a>.</p> <p>Chronic pain, of which back pain is the most common, is the world’s <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7186678/">most disabling</a> health problem. Its economic impact <a href="https://www.ncbi.nlm.nih.gov/books/NBK92510/">dwarfs other health conditions</a>.</p> <p>If you get back pain, how long will it take to go away? We scoured the scientific literature to <a href="https://www.cmaj.ca/content/cmaj/196/2/E29.full.pdf">find out</a>. We found data on almost 20,000 people, from 95 different studies and split them into three groups:</p> <ul> <li>acute – those with back pain that started less than six weeks ago</li> <li>subacute – where it started between six and 12 weeks ago</li> <li>chronic – where it started between three months and one year ago.</li> </ul> <p>We found 70%–95% of people with acute back pain were likely to recover within six months. This dropped to 40%–70% for subacute back pain and to 12%–16% for chronic back pain.</p> <p>Clinical guidelines point to graded return to activity and pain education under the guidance of a health professional as the best ways to promote recovery. Yet these effective interventions are underfunded and hard to access.</p> <h2>More pain doesn’t mean a more serious injury</h2> <p>Most acute back pain episodes are <a href="https://www.racgp.org.au/getattachment/75af0cfd-6182-4328-ad23-04ad8618920f/attachment.aspx">not caused</a> by serious injury or disease.</p> <p>There are rare exceptions, which is why it’s wise to see your doctor or physio, who can check for signs and symptoms that warrant further investigation. But unless you have been in a significant accident or sustained a large blow, you are unlikely to have caused much damage to your spine.</p> <p>Even very minor back injuries can be brutally painful. This is, in part, because of how we are made. If you think of your spinal cord as a very precious asset (which it is), worthy of great protection (which it is), a bit like the crown jewels, then what would be the best way to keep it safe? Lots of protection and a highly sensitive alarm system.</p> <p>The spinal cord is protected by strong bones, thick ligaments, powerful muscles and a highly effective alarm system (your nervous system). This alarm system can trigger pain that is so unpleasant that you cannot possibly think of, let alone do, anything other than seek care or avoid movement.</p> <p>The messy truth is that when pain persists, the pain system becomes more sensitive, so a widening array of things contribute to pain. This pain system hypersensitivity is a result of neuroplasticity – your nervous system is becoming better at making pain.</p> <h2>Reduce your chance of lasting pain</h2> <p>Whether or not your pain resolves is not determined by the extent of injury to your back. We don’t know all the factors involved, but we do know there are things that you can do to reduce chronic back pain:</p> <ul> <li> <p>understand how pain really works. This will involve intentionally learning about modern pain science and care. It will be difficult but rewarding. It will help you work out what you can do to change your pain</p> </li> <li> <p>reduce your pain system sensitivity. With guidance, patience and persistence, you can learn how to gradually retrain your pain system back towards normal.</p> </li> </ul> <h2>How to reduce your pain sensitivity and learn about pain</h2> <p>Learning about “how pain works” provides the most sustainable <a href="https://www.bmj.com/content/376/bmj-2021-067718">improvements in chronic back pain</a>. Programs that combine pain education with graded brain and body exercises (gradual increases in movement) can reduce pain system sensitivity and help you return to the life you want.</p> <p>These programs have been in development for years, but high-quality clinical trials <a href="https://jamanetwork.com/journals/jama/fullarticle/2794765">are now emerging</a> and it’s good news: they show most people with chronic back pain improve and many completely recover.</p> <p>But most clinicians aren’t equipped to deliver these effective programs – <a href="https://www.jpain.org/article/S1526-5900(23)00618-1/fulltext">good pain education</a> is not taught in most medical and health training degrees. Many patients still receive ineffective and often risky and expensive treatments, or keep seeking temporary pain relief, hoping for a cure.</p> <p>When health professionals don’t have adequate pain education training, they can deliver bad pain education, which leaves patients feeling like they’ve just <a href="https://www.jpain.org/article/S1526-5900(23)00618-1/fulltext">been told it’s all in their head</a>.</p> <p>Community-driven not-for-profit organisations such as <a href="https://www.painrevolution.org/">Pain Revolution</a> are training health professionals to be good pain educators and raising awareness among the general public about the modern science of pain and the best treatments. Pain Revolution has partnered with dozens of health services and community agencies to train more than <a href="https://www.painrevolution.org/find-a-lpe">80 local pain educators</a> and supported them to bring greater understanding and improved care to their colleagues and community.</p> <p>But a broader system-wide approach, with government, industry and philanthropic support, is needed to expand these programs and fund good pain education. Been scammed? Here's how to financially recover

<p>Many people feel shame and embarrassment after realising they have been scammed. But you shouldn’t. You did nothing wrong; you are the victim of a crime. </p> <p>Not only are such feelings bad for your mental wellbeing, but they also often stop people reporting the scam or taking action to avoid further losses. </p> <p>Remember too that you’re not alone: victims reported more than 601,000 scams to the ACCC in 2023, together losing a staggering $2.74 billion. People of all ages, professions, and backgrounds have been affected. </p> <p>As hard as it may be, try to leave emotion aside and approach this like any other money matter – logically and methodically. Doing so will help you act faster and more decisively, which is crucial to your financial recovery. </p> <p>The following checklist will help you through this process:</p> <ul> <li><strong>Step 1 – Try to recoup your stolen money</strong></li> </ul> <p>Report the scam immediately. Contact your bank or card provider to stop the transaction being processed. Notify the company or marketplace where it occurred – they may have options to reverse the payment or for you to claim compensation for fraud. </p> <p>Also inform the ACCC’s Scamwatch and police if relevant, which may aid in tracking down the scammer and will help them alert the wider public on what to look out for. </p> <p>Unfortunately, the money is likely gone for good, but prompt action may just help you get some or all of it back. </p> <ul> <li><strong>Step 2 – Secure your accounts from further thefts</strong></li> </ul> <p>Once scammers have found a way to steal money, they often go back to try for more. Don’t let them! </p> <p>Freeze or cancel affected debit and credit cards, accounts etc. Change and strengthen all your passwords. Set up two-factor authentication if you haven’t already. Remove any suspicious applications on electronic devices. </p> <p>Double check the registrations of any business, adviser or tradesperson before engaging their services. Regularly check your superannuation, investments etc. to monitor for any inconsistencies.</p> <ul> <li><strong>Step 3 – Safeguard your cash flow</strong></li> </ul> <p>Don’t multiplying your losses by racking up new debts to cover the stolen money. That means limiting the use of credit cards, payday lenders and Buy Now, Pay Later schemes. Consider paying with cash instead to help you stick to a budget.</p> <p>If you have lost everything, register with Centrelink for income support. You may also be able to apply for hardship provisions with your bank, phone and energy providers and other essential services.</p> <ul> <li><strong>Step 4 – Get reputable advice</strong></li> </ul> <p>Legal advice may be able to get you out of bogus contracts, like loans or phone plans, and help you in the event your personal information has been stolen (which can be used in various ways to steal money). If you can’t afford a lawyer, there are free alternatives such as Legal Aid or Community Legal Centres. Specialist services such as the Women’s Legal Service may offer support where partner coercion or domestic abuse is involved.</p> <p>Accounting and financial advice may also help you navigate assistance options and longer term recovery efforts.</p> <ul> <li><strong>Step 5 – Rebuild your finances</strong></li> </ul> <p>Your ability to rebuild your finances after a scam will depend on a range of factors, including how much was lost plus your age and circumstances.</p> <p>You could seek to increase your earnings and/or cut your spending by tweaking your household budget, delaying retirement, or temporarily taking a second job to boost your income. </p> <p>Another option is to make your remaining finances work harder than before, such as adjusting your investment strategies (e.g. changing your risk weightings or selling assets) including within your superannuation or accessing equity in your home.</p> <p>If you’re a self-funded retiree, you may now qualify for a part or full pension if your scam losses push your total assets below the means test threshold.</p> <p>Ultimately, the most important things when dealing with the fallout from a scam is to look after yourself and protect what you have left.</p> <p>Scammers have already taken off with your dollars. War in Ukraine affected wellbeing worldwide, but people’s speed of recovery depended on their personality

<p><em><a href="https://theconversation.com/profiles/luke-smillie-7502">Luke Smillie</a>, <a href="https://theconversation.com/institutions/the-university-of-melbourne-722">The University of Melbourne</a></em></p> <p>The war in Ukraine has had impacts around the world. <a href="https://mitsloan.mit.edu/ideas-made-to-matter/ripple-effects-russia-ukraine-war-test-global-economies">Supply chains</a> have been disrupted, the <a href="https://news.un.org/pages/wp-content/uploads/2022/06/GCRG_2nd-Brief_Jun8_2022_FINAL.pdf?utm_source=United+Nations&amp;utm_medium=Brief&amp;utm_campaign=Global+Crisis+Response">cost of living</a> has soared and we’ve seen the <a href="https://www.unhcr.org/hk/en/73141-ukraine-fastest-growing-refugee-crisis-in-europe-since-wwii.html">fastest-growing refugee crisis since World War II</a>. All of these are in addition to the devastating humanitarian and economic impacts within Ukraine.</p> <p>Our international team was conducting a global study on wellbeing in the lead up to and after the Russian invasion. This provided a unique opportunity to examine the psychological impact of the outbreak of war.</p> <p>As we explain in a new study published in <a href="https://www.nature.com/articles/s41467-024-44693-6">Nature Communications</a>, we learned the toll on people’s wellbeing was evident across nations, not just <a href="https://ijmhs.biomedcentral.com/articles/10.1186/s13033-023-00598-3">in Ukraine</a>. These effects appear to have been temporary – at least for the average person.</p> <p>But people with certain psychological vulnerabilities struggled to recover from the shock of the war.</p> <h2>Tracking wellbeing during the outbreak of war</h2> <p>People who took part in our study completed a rigorous “<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2773515/">experience-sampling</a>” protocol. Specifically, we asked them to report their momentary wellbeing four times per day for a whole month.</p> <p>Data collection began in October 2021 and continued throughout 2022. So we had been tracking wellbeing around the world during the weeks surrounding the outbreak of war in February 2022.</p> <p>We also collected measures of personality, along with various sociodemographic variables (including age, gender, political views). This enabled us to assess whether different people responded differently to the crisis. We could also compare these effects across countries.</p> <p>Our analyses focused primarily on 1,341 participants living in 17 European countries, excluding Ukraine itself (44,894 experience-sampling reports in total). We also expanded these analyses to capture the experiences of 1,735 people living in 43 countries around the world (54,851 experience-sampling reports) – including in Australia.</p> <h2>A global dip in wellbeing</h2> <p>On February 24 2022, the day Russia invaded Ukraine, there was a sharp decline in wellbeing around the world. There was no decline in the month leading up to the outbreak of war, suggesting the change in wellbeing was not already occurring for some other reason.</p> <p>However, there was a gradual increase in wellbeing during the month <em>after</em> the Russian invasion, suggestive of a “return to baseline” effect. Such effects are commonly reported in psychological research: situations and events that impact our wellbeing often (<a href="https://www.researchgate.net/publication/237535630_Adaptation_and_the_Set-Point_Model_of_Subjective_Well-BeingDoes_Happiness_Change_After_Major_Life_Events">though not always</a>) do so <a href="https://www.researchgate.net/publication/7062343_Beyond_the_Hedonic_Treadmill_Revising_the_Adaptation_Theory_of_Well-Being">temporarily</a>.</p> <p>Unsurprisingly, people in Europe experienced a sharper dip in wellbeing compared to people living elsewhere around the world. Presumably the war was much more salient for those closest to the conflict, compared to those living on an entirely different continent.</p> <p>Interestingly, day-to-day fluctuations in wellbeing mirrored the salience of the war on social media as events unfolded. Specifically, wellbeing was lower on days when there were more tweets mentioning Ukraine on Twitter/X.</p> <p>Our results indicate that, on average, it took around two months for people to return to their baseline levels of wellbeing after the invasion.</p> <h2>Different people, different recoveries</h2> <p>There are <a href="https://pubmed.ncbi.nlm.nih.gov/31944795/">strong links</a> between our wellbeing and our individual personalities.</p> <p>However, the dip in wellbeing following the Russian invasion was fairly uniform across individuals. None of the individual factors assessed in our study, including personality and sociodemographic factors, predicted people’s response to the outbreak of war.</p> <p>On the other hand, personality did play a role in how quickly people recovered. Individual differences in people’s recovery were linked to a personality trait called “stability”. Stability is a broad dimension of personality that combines low neuroticism with high agreeableness and conscientiousness (three traits from the <a href="https://www.sciencedirect.com/topics/social-sciences/big-five">Big Five</a> personality framework).</p> <p>Stability is so named because it reflects the stability of one’s overall psychological functioning. This can be illustrated by breaking stability down into its three components:</p> <ol> <li> <p>low neuroticism describes <a href="https://www.pnas.org/doi/full/10.1073/pnas.2212154120">emotional stability</a>. People low in this trait experience less intense negative emotions such as anxiety, fear or anger, in response to negative events</p> </li> <li> <p>high agreeableness describes <a href="https://psycnet.apa.org/record/2018-63285-010">social stability</a>. People high in this trait are generally more cooperative, kind, and motivated to maintain social harmony</p> </li> <li> <p>high conscientiousness describes <a href="https://doi.org/10.1016/j.paid.2023.112331">motivational stability</a>. People high in this trait show more effective patterns of goal-directed self-regulation.</p> </li> </ol> <p>So, our data show that people with less stable personalities fared worse in terms of recovering from the impact the war in Ukraine had on wellbeing.</p> <p>In a supplementary analysis, we found the effect of stability was driven specifically by neuroticism and agreeableness. <p dir="ltr">Madonna has finally issued a health update after being <a href="https://www.oversixty.co.nz/health/caring/madonna-rushed-to-intensive-care" target="_blank" rel="noopener">rushed to hospital</a> on June 27 when she was found unresponsive.</p> <p dir="ltr">The 64-year-old singer took to Instagram to announce that she is rescheduling the North American leg of her world tour following her hospitalisation.</p> <p dir="ltr">"Thank you for your positive energy, prayers and words of healing and encouragement. I have felt your love," the Material Girl wrote.</p> <p dir="ltr">“I'm on the road to recovery and incredibly grateful for all the blessings in my life," she added.</p> <p dir="ltr">She then revealed her thoughts after waking up in hospital.</p> <p dir="ltr">"My first thought when I woke up in the hospital was my children. My second thought was that I did not want to disappoint anyone who bought tickets for my tour. I also didn't want to let down the people who worked tirelessly with me over the last few months to create my show. "It's bloody terrifying": New details from John Farnham's sons

<p dir="ltr">John Farnham’s sons have opened up about the signs they mistook for a painful mouth ulcer instead of a cancerous lump.</p> <p dir="ltr">The legendary singer is recovering after a gruelling surgery on August 23 in which he had a part of his jaw removed due to a cancerous tumour in his mouth. </p> <p dir="ltr">Robert, 41, and James, 34, said they were worried when they saw their dad after the surgery because he couldn’t speak before he reassured them he would be getting better.</p> <p dir="ltr">“When we first went in and saw him, he couldn't actually talk but we're like 'Dad, you're gonna push through. You're gonna fight',” Robert said on <em>A Current Affair</em>. </p> <p dir="ltr">“He kept sticking his fist in the air, going 'yeah'.”</p> <p dir="ltr">The pair mentioned that the first sign that led them down to the harrowing discovery of cancer was when their father had trouble sleeping. </p> <p dir="ltr">“He likes to sleep on his side, and he had a lump in his cheek which mum noticed too and made him go to the dentist,” James said. </p> <p dir="ltr">“The dentist noticed it too and referred him to someone that knew more about it.”</p> <p dir="ltr">The “ulcer” was causing John incredible pain when it rubbed against his cheek and soon enough they were told it was cancer. </p> <p dir="ltr">“It turned out to be cancerous. It's bloody terrifying,” Robert said. </p> <p dir="ltr">“It was unexpected. It wasn't something anyone was prepared for,” James said.</p> <p dir="ltr">The boys credited their mum for her strength and support as the family pushed through the terrifying diagnosis. </p> <p dir="ltr">“Without Mum, I think things would fall apart very, very fast,” Robert said.</p> <p dir="ltr">“She's a very strong woman,” his younger brother James agreed.  </p> <p dir="ltr">The pair also thanked fans for their outpouring of support toward their dad as he went into surgery and during his recovery. </p> <p dir="ltr">“It was kind of comforting to know that Australia was behind him, I know that he appreciated that, I remember when we first told him, he got teary,” James said.</p> <p dir="ltr">“He never expects everyone to still love him. </p> <p dir="ltr">“But they do, so it was nice for him to hear that, thank you everyone.”</p> <p dir="ltr"><em>Images: Getty/A Current Affair</em></p>

John Blackman’s advice to John Farnham following similar diagnosis

Understanding the “window of tolerance” in trauma recovery

<p><span style="font-weight: 400;">For survivors of trauma, recovering involves learning how to cope with distress and how to increase the capacity for positive and enjoyable experiences.</span></p> <p><span style="font-weight: 400;">A common framework psychologists use while working with trauma survivors is called the “window of tolerance”.</span></p> <p><span style="font-weight: 400;">Understanding our personal windows of tolerance can help us respond to demands of daily life and utilise different strategies to return to it during stressful moments.</span></p> <p><strong>What is the window of tolerance?</strong></p> <p><span style="font-weight: 400;">Coined by psychiatrist Dan Spiegel, the </span><a rel="noopener" href="https://www.goodtherapy.org/blog/psychpedia/window-of-tolerance" target="_blank"><span style="font-weight: 400;">window of tolerance</span></a><span style="font-weight: 400;"> describes a state of arousal where a person can function well and respond to stimuli with much difficulty.</span></p> <p><span style="font-weight: 400;">In this state, a person is likely able to think rationally, reflect, and make decisions without feeling overwhelmed.</span></p> <p><span style="font-weight: 400;">When a person experiences extreme stress, they can leave this window and enter a state of hyper- or hypo-arousal.</span></p> <p><span style="font-weight: 400;">Hyper-arousal is also known as the fight or flight response, with a person usually experiencing hypervigilance, anxiety or panic, and racing thoughts.</span></p> <p><span style="font-weight: 400;">In contrast, hypo-arousal is the freeze response, where someone may feel emotionally numb, empty, or paralysed.</span></p> <p><span style="font-weight: 400;">Being in either of these states can mean that a person is unable to effectively process stimuli.</span></p> <p><span style="font-weight: 400;">They may be unable to think as rationally and can feel dysregulated.</span></p> <p><span style="font-weight: 400;">Everyone’s window of tolerance is different and can be affected by their environment - such as how supported they feel - and trauma.</span></p> <p><img style="width: 386.4533965244866px; height: 500px;" src="https://oversixtydev.blob.core.windows.net/media/7844316/nicabm-infog-window-of-tolerance-revised.jpg" alt="" data-udi="umb://media/82982d740495463a868203412a0187d2" /></p> <p><em><span style="font-weight: 400;">Image: NICABM</span></em></p> <p><span style="font-weight: 400;">When a person feels supported and safe, they are generally able to stay in their window of tolerance.</span></p> <p><span style="font-weight: 400;">For those who have experienced trauma, the experience may “push” a person out of their window of tolerance, or make it much more narrow or inflexible.</span></p> <p><span style="font-weight: 400;">This can result in someone responding to even minor stressors with extreme hyper- or hypo-arousal, or believing the world is unsafe.</span></p> <p><span style="font-weight: 400;">Frequently staying outside the window of tolerance can result in an individual experiencing mental health issues such as depression and anxiety.</span></p> <p><strong>Manage mental health with the window of tolerance</strong></p> <p><span style="font-weight: 400;">People who feel dysregulated and often stay outside of their window of tolerance can return using a few different techniques.</span></p> <p><span style="font-weight: 400;">Practicing grounding and mindfulness skills can often help people be present and in the moment.</span></p> <p><span style="font-weight: 400;">With the help of a mental health professional, it is also possible for a person to expand their window of tolerance, feel a greater sense of calm, and become better equipped to deal with stress.</span></p> <p><span style="font-weight: 400;">Therapy can provide individuals with a safe space to process trauma and other painful memories, as well as a place to practice emotional regulation.</span></p> <p><em><span style="font-weight: 400;">Image: Getty Images</span></em></p>

White Island volcano tour guide's miraculous recovery after surviving eruption

White Island volcano eruption: Risky recovery operation begins

<p><span>A New Zealand military team has landed on the volcanic White Island to recover the bodies of eight people killed in Monday’s eruption, the police announced on Friday morning.</span></p> <p><span>The eight-member NZ Defence Force team has arrived on Whakaari/White Island to airlift the bodies off to the naval ship HMNZS Wellington.</span></p> <p><span>Located about 50 kilometres off the east coast of New Zealand’s North Island, the White Island is still “highly volatile” with 50 to 60 per cent chance of erupting in the next 24 hours, geological agency GNS Science said <a href="https://www.gns.cri.nz/Home/News-and-Events/Media-Releases/GNS-Science-maps-show-risk-levels-at-Whakaari-White-Island-still-high-12-12-2019">on Thursday</a>.</span></p> <p><span>“Whakaari/White Island is an active volcano, and the estimated chance of an eruption is increasing every day,” said the agency’s volcanologist Graham Leonard.</span></p> <p><span>“This level of volcanic activity is the highest we’ve seen since the eruption in 2016.”</span></p> <p><span>Six Australians are believed to be among the victims on the island: Karla Matthews and Richard Elzer from Coffs Harbour; Julie and Jessica Richards from Brisbane; Zoe Hosking from Adelaide; and Krystal Browitt from Melbourne.</span></p> <p><span>There were 47 people on the island at the time of eruption. Twenty-four of those were from Australia, nine from the United States, five from New Zealand, four from Germany, two each from China and Britain, and one from Malaysia.</span></p> <p><span>Police said on Thursday two people who were hospitalised from injuries sustained during the eruption had died, bringing the official death toll to eight. Police believe a total of 16 people have died.</span></p> <p><span>New Zealand Prime Minister Jacinda Ardern told ABC Radio National on Friday morning the country’s workplace health and safety regulator WorkSafe was <a href="https://www.theguardian.com/world/live/2019/dec/12/new-zealand-volcano-eruption-white-island-nz-police-victim-recovery-retrieval-operation-whakaari-live-news-latest-updates">investigating the circumstances</a> around the eruption.</span></p> <p><span>“I’m passing no judgement,” she said. “I need that job to be done properly and [families and the community] deserve to have their questions answered.”</span></p>

Michael Schumacher “spotted” in Majorca for first time since 2013

<p>Formula One legend Michael Schumacher has been spotted recovering from his devastating skiing injury in his holiday home in Majorca.</p> <p>Schumacher has not been seen in public since 2013, after a horrific skiing accident left him in a coma for five years.</p> <p>The Formula One legend had people thinking he was being taken care of in his home in Switzerland ahead of a permanent move to Majorca, but it appears he’s spent the holidays at the luxury villa he owns before heading back to his home in Lake Geneva.</p> <p>Schumacher spent Christmas, New Year's and his milestone 50th birthday in Majorca.</p> <p>He was spotted arriving on a helicopter at Port D'Andratx in Majorca, German magazine <a href="https://www.bunte.de/stars/star-life/schicksalsgeschichten-der-stars/michael-schumacher-verrat-corinna.html">Bunte</a> reported.</p> <p>The villa that Schumacher was taken to in the Spanish island’s mountains features two swimming pools, a large garden and a helipad.</p> <p>Schumacher’s family have dismissed suggestions he’s relocating to the island and have said that they’ve bought the home as a holiday home.</p> <p>A German archbishop last month had seen the Formula One legend, saying that he looked the same, but his face had “become a little fuller”.</p> <p>He also approved of the family’s tenacity to keep Schumacher out of the eyes of the public.</p> <p>Georg Ganswein told <a href="https://www.express.co.uk/news/world/1093841/michael-schumacher-latest-news-majorca-holiday-villa-formula-one-f1">The Express</a> that Schumacher himself “senses that loving people are around him, caring for him and keeping the overly curious public away”.</p> <p>His family have kept his health battle extremely private and have said repeatedly that it is not a matter for the public.</p>

How I found strength on my long walk to recovery

<p>On the phone, her voice is calm, clear and full of positivity. You wouldn’t guess that six years ago, Kathleen Jordan was lying in a hospital bed at Royal Melbourne Hospital after suffering a major right haemorrhagic stroke – one of the most severe and deadly forms of stroke.</p> <p>“It was a major bleed,” Kathleen explains, “In fact, one doctor said they’d never seen such a big bleed in anyone that had survived.”</p> <p>The doctors had warned her family to prepare for the fact that she may end up in an aged care ward for the rest of her life, or worse, she could be in a permanent vegetative state.</p> <p>“They actually said to my daughters that they should probably consider an NFR. An NFR means a ‘not-for-resuscitation’ order, but the girls said, ‘No, that’s not gonna happen. She’s going to be fine.’”</p> <p>And Kathleen proved her daughters right. “I’m back doing all the things I used to do. I see my grandchildren, I go to the ballet and the opera, I see my friends, I’m living in the most wonderful retirement complex in Carlton … and I’ve got lots and lots of friends,” she says. “A lot of it is good and I’m very grateful for the life I’ve got.”</p> <p>Prior to the incident, Kathleen led a busy life, running her own leadership coaching business which sent her travelling around Australia and across the world. By contrast, having to spend almost two years recovering in hospital was a huge adjustment.</p> <p>Stroke is one of Australia’s biggest killers and a leading cause of disability. While Kathleen says her blood pressure was under control and there was no clear cause for the stroke, she recalls her doctor warning her to take it easy.</p> <p>“My doctor had been saying to me, ‘You need to slow down a bit, you’re doing a lot.’ And I would always say, ‘But I’m loving what I’m doing.’ I guess it’s when your body is in resolves with your mind really.”</p> <p>Sharon McGowan, CEO of the National Stroke Foundation, says 80 per cent of strokes can be prevented. “The advice is to know your blood pressure and maintain it within normal range, eat well, keep a healthy weight, don’t smoke, keep blood pressure down, exercise regularly and keep alcohol consumption to a minimum.”</p> <p>“Stroke doesn’t discriminate. It can happen to anyone at any age, however risk factors do increase with age,” she adds.</p> <p><strong>Recovering after a stroke</strong><br />For Kathleen, the haemorrhagic stroke resulted in partial paralysis on the left side of her body. She is unable to use her left hand and will have to continue doing physiotherapy, probably for the rest of her life.</p> <p>“Every single day is a struggle day, but I’m not going to give up that struggle because – even six years later – I’m still making improvements,” says Kathleen.</p> <p>“It’s still very hard. Some silly things are difficult, like I was trying to get something out of a package before – and trying to do it with one hand. I was just getting very cross because I couldn’t do it.”</p> <p>The operation to stop the significant bleed in her brain damaged some neurons, sometimes causing Kathleen to search for words when speaking – pausing mid-sentence or often repeating herself.</p> <p>“It’s called aphasia,” explains Kathleen. “However, when I say that to my friends, they all just laugh and say, ‘But we all have trouble finding words!’”</p> <p><strong>Overcoming adversity</strong><br />Kathleen’s resolve and tenacity following her recovery is inspiring, despite her initial fears of being unable to live a normal life again.</p> <p>“I had to just rely on thinking about [my] strengths, and every time I made a little bit of progress, my family and my friends [would] help me celebrate that progress.</p> <p>“For a long time, I couldn’t really walk or sit up without falling over but, with physio and determination, I am now walking around my apartment.”</p> <p>Kathleen actually set in place what she calls her ‘Hope Team’, made up of her close friends and family members. “Whenever I was feeling low, I could ring one of the people in my Hope Team and say, ‘Help me, what do I need to do?’ and they would very quickly give me some encouragement. So, my Hope Team helped me tremendously. They just wanted to support me, even more than what was perhaps expected of them.”</p> <p>In her book, <span><em><a href="http://t.dgm-au.com/c/185116/69171/1880?u=https://www.booktopia.com.au/standing-up--kathleen-with-steggall-vicki-jordan/prod9781925384024.html">Standing Up! My Story of Hope, Advocacy &amp; Survival after Stroke</a></em></span>, Kathleen shares the tools that helped her progress during her stroke journey, and how she achieved resilience and happiness by reframing issues.</p> <p>“I wrote the book for stroke victims and their loved ones. I want to give people hope that they too can recover from stroke, from other illness, by really focusing their minds ... [with] the belief that you can.</p> <p>“Have hope that you can recover and work hard on your physio,” she suggests. “Surround yourself with positive people – perhaps create a ‘Hope Team’ that will help you feel strong and focussed on your recovery.”</p> <p>While the road to recovery can be an uphill battle, there are platforms for the Australian stroke community to discuss and seek support. The <a href="http://www.enableme.org.au/">enableme</a> website is a good place to start. It provides a forum for stroke survivors, carers and loved ones to share their experiences, set recovery goals and gain further knowledge about stroke.</p> <p>“Setting personal recovery goals and self-directed rehabilitation continues to play an important role in Kathleen’s journey after stroke,” says Sharon McGowan of the Stroke Foundation. “[She] is a true inspiration and her experience demonstrates the determination, vital support and services stroke survivors need to live well after stroke. Kathleen shows that there is life after stroke.”</p> <p>Although Kathleen has been able to return to semi-normality, she will have carers for the rest of her life. “Sometimes I got a bit miserable about that,” says Kathleen. “And then I thought, ‘Kathleen, just be grateful for the fact there are carers available, and that they want to do a good job and look after you.’ So that made me feel a lot better. Rather than feeling miserable, just think, ‘What have you got?’”</p> <p>“I am just so happy that my life is back on an even keel,” she adds. “I can’t do everything I used to do – for instance, I can’t drive a car anymore, but I’ve got people who look after [me] and who care for me. Dying golden retriever makes miracle recovery

<p>The sad fact of pet ownership is that while they bring a lot of joy into our lives, we often have to say goodbye to them before we’re ready to, which can be heartbreaking.</p> <p>When Florida couple Joey and Allison Maxwell found out that their beloved eight-year-old golden retriever Maverick was losing his battle with cancer, they were devastated.</p> <p>Mr Maxwell told <span style="text-decoration: underline;"><strong><a href="http://www.9news.com.au/" target="_blank">9news.com.au</a></strong></span>, “We went to the vet and the vet said there was nothing left we could do for him. We were looking at his last few days.</p> <p>“But my wife and I were sitting there depressed, in tears, trying to make sense of it all, and that’s not who we are. So, we wanted to come up with some way to get Maverick out, let him experience life to the fullest one last time.”</p> <p>Maverick has lost the use of his back two legs, so Joey and Allison decided to buy a wagon to pull him around town and let him get some much needed air.</p> <p>While the sight of a golden retriever pulled on a custom-made wagon might seem odd to some, everyone who encounters Maverick showed him a lot of love.</p> <p>But what happened the next day surprised everyone.</p> <p><img width="497" height="360" src="https://oversixtydev.blob.core.windows.net/media/29178/maverick-in-text-2_497x360.jpg" alt="Maverick -in -text -2" style="display: block; margin-left: auto; margin-right: auto;"/></p> <p>Mr Maxwell said, “We were expecting a couple of hard days, but after our trip in the wagon that night, Maverick woke up the next day feeling better.</p> <p>“We took him back to the vet and she did a full exam and it turns out his cancer hasn't spread, he is still in remission and now we’re looking at long term care for the guy.</p> <p>"I really believe that had we gone home that night and sat around the house and cried and let Maverick feel that energy, he would have felt the same and given up."</p> <p>What a lovely story, and an important message about the power of positivity. Expert tips to survive (and thrive) when you’re admitted to hospital

<p><em><strong>Dr Kate Gregorevic is a geriatrician with a research interest in health and lifestyle factors that are associated with healthy ageing and recovery from illness.</strong></em> </p> <p>Hospitalisation is a sentinel event for older adults. Being admitted to hospital, especially if it unplanned for an illness like pneumonia, is very scary. Sometimes the illness can even be life threatening. The majority of people will leave hospital, but for those aged 75 and older around 30-40 per cent will experience functional decline. This is a particular risk for those who are already frail. As a doctor working in the hospital setting, it is heartbreaking to explain this loss of independence to patients and their loved ones. Appropriate medical, nursing and allied health care are critical, but it is not just up to the staff. There are things that you and your family can do to not only survive but make a full recovery following your illness.</p> <p>Frail older adults are at higher risk for adverse outcomes than others. Frailty is a loss of physiological reserve leading to an individual being at risk of new disability. As people become frailer, they often need more help doing day to day tasks, may experience falls and may even begin losing weight. People who have cognitive impairment or dementia are also at increased risk. If you think that this applies to you or your loved one, it is important you let hospital staff know.</p> <p><strong>Eat</strong></p> <p>It is surprisingly easy to become malnourished in hospital. When people are sick, they often don’t have much appetite. Although some hospitals have invested in improving food quality, many hospitals still serve food, which is unfamiliar and unappetising. If you are having surgery or another test you may have to fast. Even in this short time period, inadequate food intake can increase loss of muscle. Protein intake is particularly important. If you are finding that you don't have much of an appetite, it is important to focus on high protein, high kilojoule foods, like full fat yoghurt and milk with sustagen. Ask your family to bring in some favourite meals. There will be a dietician available to help guide these choices, particularly if you have a medical condition like diabetes. If you have been asked to fast, always ask why and find out when you will be able to eat again. By taking these steps to maintain nutrition, you are more likely to recover faster and leave hospital sooner.</p> <p><strong>Move</strong></p> <p>When you are unwell, it is hard to contemplate getting out of bed and walking, but this is critical for recovery. There are often restrictions on walking in hospital to minimise the risks of falls. The physiotherapist is the best person on the team to discuss safe mobility. With some guidance from the physiotherapist, it is important that you mobilise as much as you can. Even just sitting out of bed will enable you to breathe more deeply and ward off the dreaded pneumonia. Basically the more you walk in hospital, the faster you will walk out!</p> <p><strong>Ask questions</strong></p> <p>Doctors and nurses are very bad at recognising when they are using medical jargon. They spend so much time working and socialising with other medical people that they just don't realise they are using technical language. Many people are intimidated by the hospital environment and fear that asking for explanations will make them look silly, but it is actually the responsibility of the healthcare professionals to explain things in terms everyone can understand. If you don’t understand your illness or your treatments, keep asking questions until you do. The better you understand what’s happening to you, the better you can take an active role in managing your recovery.</p> <p><strong>Have the difficult conversations</strong></p> <p>No one likes to think about the worst things happening when they become unwell and are admitted for hospital. An illness is often a time of great reflection. There is increasing awareness of advanced care planning in the community. This means recording your wishes and values regarding healthcare and quality of life so that if you are not able to make decisions regarding your medical treatment, other people can still act according to what you would want. If you feel that for quality of life, there are certain things that are important to you, like not being kept alive in a vegetative state or completely dependent for all care, make sure those you love know this, to give you peace of mind.</p> <p><strong>Advice for friends and family</strong></p> <p>What if it’s not you, but your loved one who is admitted to hospital? There are key things that only family can do to aid recovery.</p> <p><span style="text-decoration: underline;">Watch out for delirium</span></p> <p>Frail older adults are particularly vulnerable in the hospital environment. Around 30-40 per cent of older adults will experience delirium. Delirium is a sudden onset of confusion and a change in alertness. The confusion can fluctuate throughout the day. This can happen before or during the hospital stay. Some people will be more agitated some will be sleepier. The biggest risk factor for delirium is pre-existing impaired cognition, particularly dementia.</p> <p>Delirium can be difficult to identify and diagnose. Patients may describe odd stories, like policemen coming into the room in the middle of the night, little children running around or insects. They may seem to pluck at the air, or drift off in the middle of a conversation. The type of delirium most likely to go unnoticed is hypoactive delirium, where someone becomes very sleepy. Medical staff need to be on high alert for any of these symptoms, but often it is the patient’s family and friends who are first to notice that the person they love is not their usual self.</p> <p><span style="text-decoration: underline;">Advocate</span></p> <p>It is very hard to see someone you love struggling with an illness. Admission to hospital is incredibly stressful for the patient and their loved ones. For a frail older person who is feeling sick and stressed, it can be very difficult to speak up if they have concerns. The friends and relatives will often be the first to notice something is not right. If your loved one seems weaker than prior to hospital, more confused or you’re just not sure they are getting better, speak up.</p> <p>It is impossible to overstate how important friends and family are to recovery. With so much focus on the physical aspects of illness, it is easy to forget the importance of emotional support – just visiting, bringing some favourite foods and sharing some time with your loved one, may be enough to help them walk back out of hospital.</p> <p>We as health professionals have a long way to go to make hospitals a better environment for older adults. The good news is that there is increasing recognition within hospitals of the importance of providing specialised care to older adults, and steps are being taken to improve processes and environment.</p> <p>It is quite disempowering to be dressed in a hospital gown, feeling unwell in a hospital bed and not really understanding what is going on, but you are not a passive agent in your recovery.
