Loneliness could kill you

<p><em><a href="https://theconversation.com/profiles/gillian-leithman-414232">Gillian Leithman</a>, <a href="https://theconversation.com/institutions/concordia-university-1183">Concordia University</a></em></p> <p>Independence is glorified in North American culture as a symbol of strength. As a society, we value individual achievement and extol self-reliance.</p> <p>I am an expert on aging and retirement and I also help employees transition from work to retirement by facilitating seminars and workshops in corporate Canada. And I often wonder however if our “go at it alone” attitude has led us down a lonely and isolating path.</p> <p>Here are some recent stats:</p> <ul> <li> <p>40 per cent of Americans <a href="https://hbr.org/cover-story/2017/09/work-and-the-loneliness-epidemic">don’t feel close to others</a> at any given time. And the number of lonely Americans has doubled since the 1980s.</p> </li> <li> <p><a href="http://www.carp.ca/2017/06/05/loneliness-survey-results/">In a recent Canadian Association of Retired People poll</a>, 16 per cent of Canadians indicated that they lacked companionship</p> </li> <li> <p>Fifteen per cent in the CARP poll said they had nobody to turn to or talk to</p> </li> <li> <p>Fifteen per cent were unhappy doing things alone.</p> </li> </ul> <p>I suspect that these numbers are even higher among the general Canadian population, not just CARP members.</p> <p>According to science, loneliness shortens our lifespan. <a href="https://news.uchicago.edu/article/2014/02/16/aaas-2014-loneliness-major-health-risk-older-adults">Twice as much as obesity.</a> Yes, you read that right.</p> <p>Dr. John <a href="http://psychology.uchicago.edu/people/faculty/cacioppo/">Cacioppo</a>, the world’s foremost authority on loneliness, maintains that the number of people in your life does not inoculate you from experiencing loneliness. Rather, it’s the feeling of being lonely that places the brain and body at risk.</p> <p>Cacioppo equates feeling lonely with feeling hungry. We compromise our survival and well-being when either is ignored.</p> <p>We are biologically hardwired to respond to our environment. When we experience low blood-sugar levels, we crave food. The feeling of our stomachs being empty is a warning sign to eat and it’s essential to our very survival.</p> <p>When we feel lonely, we desire connection with others, much like the loud rumble that your tummy makes when hungry.</p> <h2>A lonely brain is restless</h2> <p>Loneliness triggers “hyper-vigilance.” That is your brain is on the lookout for social threats, which consequently puts us on the defensive. We become more reactive to negative events and perceive daily hassles as more stressful.</p> <p>A lonely brain awakens often, experiences fragmented sleep and cannot recover from the day’s stressful events.</p> <p>A lonely brain is also subject to an increase in depressive symptoms and has difficulty self-regulating. That is why you may find yourself irritable and impulsive.</p> <p>A lonely brain is also at risk of cognitive and physical decline.</p> <p><a href="https://www.ncbi.nlm.nih.gov/pubmed/23232034">A three-year Dutch study</a> followed more than 2,000 participants aged 65 to 86. While none of the participants had signs of dementia at the outset of the study, results revealed that those who reported feeling lonely had a 64 per cent increase in the risk of developing dementia.</p> <p>People also experience an increase in loneliness when they retire from work. That’s why you want <a href="http://rewiretoretire.com/delay-retirement/">to make sure that you’re retiring <em>to</em> something</a>, and that you have friends outside of your place of employment.</p> <h2>A lonely body</h2> <p>Loneliness also affects the body. Psychologist <a href="https://irp.nih.gov/pi/stephen-suomi">Stephen Suomi’s</a> research indicates that loneliness distorts the expression of certain genes. An experiment separating newborn primates from their mothers during their first four months of life resulted in the altered development of immunity-related genes that help the body fight viruses.</p> <p>Social psychologist <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3633610/">Lisa Jaremka’s research</a> indicates that lonely people have higher levels of activated viruses in their system and are at greater risk of suffering from chronic inflammation, which has been linked to Type 2 diabetes, arthritis, heart disease and even suicide.</p> <p>While obesity increases your odds of an early death by 20 per cent, loneliness increases <a href="https://www.ucsf.edu/news/2012/06/12184/loneliness-linked-serious-health-problems-and-death-among-elderly">your odds by 45 per cent</a>.</p> <p>What are we to do with an emotional state that is so powerful that it can alter our brains, compromise our physiology and cut short our longevity?</p> <h2>The antidote to loneliness</h2> <ol> <li> <p>Seek out connection: We all need a tribe!</p> </li> <li> <p>Stop denying and accept “feeling lonely” as simply a craving for connection.</p> </li> <li> <p>Acknowledge the consequences of prolonged loneliness. If you ignore hunger, you starve. Same is true of our need for belonging. Hospice nurse shares the four physical stages of dying

<p>A hospice nurse has shared the four things that happen to your body in the months, weeks and days before you die. </p> <p>Julie McFadden, who specialises in end of life care, shares videos about death and dying on social media to open up the conversation on the taboo topic, to help better prepare people for death. </p> <p>In her latest video, a viewer asked Julie what the dying process actually looks like, as the nurse explained that it all depends on how, when and why you pass away. </p> <p>However, she said there are four things that happen to the body as the end draws near. </p> <p>The first stage of dying is slowing down, which can happen up to six months before you die, with the symptoms being very "generalised". </p> <p><span style="font-size: 16px; font-family: -apple-system, system-ui, BlinkMacSystemFont, 'Segoe UI', Roboto, 'Helvetica Neue', Arial, sans-serif; background-color: #ffffff;">Julie says, "For instance, you’re just going to be generally tired, generally lethargic, not eating and drinking as much, probably being less social."</span></p> <p><span style="background-color: #ffffff;"><span style="font-family: -apple-system, system-ui, BlinkMacSystemFont, Segoe UI, Roboto, Helvetica Neue, Arial, sans-serif;">According to Julie, the second stage is a sharp decline in strength, as she explains, "</span></span><span style="font-family: -apple-system, system-ui, BlinkMacSystemFont, 'Segoe UI', Roboto, 'Helvetica Neue', Arial, sans-serif; font-size: 1rem;">The closer you get to death – let’s say three months out – you’re going to be more debilitated."</span></p> <p><span style="font-family: -apple-system, system-ui, BlinkMacSystemFont, 'Segoe UI', Roboto, 'Helvetica Neue', Arial, sans-serif; font-size: 1rem;">"It’s going to be difficult for you to leave the house, you probably are eating and drinking very little throughout the day, and you’re sleeping more than you’re awake."</span></p> <p style="font-size: 1rem; font-family: -apple-system, system-ui, BlinkMacSystemFont, 'Segoe UI', Roboto, 'Helvetica Neue', Arial, sans-serif; background-color: #ffffff;">She says that typically, someone "can be up and having a normal conversation with their family", all the while "saying they’re seeing their dead father in the corner who is smiling and telling them he’s coming to get them soon and not to worry."</span></p> <p style="font-size: 1rem; font-optical-sizing: inherit; font-variation-settings: inherit; margin: 0px 0px 1.4rem; padding: 0px; vertical-align: baseline;">‘Metabolic changes’ such as a difference in skin colour, high and low temperature, or the ‘death rattle’ – a gurgling noise (also known as terminal secretions) caused by a buildup of fluids in the throat and upper airways – follow before they later pass on.</p> <p style="font-size: 1rem; You don’t need a doctor to get more physically active – here are 10 simple steps you can take by yourself

<div class="theconversation-article-body"><em><a href="https://theconversation.com/profiles/nerys-m-astbury-410114">Nerys M Astbury</a>, <a href="https://theconversation.com/institutions/university-of-oxford-1260">University of Oxford</a></em></p> <p>We all know physical activity has many <a href="https://www.nhs.uk/live-well/exercise/exercise-health-benefits/">health benefits</a>, including for mental health. It helps <a href="https://theconversation.com/exercise-can-reduce-stress-and-improve-sleep-particularly-for-women-with-breast-cancer-186144">manage stress</a>, ease joint or back pain, and boost energy levels.</p> <p>Exercise can also improve <a href="https://theconversation.com/exercise-and-the-brain-three-ways-physical-activity-changes-its-very-structure-150203">brain function</a> and <a href="https://theconversation.com/exercise-really-can-help-you-sleep-better-at-night-heres-why-that-may-be-192427">sleep</a>, and lift mood. In contrast, inactivity or spending too much time <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6308180/">sedentary</a> is a leading factor in developing a range of diseases.</p> <p>The <a href="https://www.who.int/publications/i/item/9789240015128">World Health Organization</a> recommends we should do a weekly minimum of 150-300 minutes of moderate intensity physical activity, such as walking, or 75 minutes of vigorous physical activity, such as swimming, jogging or an exercise class – as well as <a href="https://theconversation.com/strength-training-could-be-the-answer-to-one-of-the-worlds-worst-killers-228665">regular strength training</a>.</p> <p>However, many people <a href="https://www.who.int/teams/health-promotion/physical-activity/global-status-report-on-physical-activity-2022">fail to meet these guidelines</a>. So what to do about this <a href="https://www.weforum.org/agenda/2022/12/lack-exercise-inactivity-preventable-diseases/">health crisis</a>?</p> <p>There is already <a href="https://www.bmj.com/content/376/bmj-2021-068465">evidence</a> that when GPs give patients guidance and continued support to increase physical activity, this encourages them to be more physically active – at least in the short term. However, we don’t yet know the best way for doctors to communicate with patients to help them sustain these increased activity levels so the current guidance and support on offer to patients isn’t as effective as it could be.</p> <figure><iframe src="https://www.youtube.com/embed/vCCD1xHKpZc?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <p>For example, my <a href="https://www.bmj.com/content/386/bmj-2023-078713">latest research</a> examines the <a href="https://www.bmj.com/content/340/bmj.c1900">“motivational interviewing” (MI)</a> method GPs currently use to encourage patients to change their lifestyle. MI is a patient-centred, non-confrontational communication style that helps patients address any problem behaviour by exploring their ambivalence towards changing it. MI has been shown to help patients with a host of health problems, including <a href="https://pubmed.ncbi.nlm.nih.gov/25577724/">addiction issues</a>, <a href="https://pubmed.ncbi.nlm.nih.gov/23001832/">eating disorders</a>, <a href="https://pubmed.ncbi.nlm.nih.gov/25726920/">smokers</a> and those with <a href="https://pubmed.ncbi.nlm.nih.gov/33637368/">diabetes</a> to change their behaviour.</p> <p>However, I found that while MI programmes can help patients increase their total amount of physical activity – the benefits are only short term.</p> <h2>Ten simple ways to be more physically active</h2> <p>If you want more physical activity in your life, then, there are many self-directed things you can do to help yourself, without joining a programme or seeing your GP.</p> <p>Here are ten simple and effective ways to help you become – and stay – more physically active:</p> <p><strong>1) Don’t sit, stand</strong></p> <p>We <a href="https://theconversation.com/sitting-is-bad-for-your-health-and-exercise-doesnt-seem-to-offset-the-harmful-effects-225056">sit a lot</a>. In fact, it’s likely you’re sitting right now – and you needn’t be. Sitting for long periods has been <a href="https://doi.org/10.1016/j.amepre.2010.05.024">linked</a> with many adverse health outcomes, so try to stand more.</p> <figure><iframe src="https://www.youtube.com/embed/wUEl8KrMz14?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <p><strong>2) Take the stairs</strong></p> <p>Being physically active needn’t mean expensive gym memberships. Try building physical activity into your daily routine. One easy way to do this is by swapping the lift or escalator for the stairs.</p> <p><strong>3) Make it fun</strong></p> <p>If you like doing something, you’re <a href="https://theconversation.com/why-you-shouldnt-let-guilt-motivate-you-to-exercise-220342">more likely</a> to continue doing it. Why not try an activity you liked doing as a child, or even something new? Who knows, you might enjoy it.</p> <p><strong>4) Phone a friend</strong></p> <p>Exercising <a href="https://theconversation.com/exercise-can-be-punishing-but-heres-how-to-stop-thinking-of-it-as-a-punishment-76167">with a friend</a> or loved one is a great way to stay motivated, and it can make physical activity more fun too.</p> <p><strong>5) Do less, more often</strong></p> <p><a href="https://pilotfeasibilitystudies.biomedcentral.com/articles/10.1186/s40814-023-01272-8">“Snacktivity”</a> – a term for breaking up your activity into shorter <a href="https://theconversation.com/forget-the-gym-in-january-exercise-snacking-is-the-way-forward-69702">activity “snacks”</a> – can help you increase activity in convenient, manageable bursts while reaping the health benefits.</p> <p><strong>6) Track your progress</strong></p> <p>Activity trackers aren’t a fad. There is <a href="https://doi.org/10.1016/S2589-7500(22)00111-X">evidence</a> that just using an activity tracker such as a pedometer to count steps or a smart watch that logs activity can help increase your activity levels, reduce body fat and increase muscle mass – and increase your overall physical fitness.</p> <p><strong>7) Get into a habit</strong></p> <p>We know it takes about ten weeks to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3505409/">form a habit</a>. Repetition is key – so stick with it and keep going. Once you’ve formed a physical activity habit, it will be <a href="https://www.psychologytoday.com/gb/basics/habit-formation#:%7E:text=Building%20healthy%20habits%20can%20involve,listening%20to%20music%20while%20exercising">hard to shake it off</a>.).</p> <p><strong>8) Hold still</strong></p> <p>Try to incorporate <a href="https://en.wikipedia.org/wiki/Isometric_exercise">isometric exercises</a> like the plank or wall squats into your routine. These exercises, which need no equipment, require you to tighten muscles and hold still – and have been shown to <a href="https://bjsm.bmj.com/content/57/20/1317">lower your blood pressure</a>.</p> <p><strong>9) Set a goal</strong></p> <p>Give yourself an achievable target to work towards – it will <a href="https://theconversation.com/three-tips-to-help-you-stay-motivated-to-keep-exercising-all-year-long-175868">motivate you</a> to reach your goal.</p> <p><strong>10) Reward yourself</strong></p> <p>And don’t forget to reward yourself when you meet that goal. You can also build in rewards to mark your progress along the way. After all, who doesn’t like to treat themselves when they’ve done well?<!-- Below is The Conversation's page counter tag. How do airplanes fly? An aerospace engineer explains the physics of flight

The <a href="https://airandspace.si.edu/explore/stories/wright-brothers">invention of the airplane</a> allows people to travel from one side of the planet to the other in less than a day, compared with weeks of travel by boat and train.</p> <p>Understanding precisely why airplanes fly is an ongoing challenge for <a href="https://www.clarkson.edu/people/craig-merrett">aerospace engineers, like me</a>, who study and design airplanes, rockets, satellites, helicopters and space capsules.</p> <p>Our job is to make sure that flying through the air or in space is safe and reliable, by using tools and ideas from science and mathematics, like computer simulations and experiments.</p> <p>Because of that work, flying in an airplane is <a href="https://usafacts.org/articles/is-flying-safer-than-driving/">the safest way to travel</a> – safer than cars, buses, trains or boats. Engineers use these forces to help design the shape of the airplane, the size of the wings, and figure out how many passengers the airplane can carry.</p> <p>For example, when an airplane takes off, the thrust must be greater than the drag, and the lift must be greater than the weight. If you watch an airplane take off, you’ll see the wings change shape using flaps from the back of the wings. The flaps help make more lift, but they also make more drag, so a powerful engine is necessary to create more thrust.</p> <p>When the airplane is high enough and is cruising to your destination, lift needs to balance the weight, and the thrust needs to balance the drag. So the pilot pulls the flaps in and can set the engine to produce less power.</p> <p>That said, let’s define what force means. According to <a href="https://ca.pbslearningmedia.org/resource/4079abf0-7a4b-4f49-80ad-c69cd06a80f9/newtons-second-law-of-motion/">Newton’s Second Law</a>, a force is a mass multiplied by an acceleration, or F = ma.</p> <p>A force that everyone encounters every day is <a href="https://spaceplace.nasa.gov/what-is-gravity/en/#:%7E">the force of gravity</a>, which keeps us on the ground. When you get weighed at the doctor’s office, they’re actually measuring the amount of force that your body applies to the scale. When your weight is given in pounds, that is a measure of force.</p> <p>While an airplane is flying, gravity is pulling the airplane down. That force is the weight of the airplane.</p> <p>But its engines push the airplane forward because they create <a href="https://www1.grc.nasa.gov/beginners-guide-to-aeronautics/what-is-thrust/">a force called thrust</a>. The engines pull in air, which has mass, and quickly push that air out of the back of the engine – so there’s a mass multiplied by an acceleration.</p> <p>According to <a href="https://www.youtube.com/watch?v=a-wh3fJRdjo">Newton’s Third Law</a>, for every action there’s an equal and opposite reaction. When the air rushes out the back of the engines, there is a reaction force that pushes the airplane forward – that’s called thrust.</p> <p>As the airplane flies through the air, the shape of the airplane pushes air out of the way. Again, by Newton’s Third Law, this air pushes back, <a href="https://www1.grc.nasa.gov/beginners-guide-to-aeronautics/what-is-drag/#:%7E">which leads to drag</a>.</p> <p>You can experience something similar to drag when swimming. Paddle through a pool, and your arms and feet provide thrust. Stop paddling, and you will keep moving forward because you have mass, but you will slow down. The reason that you slow down is that the water is pushing back on you – that’s drag.</p> <h2>Understanding lift</h2> <p><a href="https://www1.grc.nasa.gov/beginners-guide-to-aeronautics/what-is-lift/">Lift</a> is more complicated than the other forces of weight, thrust and drag. It’s created by the wings of an airplane, and the shape of the wing is critical; that shape is <a href="https://howthingsfly.si.edu/media/airfoil#:%7E">known as an airfoil</a>. Basically it means the top and bottom of the wing are curved, although the shapes of the curves can be different from each other.</p> <p>As air flows around the airfoil, <a href="https://www.youtube.com/watch?v=UO75jDwGCdQ">it creates pressure</a> – a force spread out over a large area. Lower pressure is created on the top of the airfoil compared to the pressure on the bottom. Or to look at it another way, air travels faster over the top of the airfoil than beneath.</p> <p>Understanding why the pressure and speeds are different on the top and the bottom is <a href="https://airandspace.si.edu/multimedia-gallery/lift-and-copjpg">critical to understand lift</a>. We can create models of different wings to predict if they will fly well. We can also change lift by changing a wing’s shape to create airplanes that fly for long distances or fly very fast.</p> <p>Even though we still don’t fully know why lift happens, aerospace engineers work with mathematical equations that recreate the different speeds on the top and bottom of the airfoil. Those equations describe a process <a href="https://howthingsfly.si.edu/media/circulation-theory-lift">known as circulation</a>.</p> <p>Circulation provides aerospace engineers with a way to model what happens around a wing even if we do not completely understand why it happens. In other words, through the use of math and science, we are able to build airplanes that are safe and efficient, even if we don’t completely understand the process behind why it works.</p> <p>Ultimately, if aerospace engineers can figure out why the air flows at different speeds depending on which side of the wing it’s on, we can design airplanes that use less fuel and pollute less.</p> <p><!-- End of code. "I want answers": Simon Dorante-Day's cunning plan to prove royal connections

<p>The Queensland man claiming to be the son of King Charles and Queen Camilla has shared his new plan to prove once and for all that he has royal family connections. </p> <p>Simon Dorante-Day has long claimed through his 30 years of research, he discovered that he is the illegitimate son of the monarch, and has attempted to prove his theory through various means. </p> <p>Now, his new plan to get his hands on royal DNA to unequivocally prove his family heritage involves another member of the royal family. </p> <p>Speaking with <a href="https://7news.com.au/lifestyle/aussie-man-claiming-to-be-charles-son-drops-new-prince-harry-dna-bombshell-c-13488374" target="_blank" rel="noopener"><em>7News</em></a>, the 57-year-old said he and his wife have been told by a “trusted source” that Prince Harry could be amenable to helping them gather proof.</p> <p>“We got advice from a really good, really trusted source that we should approach Harry and make a connection there and ask for DNA,” Elvianna said.</p> <p>Simon added, “I was really taken aback when he said that. But it’s definitely something I’m going to do. I’ve long been a supporter of Harry and Meghan, I think it’s disgraceful the way they’ve been treated by the royal family."</p> <p>“The way they are treated on social media too, it’s just one big ‘Punch-Meghan-and-Harry-a-thon’, seriously. It’s really starting to annoy me."</p> <p>He said he is planning to "make contact with him and explore this as an opportunity," pointing out that the royal family's treatment of Harry could make him more willing to help.</p> <p>"At the end of the day, I feel like he just might be as keen as I am to expose what Charles and Camilla and the powers that be have done to me. The injustice."</p> <p>“It’s worth me pointing out that Charles and Camilla, Buckingham Palace, the entire royal family - not one of them has ever said my claims are not true. Steep physical decline with age is not inevitable – here’s how strength training can change the trajectory

<p><em><a href="https://theconversation.com/profiles/zachary-gillen-1251178">Zachary Gillen</a>, <a href="https://theconversation.com/institutions/mississippi-state-university-1970">Mississippi State University</a></em></p> <p>Raise your hand if you regularly find yourself walking up a flight of stairs. What about carrying heavy bags of groceries? How about picking up your child or grandchild? Most of us would raise our hands to doing at least one of those weekly, or even daily.</p> <p>As people age, it can become more and more difficult to perform some physical tasks, even those that are normal activities of daily living. However, prioritizing physical fitness and health as you get older can help you go through your normal day-to-day routine without feeling physically exhausted at the end of the day.</p> <p>It can also help you continue to have special memories with your family and loved ones that you might not have been able to have if you weren’t physically active. For example, I ran two half-marathons with my dad when he was in his 60s!</p> <p>I am an exercise physiologist who studies how people can <a href="https://scholar.google.com/citations?user=gn8ZiLMAAAAJ&amp;hl=en">use resistance training to improve human performance</a>, whether it be in sports and other recreational settings, in everyday life, or both. I am also a certified strength and conditioning specialist. My career has given me the opportunity to design exercise programs for kids, college athletes and elderly adults.</p> <p>Staying physically active as you get older doesn’t need to include running a half-marathon or trying to be a bodybuilder; it could be as simple as trying to get through the day without feeling winded after you go up a flight of stairs. Although our muscles naturally get weaker as we age, there are ways we can combat that to help improve quality of life as we get older.</p> <h2>Muscle loss and chronic disease</h2> <p>One of the most important parts of exercise programming, no matter who I am working with, is proper resistance training to build muscle strength. Some amount of age-related loss of muscle function is normal and inevitable. But by incorporating resistance training that is appropriate and safe at any ability level, you can slow down the rate of decline and even prevent some loss of muscle function.</p> <p>The medical term for a condition that involves <a href="https://doi.org/10.1093/ageing/afy169">age-related loss of muscle function and mass is sarcopenia</a>. Sarcopenia can begin as early as age 40, but it tends to be <a href="https://doi.org/10.1097/01.mco.0000134362.76653.b2">more common in adults age 60 and older</a>. Sarcopenia is associated with a number of health issues such as <a href="https://doi.org/10.1093/gerona/glx245">increased risk of falling</a>, <a href="https://doi.org/10.1161/CIRCULATIONAHA.123.064071">cardiovascular disease</a> and <a href="https://doi.org/10.3390/nu13103519">metabolic disease</a>, among others.</p> <p>In one of our team’s previous studies, we saw that otherwise healthy individuals with sarcopenia had issues <a href="https://doi.org/10.1002/jcsm.12932">delivering vital nutrients to muscle</a>. This could lead to greater likelihood of various diseases, such as Type 2 diabetes, and slow down recovery from exercise.</p> <p>Recent estimates suggest that sarcopenia affects <a href="https://doi.org/10.1016/j.metabol.2023.155533">10% to 16% of the elderly population worldwide</a>. But even if a person doesn’t have clinically diagnosed sarcopenia, they may still have some of the underlying symptoms that, if not dealt with, could lead to sarcopenia.</p> <h2>Strength training is key</h2> <p>So the question is, what can be done to reverse this decline?</p> <p>Recent evidence suggests that one of the key factors leading to sarcopenia is <a href="https://doi.org/10.1093/gerona/glx245">low muscle strength</a>. In other words, combating or reversing sarcopenia, or both, may be best done with a proper resistance-training program that <a href="https://doi.org/10.1186/s12877-021-02642-8">prioritizes improving strength</a>. The best way to make sure you are strength training properly is to seek out guidance from a qualified individual such as a personal trainer or strength and conditioning specialist.</p> <p>Despite the clear benefits of strength training, it’s been shown that only about 13% of Americans age 50 and older do some form of <a href="https://pubmed.ncbi.nlm.nih.gov/17572957/">strength training at least twice a week</a>.</p> <h2>Finding what works for you</h2> <p>So how does a person properly strength train as they age?</p> <p>The National Strength and Conditioning Association, a leading organization in advancing strength and conditioning around the world, states that for older adults, <a href="https://doi.org/10.1519/JSC.0000000000003230">two to three days per week of strength training</a> can be incredibly helpful for maintaining healthy muscle and bone and combating a number of chronic conditions.</p> <p>The organization recommends that these workouts involve one to two exercises involving multiple joints per major muscle group, with six to 12 repetitions per set. These are done at an intensity of 50% to 85% of what’s known as one-repetition maximum – the most weight you could handle for a single repetition – with the exception of body weight exercises that use one’s own body weight as the resistance, such as pushups.</p> <p>I would also recommend resting for about two to three minutes between sets, or even up to five minutes if the set was challenging. For older adults, particularly those age 60 and older, the National Strength and Conditioning Association guidelines suggest that a program like this be performed two to three days per week, with 24 to 48 hours between sessions.</p> <p><iframe id="sGvo5" class="tc-infographic-datawrapper" style="border: none;" src="https://datawrapper.dwcdn.net/sGvo5/3/" width="100%" height="400px" frameborder="0"></iframe></p> <h2>Making life’s tasks lighter</h2> <p>The guidelines above are only one example out of many options, but they provide a framework that you can use to build your own program. However, I would highly recommend seeking out a professional in the field to give specific exercise programming advice that can be tailored to your own needs and goals as you age.</p> <p>Following such a program would give your muscles an excellent stimulus to enhance strength, while also allowing enough recovery, a very important consideration as people age. You might think it looks like a huge time commitment, but an exercise routine like this can be done in less than an hour. This means that in less than three hours of strength training per week you can help improve your muscle health and reduce the risk of getting sarcopenia and associated health issues.</p> <p>It’s also important to note that there is no one right way to do resistance training, and it needn’t involve traditional weight equipment. Group classes like Pilates and yoga or those that involve circuit training and work with resistance bands can all produce similar results. The key is to get out and exercise regularly, whatever that entails.<!-- Below is The Conversation's page counter tag. Homesick for ourselves – the hidden grief of ageing

<p><em><a href="https://theconversation.com/profiles/carol-lefevre-1341823">Carol Lefevre</a>, <a href="https://theconversation.com/institutions/university-of-adelaide-1119">University of Adelaide</a></em></p> <p>Anyone parenting young children will be familiar with the phrase “there’ll be tears before bedtime”. But in a quieter, more private way, the expression seems perfectly pitched to describe the largely hidden grief of ageing.</p> <p>Not the sharp grief that follows a bereavement (though bereavements do accumulate with the years), but a more elusive emotion. One that is, perhaps, closest to the bone-gnawing sorrow of homesickness.</p> <p>Sarah Manguso <a href="https://www.panmacmillan.com.au/9781509883295/">evokes</a> this sense of having travelled further from our younger selves than we could ever have imagined: "Sometimes I feel a twinge, a memory of youthful promise, and wonder how I got here, of all the places I could have got to."</p> <p>Historically, the phenomenon of homesickness was identified in 1688 by the Swiss medical student <a href="https://www.theatlantic.com/health/archive/2013/08/when-nostalgia-was-a-disease/278648/">Johannes Hofer</a>, who named it nostalgia from the Greek <em>nostos</em>, meaning homecoming, and <em>algos</em>, meaning an ache, pain, grief and distress.</p> <p>It was the disease of soldiers, sailors, convicts and slaves. And it was particularly associated with soldiers of the Swiss army, who served as mercenaries and among whom it was said that a well-known milking song could bring on a fatal longing. (So singing or playing that song was made punishable by death.) Bagpipes stirred the same debilitating nostalgia in Scottish soldiers.</p> <p>Deaths from homesickness were recorded, but the only effective treatment was to send the afflicted person back to wherever they belonged.</p> <p>The nostalgia associated with old age, if it occurs, appears incurable, since there can be no possibility of a return to an irrecoverable youth. But as with homesickness, how badly those afflicted suffer seems to depend on how they manage their relationship with the past.</p> <h2>The phantom was me</h2> <p>American writer Cheryl Strayed <a href="https://cherylstrayed.substack.com/p/what-you-know-changes">describes</a> deciding to transcribe her old journals. On reading one of them from cover to cover, she is left feeling:</p> <p>"kind of sick for the rest of the day, as if I’d been visited by a phantom who both buoyed and scared the bejesus out of me. And the weirdest of all is that phantom was me! Did I even know her anymore? Where did the woman who’d written those words go? How did she become me?"</p> <p>I’ve experienced a similar rush of bafflement and grief upon opening a letter I’d written some time before I turned 50. My mother had saved it and returned it to me 20 years later. Within its pages I found a younger, more energetic and vibrant self. The realisation this woman who inhabited the letter so vividly was no longer available to me came with a jolt of emotion that felt like a bereavement.</p> <p>I was so knocked off-kilter by this ghost-like encounter that the letter (along with others I had been planning to transcribe) had to be set aside for a day when I might be able to muster the necessary courage and detachment. Whether that day ever comes will depend, I suppose, on how I navigate my own relationship with time, and on reaching a calm acceptance of the distance travelled.</p> <p>Disbelief at the distance between the young self and the old self is one of the factors in this late-life grieving. At its root, perhaps, is an internalised ageism: innate, or else massaged into us by the culture we spring from.</p> <p>In a series of recent conversations with people over 70, I encouraged them to tell their stories and to reflect on the effects of time on their lives. Childhood sometimes emerged as a place they were pleased to have left behind – and occasionally, as a place to be held close.</p> <p>Trevor emigrated alone to Australia when he was just 18. I asked him how often now, at 75, he thinks about his childhood. “Do you have a sense of who you were back then, and is that person still part of who you are?”</p> <p>“I think about my childhood quite a lot, especially putting some distance between where I was then and where I am now,” he told me. “I didn’t have a really happy upbringing, and coming to Australia was a way of getting away from home and experiencing a new culture.”</p> <p>In response to the same question, Jo, at 84, led me to a framed photograph, enlarged to poster-size, which has hung on the wall of both his homes. It shows him aged three, in a garden – a radiant child wearing a plain white shirt and dark shorts, arms out-flung as if to embrace the natural world. He bursts with exuberance, curiosity, and joy.</p> <p>"I relate to that as an idea, as a concept of my life. I want to maintain that freshness, that child-like freshness. You’ve got no responsibilities; every day is a new day. You’re looking at things in a different light, you’re aware of everything around you. That’s what I wanted to maintain, that feeling through my life – I’m talking age-wise. My concept of my ageing is there in that photograph.“</p> <p>While older voices are often absent in the media, and in fiction they are too often presented as stereotypes, in conversation what arises can both surprise and inspire.</p> <h2>‘How can I be old?’</h2> <p>As I approached my own 70th birthday, I realised I was about to cross a border. Once I was on the other side, I would be old – no question. Yet the word "old”, especially when coupled with the word “woman”, is carefully avoided in our culture. Old is a country no one wants to visit.</p> <p><a href="https://www.penguin.com.au/books/metamorphosis-9780241514771">Penelope Lively’s</a> novella-length story Metamorphosis, or the Elephant’s Foot, written when Lively was in her mid-eighties, explores this evolution from youth to old age through the character of Harriet Mayfield. As a nine-year-old, Harriet is reprimanded by her mother for not behaving well on a visit to her great-grandmother.</p> <p>“She’s old,” says Harriet. “I don’t like old.”</p> <p>When her mother points out that one day Harriet, too, will be old, like her great-grandmother, Harriet laughs.</p> <p>“No, I won’t. You’re just being silly,” says Harriet “how can I be old? I’m me.”</p> <p>Towards the end of the story, Harriet is 82 and must somehow accept that she is “in the departure lounge. Check-in was a very long time ago.” With her equally elderly husband, Charles, Harriet ponders what they can do with the time remaining. Charles decides “it’s a question of resources. What do we have that could be used – exploited?” Harriet replies, “Experience. That’s it. A whole bank of experience.”</p> <p>“And experience is versatile stuff. Comes in all shapes and sizes. Personal. Collective. Well, then?”</p> <p>If distance travelled is a factor in late-life grief, so too is a sense of paths not taken: of a younger self, or selves, that never found expression.</p> <p>In Jessica Au’s recent, much-awarded novella <a href="https://giramondopublishing.com/books/jessica-au-cold-enough-for-snow/">Cold Enough For Snow</a>, there is a scene where the narrator explains to her mother the existence, in some old paintings, of a <a href="https://www.britannica.com/art/pentimento-oil-painting">pentimento</a> – an earlier image of something the artist had decided to paint over. “Sometimes, these were as small as an object, or a colour that had been changed, but other times, they could be as significant as a whole figure.”</p> <figure class="align-right zoomable"><figcaption></figcaption></figure> <p>Art historians, using X-rays and infrared reflectography, have identified pentimenti in many famous paintings, from the adjusted placement of a controversial off-the-shoulder strap in <a href="https://www.virtualartacademy.com/madame-x/">John Singer Sargent</a>’s Portrait of Madam X, to the painted-over figure of a woman nursing a child in Picasso’s <a href="https://www.pablopicasso.org/old-guitarist.jsp">The Old Guitarist</a>, and a man with a bow-tie concealed beneath the brushwork of his work <a href="https://en.wikipedia.org/wiki/The_Blue_Room_(Picasso)">The Blue Room</a>.</p> <p>Singer Seargent’s adjustment was his response to an outcry at the perceived indecency of Madame X’s lowered shoulder strap, which both the public and art critics of the time declared to be indecent. By contrast, the model’s icy pallor caused only a ripple of interest.</p> <p>Picasso’s hidden figures <a href="https://www.singulart.com/en/blog/2019/10/29/pablo-picassos-blue-period-and-the-old-guitarist/">are assumed</a> to be the outcome of a shortage of canvas during his <a href="https://www.pablopicasso.org/blue-period.jsp">Blue Period</a>, but shortages aside, the word pentimento, which derives from the Italian verb <em>pentirsi</em>, meaning “to repent”, brings to these lost figures a sense of regret that resonates with the feeling in old age of having lost the younger self, or of carrying traces, deeply buried, of other lives one might have lived.</p> <p>In Cold Enough for Snow, Au’s narrator remarks of her mother that,</p> <p>"Perhaps, over time, she found the past harder and harder to evoke, especially with no-one to remember it with."</p> <p>The mother’s situation references another source of grief: that of the person who becomes the last of their friends and family still standing.</p> <p>In childhood games of this nature there would be a prize for the survivor. But for those who reach an extreme old age, having lost parents, siblings and contemporaries who knew them when they were young, even the presence of children and grandchildren may not entirely erase this “last man standing” loneliness. There is, too, the darkness of a projected future where there is no one still living who remembers us.</p> <p>In Jessica Au’s book the narrator occasionally speaks of the past as “a time that didn’t really exist at all”. And yet in my recent conversations with people in their seventies and above, every one of them admits to feeling a vivid sense of the past, and of the continuing presence of a younger self. As one of them wistfully remarked: “Sometimes she even seeps through.”</p> <h2>Memory and detail</h2> <p>Perhaps part of the problem is the mass of ordinary detail that disappears from memory on any given day. Life is made up of so many small moments that it’s impossible to hold onto them all – and if we did it might even be damaging.</p> <p>Imagine someone casually asking how your day had been, and responding with the tsunami of detail those hours actually contained.</p> <p>After opening your eyes at first light, you’d describe your shower, your breakfast, and how you slipped your keys into your handbag as you left the house; in the street you’d passed two women with a pram, a child with a small white dog on a lead, and an elderly man with a walking stick. And so on.</p> <p>If our minds swarmed with the trivia of daily life, more important events might be forgotten, and possibly the neural overload would even make us ill. Yet with the realisation of the loss of these minutes and hours arises the anxiety that in time, the things we do want to remember will slither away from us into the dark.</p> <p>I imagine this fear is what compels people to fill social media with photographs of their breakfasts, and of their relentless selfie-taking. It is surely the impulse behind keeping a journal.</p> <p>The anxiety of losing even the passing moments in a day afflicts the author of <a href="https://www.panmacmillan.com.au/9781509883295/">Ongoingness: The End of a Diary</a>. In it, the American writer Sara Manguso describes her compulsive need to document and hold onto her life. “I didn’t want to lose anything. That was my main problem.”</p> <p>After 25 years of paying attention to the smallest moments, Manguso’s diary is 800,000 words long. “The diary was my defense against waking up at the end of my life and realizing I’d missed it.” But despite her continuous effort,</p> <p>"I knew I couldn’t replicate my whole life in language. I knew that most of it would follow my body into oblivion."</p> <p>Is it possible that women experience grief around ageing earlier, and more emphatically than men? After all, by the age of 50, the bodies of even those women who remain fit send the implacable signal that things have changed.</p> <p>In Alice Munro’s story Bardon Bus, from her collection <a href="https://www.penguin.com.au/books/the-moons-of-jupiter-9780099458364">The Moons of Jupiter</a>, the female narrator endures dinner in the company of a rather malicious man, Dennis, who explains that women are.</p> <p>"forced to live in the world of loss and death! Oh, I know, there’s face-lifting, but how does that really help? The uterus dries up. The vagina dries up."</p> <p>Dennis compares the opportunities open to men as opposed to those available to women.</p> <p>"Specifically, with ageing. Look at you. Think of the way your life would be, if you were a man. The choices you would have. I mean sexual choices. You could start all over. Men do."</p> <p>When the narrator responds cheerfully that she might resist starting over, even if it were possible, Dennis is quick to retort:</p> <p>"That’s it, that’s just it, though, you don’t get the opportunity! You’re a woman and life only goes in one direction for a woman."</p> <p>In another story in the same collection, Labor Day Dinner, Roberta is in the bedroom dressing for an evening out when her lover George comes in and cruelly remarks: “Your armpits are flabby.” Roberta says she will wear something with sleeves, but in her head she hears the,</p> <p>"harsh satisfaction in his voice. The satisfaction of airing disgust. He is disgusted by her aging body. That could have been foreseen."</p> <p>Roberta thinks bitterly that she has always sought to remedy the least sign of deterioration.</p> <p>"Flabby armpits – how can you exercise the armpits? What is to be done? Now the payment is due, and what for? For vanity. Hardly even for that. Just for having those pleasing surfaces once, and letting them speak for you; just for allowing an arrangement of hair and shoulders and breasts to have its effect. You don’t stop in time, don’t know what to do instead; you lay yourself open to humiliation. So thinks Roberta, with self-pity […] She must get away, live alone, wear sleeves."</p> <p>As with most emotions that arise around our ageing, it can usually be traced back to a fraught relationship with time. French philosopher and Nobel Prize winner <a href="https://plato.stanford.edu/entries/bergson/">Henri Bergson</a> <a href="https://www.gutenberg.org/files/56852/56852-h/56852-h.htm">says</a>: “Sorrow begins by being nothing more than a facing towards the past.”</p> <p>For Roberta, as for many of us, it was a past in which we relied on those “pleasing surfaces”, perhaps even took them for granted, until they no longer produced the desired effect.</p> <p>But the truth is that our bodies are capable of more severe betrayals than mere flabby armpits. In time they may cause us to be exposed in skimpy, front-opening or back-opening hospital gowns under the all-seeing eye of the CT scanner; they may deliver us into the skilled, ruthless hands of a surgeon. Our very blood may speak of things we will not wish to hear.</p> <h2>Glimpsing our mortality in middle age</h2> <p>Middle age is sometimes referred to as The Age of Grief. It’s when we first glimpse our own mortality; we feel youth slipping away into the past, and the young people in our lives begin to assert their independence.</p> <p>We have our mid-life crises then. We join gyms, and take up running; we speak for the first time of “bucket lists” – the term itself an attempt to diminish the sting of time’s depredations. None of this will save us from the real Age of Grief, which comes later and hits harder because it is largely hidden. And we’ll be expected to endure it in silence.</p> <p>In my conversations with people aged 70 and older, grief has surfaced from causes other than what might be called “cosmetic” changes. Following a severe stroke, 80-year-old Philippa describes the pain of having had to make the decision to relinquish her home and move into residential care.</p> <p>"It’s when you lose your garden, which you’ve loved, and you’ve got to walk away from that. I’ve got photos of the house, and I look at them and think, oh, I just love the way I did that room, decorated it, things like that. But change happens."</p> <p>“Somehow change always comes with loss, as well as bringing something new,” I said. “Yes,” she replied, “I just had to say to myself: you can’t worry about it, and you can’t change it. That sounds hard, but it’s my way of dealing with it.”</p> <p>Tucked away in residential care homes, largely invisible to those of us lucky enough to still inhabit the outside world, elderly people like Philippa are quietly raising resilience to the level of an art form.</p> <p>In her poem, <a href="https://www.poetryfoundation.org/poems/47536/one-art">One Art</a>, the Canadian poet Elizabeth Bishop advises losing something every day.</p> <blockquote> <p>Accept the fluster<br />of lost door keys, the hour badly spent.<br />Lose something every day.<br />The art of losing isn’t hard to master.</p> </blockquote> <p>Bishop goes on to list other lost items – her mother’s watch, the next-to-last of three loved houses, lovely cities, two rivers, even a continent. While the losses elderly people commonly accumulate are less grand, they are no less devastating.</p> <p>One by one, they will relinquish driver’s licenses. For many there will be the loss of the family home and their belongings, save for whatever will fit into a care home’s single room. Perhaps they have already given up the freedom of walking without the aid of a stick, or walker. There may be the dietary restrictions imposed by conditions such as diabetes, and the invisible disabilities of diminished hearing and eyesight.</p> <p>A failing memory, one would think, must be the final straw. And yet, what seems to be the actual final straw is the situation, reported time and again, where an old person feels “unseen”, or “looked through”, and for indefensible reasons finds themself being “missed” in favour of someone younger. It might, for example, be a moment when they are ignored as they patiently wait their turn at a shop counter.</p> <p>In my conversation with Philippa, she remarked that old people are often looked through when they are part of a group, or when they are waiting to be served. “I have seen it happen to other older people, as if they don’t exist. I have called out assistants who have done that to other people.”</p> <p>Surely the least we can do, as fortunate beings of fewer years, is to acknowledge the old people among us. To make them feel seen, and of equal value.</p> <h2>‘Age pride’ and destigmatising ‘old’</h2> <p><a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7246680/">Ageism, Healthy Life Expectancy and Population Ageing: How Are They Related</a> is a recent survey conducted with more than 83,000 participants from 57 countries. It found that ageism negatively impacts the health of older adults. In the United States, people with a negative attitude towards ageing live 7.5 fewer years than their more positive counterparts.</p> <p>In Australia, the National Ageing Research Institute has developed an <a href="https://www.nari.net.au/age-positive-language-guide">Age-Positive Language Guide</a> as part of its strategy to combat ageism.</p> <p>Examples of poor descriptive language include terms such as “old person”, “the elderly”, and even “seniors”. That last term appears on a card Australians receive shortly after turning 60, which enables them to receive various discounts and concessions. Instead, we are encouraged to use “older person”, or “older people”. But this is just another form of age-masking that fools no one.</p> <p>It would be better to throw the institute’s energy into destigmatising the word “old”. What, after all, is wrong with being old, and saying so?</p> <p>To begin the process of reclaiming this word from the pejorative territory it currently occupies, old people need to start claiming their years with pride. If other marginalised social groups can do it, why can’t old people? Some activists working against ageism are beginning to mention <a href="https://www.nextavenue.org/how-to-swap-ageism-for-age-pride/">“age pride”</a>.</p> <p>If we become homesick for who we once were as we age, we might remind ourselves of the meaning of <em>nostos</em> and consider old age as a kind of homecoming.</p> <h2>Narrative identity</h2> <p>The body we travel in is a vehicle for all the iterations of the self, and the position we currently inhabit is part of an ongoing creative process: the evolving story of the self. From the 1980s, psychologists, philosophers and social theorists have been calling it <a href="https://psycnet.apa.org/record/2011-21882-005">narrative identity</a>.</p> <p>The process of piecing together a narrative identity begins in late adolescence and evolves across our entire lives. Like opening a Russian doll, from whose hollow shell other dolls emerge, at our centre is a solid core composed of traits and values. It’s also composed of the narrative identity we have put together from all our days – including those we cannot now remember – and from all the selves we have ever been. Perhaps even from the selves we might have been, but chose instead to paint over.</p> <p>In Metamorphosis, or the Elephant’s Foot, Harriet Mayfield tells her husband, “At this point in life. We are who we are – the outcome of various other incarnations.”</p> <p>We know our lives, and the lives of others, through fragments. Fragments are all we have. They’re all we’ll ever have. We live in moments, not always in chronological order. But narrative identity helps us make meaning of life. And the vantage point of old age offers the longest view.</p> <p>The story of the self carries us from the deep past to the present moment. Does running water really trigger the urge to pee? Experts explain the brain-bladder connection

Why, for example, do we get the urge to pee just before getting into the shower, or when we’re swimming? What brings on those “nervous wees” right before a date?</p> <p>Research suggests our brain and bladder are in constant communication with each other via a neural network called the <a href="https://www.einj.org/journal/view.php?doi=10.5213/inj.2346036.018">brain-bladder axis</a>.</p> <p>This complex web of circuitry is comprised of sensory neural activity, including the sympathetic and parasympathetic nervous systems. These neural connections allow information to be sent <a href="https://doi.org/10.3390/diagnostics12123119">back and forth</a> between the brain and bladder.</p> <p>The brain-bladder axis not only facilitates the act of peeing, but is also responsible for telling us we need to go in the first place.</p> <h2>How do we know when we need to go?</h2> <p>As the bladder fills with urine and expands, this activates special receptors detecting stretch in the nerve-rich lining of the bladder wall. We, in turn, <a href="https://pubmed.ncbi.nlm.nih.gov/16254993/">register this</a> as that all-too-familiar feeling of fullness and pressure down below.</p> <p>Beyond this, however, a range of situations can trigger or exacerbate our need to pee, by increasing the production of urine and/or stimulating reflexes in the bladder.</p> <h2>Peeing in the shower</h2> <p>If you’ve ever felt the need to pee while in the shower (no judgement here) it may be due to the sight and sound of running water.</p> <p>In a 2015 study, <a href="https://doi.org/10.1371/journal.pone.0126798">researchers demonstrated</a> that males with urinary difficulties found it easier to initiate peeing when listening to the sound of running water being played on a smartphone.</p> <p>Symptoms of overactive bladder, including urgency (a sudden need to pee), have also been <a href="https://www.alliedacademies.org/articles/environmental-cues-to-urgency-and-incontinence-episodes-in-chinesepatients-with-overactive-urinary-bladder-syndrome.html">linked to</a> a range of environmental cues involving running water, including washing your hands and taking a shower.</p> <p>This is likely due to both physiology and psychology. Firstly, the sound of running water may have a relaxing <em>physiological</em> effect, increasing activity of the parasympathetic nervous system. This would relax the bladder muscles and prepare the bladder for emptying.</p> <p>At the same time, the sound of running water may also have a conditioned <em>psychological</em> effect. Due to the countless times in our lives where this sound has coincided with the actual act of peeing, it may trigger an instinctive reaction in us to urinate.</p> <p>This would happen in the same way <a href="https://www.simplypsychology.org/pavlov.html">Pavlov’s dog learnt</a>, through repeated pairing, to salivate when a bell was rung.</p> <h2>Cheeky wee in the sea</h2> <p>But it’s not just the sight or sound of running water that makes us want to pee. Immersion in cold water has been shown to cause a “cold shock response”, <a href="https://pubmed.ncbi.nlm.nih.gov/19945970">which activates</a> the sympathetic nervous system.</p> <p>This so-called “fight or flight” response drives up our blood pressure which, in turn, causes our kidneys to filter out more fluid from the bloodstream to stabilise our blood pressure, in a process called “<a href="https://link.springer.com/article/10.1007/BF00864230">immersion diuresis</a>”. When this happens, our bladder fills up faster than normal, triggering the urge to pee.</p> <p>Interestingly, immersion in very warm water (such as a relaxing bath) may also increase urine production. In this case, however, it’s due to activation of the parasympathetic nervous system. <a href="https://doi.org/10.1007/s004210050065">One study</a> demonstrated an increase in water temperature from 40℃ to 50℃ reduced the time it took for participants to start urinating.</p> <p>Similar to the effect of hearing running water, the authors of the study suggest being in warm water is calming for the body and activates the parasympathetic nervous system. This activation can result in the relaxation of the bladder and possibly the pelvic floor muscles, bringing on the urge to pee.</p> <h2>The nervous wee</h2> <p>We know stress and anxiety can cause bouts of nausea and butterflies in the tummy, but what about the bladder? Why do we feel a sudden and frequent urge to urinate at times of heightened stress, such as before a date or job interview?</p> <p>When a person becomes stressed or anxious, the body goes into fight-or-flight mode through the activation of the sympathetic nervous system. This triggers a cascade of physiological changes designed to prepare the body to face a perceived threat.</p> <p>As part of this response, the muscles surrounding the bladder may contract, leading to a more urgent and frequent need to pee. Also, as is the case during immersion diuresis, the increase in blood pressure associated with the stress response may <a href="https://doi.org/10.1172/JCI102496">stimulate</a> the kidneys to produce more urine.</p> <h2>Some final thoughts</h2> <p>We all pee (most of us several times a day). Yet <a href="https://doi.org/10.5489/cuaj.1150">research has shown</a> about 75% of adults know little about how this process actually works – and even less about the brain-bladdder axis and its role in urination.</p> <p><a href="https://www.continence.org.au/about-us/our-work/key-statistics-incontinence#:%7E:text=Urinary%20incontinence%20affects%20up%20to,38%25%20of%20Australian%20women1.">Most Australians</a> will experience urinary difficulties at some point in their lives, so if you ever have concerns about your urinary health, it’s extremely important to consult a healthcare professional.</p> <p>And should you ever find yourself unable to pee, perhaps the sight or sound of running water, a relaxing bath or a nice swim will help with getting that stream to flow.<!-- Below is The Conversation's page counter tag. Gratitude amplified: Unlocking the key to happiness and deep connections

<p>In today's fast-paced world, filled with negative news, challenging experiences, and personal losses, finding joy and gratitude can often be a difficult task. </p> <p>As we naturally tend to focus on the negative aspects of life, it becomes essential to pause and cultivate gratitude, which can have a profound impact on our lives and the lives of those around us.</p> <p><strong>What is gratitude? </strong></p> <p>Gratitude involves showing appreciation for the things in life that are important to you, and taking a moment to notice and acknowledge these moments have proven benefits of lifting mood and helping you feel more positive throughout the day. Have you ever stopped to notice that there is something to be grateful for every day if we just take stock? It could be a person who kindly let you merge into traffic, a refreshing breeze on a scorching day, a beautiful rainbow after a heavy downpour, or a thoughtful call or text from a friend. </p> <p>Gratitude is strongly and consistently associated with greater happiness. Gratitude helps people feel more positive emotions, relish good experiences, improve their health, deal with adversity, and build strong relationships. We also know that Gratitude begets gratitude, meaning that any kind act is likely to bring kindness back, in spades! </p> <p><strong>Practice Gratitude – Write down 3 things per day</strong></p> <p><a href="https://theresilienceproject.com.au/">The Resilience Project</a> changed my life by introducing a simple practice: at the end of each day, we take five minutes to write down three things we are grateful for that happened that day. It could be as simple as a walk in the sun, a phone call from a friend, a stroll with your dog, or even a comforting cup of tea. </p> <p>Engaging in this exercise helps you articulate your gratitude and realise the abundance of things to be grateful for in our everyday lives.</p> <p>I personally experienced the transformative power of gratitude during the two years of COVID lockdown in Victoria while working full-time and home schooling my two boys, aged 8 and 10. Our family embraced this practice each night, which not only changed our outlook on life but also sparked positive conversations during dinner. </p> <p>Surprisingly, it was often the simplest things we were all grateful for, like homemade meals or cherished moments of togetherness.</p> <p>I distinctly remember preparing a hot meal and passing it over our neighbour's fence when their family was unwell with COVID. To our surprise, a few weeks later, they reciprocated with a homecooked meal. In those challenging times, a sense of community became such a source of positivity, and we were immensely grateful for the love that went into that dinner. </p> <p>So why not take the time to stop and smell the roses and practice gratitude? Why does my back get so sore when I’m sick? The connection between immunity and pain

Or COVID?</p> <p>This discomfort, common during many illnesses, is not just a random symptom. It’s a result of complex interactions between your immune system and your brain called the “<a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3314453/">neuroimmune synapse</a>”.</p> <p>A fascinating and yet-to-be-understood consequence of this conversation between the immune and brain systems during sickness is that it is particularly noticeable in the <a href="https://academic.oup.com/brain/article/145/3/1098/6370954">lower back</a>. This is thought to be one of the body’s most sensitive regions to neuroimmune threats.</p> <h2>Immunology basics</h2> <p>Our immune system is a double-edged sword. Yes, it fights off infections for us – but it also makes us acutely aware of the job it is doing.</p> <p>When our body detects an infection, our immune system releases molecules including signalling proteins called <a href="https://www.researchgate.net/publication/227831648_The_functions_of_cytokines_and_their_uses_in_toxicology">cytokines</a>. These proteins coordinate our immune system to fight off the infection and talk to our brain and spinal cord to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2740752/#:%7E:text=Production%20of%20proinflammatory%20cytokines%20induces,to%20depression%20in%20vulnerable%20individuals.">change our behaviour</a> and physiology.</p> <p>This can result in symptoms like fatigue, loss of appetite, fever and increased sensitivity to pain. Classically, we think of this as a beneficial behavioural change to help us conserve energy to fight off the infection. It’s why we often feel the need to rest and withdraw from our usual activities when we’re sick – and also why we are grumpier than usual.</p> <h2>Invisibly small changes</h2> <p>Part of this self-protective response is a change in how we perceive threats, including sensory stimuli.</p> <p>When we are sick, touch can become painful and muscles can ache. Many changes in behaviour and sensory systems are <a href="https://doi.org/10.1159/000521476">believed</a> to have origins at the nanoscale. When molecular changes occur in part of the brain linked to cognition or mood, we think and feel differently. If these neuroimmune synapse changes happen in the sensory processing regions of the brain and spinal cord, we feel more pain.</p> <p>Such sensory changes, known as <a href="https://www.iasp-pain.org/resources/fact-sheets/allodynia-and-hyperalgesia-in-neuropathic-pain/#:%7E:text=Allodynia%20is%20pain%20due%20to,stimulus%20that%20normally%20provokes%20pain.">allodynia and hyperalgesia</a>, can lead to heightened pain sensitivity, even in areas not directly affected by the infection – <a href="https://www.sciencedirect.com/science/article/abs/pii/S0889159114001731?via%3Dihub">such as the lower back</a>.</p> <h2>Immune memories</h2> <p>This immune response happens with a range of bacterial infections and viruses like COVID or the flu. In fact, the sick feeling we sometimes get after a vaccination is the good work our immune system is doing to contribute to <a href="https://theconversation.com/you-cant-get-influenza-from-a-flu-shot-heres-how-it-works-118916">a protective immune memory</a>.</p> <p>Some of that immune-cellular conversation also alerts our brains that we are sick, or makes us think we are.</p> <p>After some viral infections, the sick feeling persists longer than the virus. We are seeing a long-term response to COVID in some people, termed <a href="https://theconversation.com/when-does-covid-become-long-covid-and-whats-happening-in-the-body-when-symptoms-persist-heres-what-weve-learnt-so-far-188976">long COVID</a>.</p> <p>Women, who generally have a <a href="https://www.nature.com/articles/nri.2016.90">stronger immune response</a> than men, may be more likely to <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7937378/">experience pain symptoms</a>. Their heightened immune response (while beneficial in resisting infections) also predisposes women to a higher risk of inflammatory conditions like <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7980266/">autoimmune diseases</a>.</p> <h2>When to worry and what to do</h2> <p>If the pain is severe, persistent, or accompanied by other concerning symptoms, seek medical attention. Mild to moderate pain is a common symptom during illness and we often notice this in the lower back. The good news is it usually subsides as the infection clears and the sickness resolves.</p> <p>While treating the underlying infection is crucial, there are also ways to dial down sickness-induced neuroimmune pain.</p> <p>Maintaining a diverse microbiome (the collection of microorganisms living in and on your body) by <a href="https://pubmed.ncbi.nlm.nih.gov/31704402/">eating well and getting outside</a> can help. Getting quality sleep, staying hydrated and minimising inflammation <a href="https://karger.com/bbe/article/97/3-4/197/821576/Sickness-and-the-Social-Brain-How-the-Immune">helps too</a>.</p> <p>Amazingly, there is <a href="https://pubmed.ncbi.nlm.nih.gov/34404209/">research</a> suggesting your grandmother’s traditional chicken broth recipe decreases the immune signals at the neuroimmune synapse.</p> <p>Scientists are also <a href="https://pubmed.ncbi.nlm.nih.gov/24799686/">showing</a> mindfulness meditation, cold water therapy and controlled breathing can drive profound cellular and molecular changes to help activate bodily systems like the autonomic nervous system and alter the immune response. These practices might not only help manage pain but also add an anti-inflammatory component to the immune response, reducing the severity and duration of sickness.</p> <p>Heat treatment (with a pack or hot water bottle) might <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8401625/">provide some relief</a> due to increased circulation. Over-the-counter pain relief maybe also be helpful but seek advice if you are taking other medications.</p> <h2>All in the mind?</h2> <p>Is this all mind over matter? A little of yes and a lot of no.</p> <p>The little of yes comes from <a href="https://pubmed.ncbi.nlm.nih.gov/26194270/">research</a> supporting the idea that if you expect your breathing, meditation and cold bath therapy to work, it may well make a difference at the cellular and molecular level.</p> <p>But by understanding the mechanisms of back pain during illness and by using some simple strategies, there is hope to manage this pain effectively. Always remember to seek medical help if your symptoms are severe or persist longer than expected. Fans fume as classic film undergoes censorship treatment

<p>Film fanatics are in outrage after discovering an unannounced edit in William Friedkin’s 1971 classic movie, The French Connection.</p> <p>The scene causing the trouble comes just 10 minutes into the drama, when two characters - Gene Hackman’s ‘Popeye’ Doyle and Roy Scheider’s Buddy ‘Cloudy’ Russo - are having a conversation, and one of them uses a racial slur. </p> <p>The sequence was removed, and the new edited version cuts to the latest in the conversation, omitting the part with the slur. </p> <p>Disney has been the subject of blame for the move - as the company took over Fox in 2019 and subsequently the rights to the film - with fans accusing them of censoring the scene in the United States, while in the United Kingdom and Canada, the unedited version of the film is still available for streaming on Disney+.</p> <p>Most took to social media to share their complaints, with the majority in agreement that Disney had missed the mark, and The Film Magazine’s Joseph Wade even calling it “corporate vandalism”.</p> <p>“In cases such as this, ‘Censor’ takes the place of ‘Vandalise’,” he tweeted. “They have vandalised a piece of art.”</p> <blockquote class="twitter-tweet"> <p dir="ltr" lang="en">Disney Censor 'The French Connection' (1971)</p> <p>In cases such as this, "Censor" takes the place of "Vandalise".</p> <p>They have vandalised a piece of art. Matthew McConaughey's surprising connection to Woody Harrelson

<p>Matthew McConaughey has shared a revelation about his relationship with his friend and Hollywood co-star Woody Harrelson. </p> <p>The acting duo, who notably shared the screen on the debut season of acclaimed crime drama <em>True Detective </em>in 2014, have been friends long before they ever acted on screen together. </p> <p>Now it has been revealed their bromance could have even greater ties, as the actors could be blood related. </p> <p>McConaughey, 53, and Harrelson, 61, are set to reunite on screen for an upcoming Apple TV+ comedy series, titled <em>Brother From Another Mother</em>, which triggered the conversation about their relationship. </p> <p>Appearing on the <em><a href="https://podcasts.apple.com/us/podcast/lets-talk-off-camera-with-kelly-ripa/id1676166337" target="_blank" rel="noopener">Let’s Talk Off Camera with Kelly Ripa</a></em> podcast, McConaughey shared that the pair could actually share the same father.</p> <p>“You know, where I start and where he ends, and where he starts and I end, has always been like a murky line,” McConaughey began.</p> <p>He continued, “In Greece a few years ago, we’re sitting around talking about how close we are and our families,” McConaughey said. “And my mum [Mary Kathlene McCabe] is there, and she says, ‘Woody, I knew your dad’ … Everyone was aware of the ellipses that my mum left after ‘knew.’ It was a loaded ‘knew’."</p> <p>“We went on to unpack what this ‘knew’ meant and did some math and found out that [Harrelson’s] dad [Charles Harrelson] was on furlough at the same time that my mum and dad [James Donald McConaughey] were in their second divorce. "I'm not afraid to challenge myself": J-Lo's secret to staying physically and mentally fit

<p>Jennifer Lopez has shared an insight into her gruelling workout routine, revealing it is the best thing for both her physical and mental wellness. </p> <p>The 53-year-old singer and actress told US Weekly how important moving her body is to help keep a positive mental attitude. </p> <p>"It's no secret that fitness is a very important part of my life... I think there's a positive correlation between exercise and mental health," she said. </p> <p>J-Lo's claims have long been backed up by healthcare professionals, with the MentalHealth.org website reading, "Research shows that people who exercise regularly have better mental health and emotional wellbeing, and lower rates of mental illness."</p> <p>"Taking up exercise seems to reduce the risk of developing mental illness... and seems to help in treating some mental health conditions, like depression and anxiety."</p> <p>Lopez went on to say, "When you find a good balance through determination and focus, we're naturally pushing to be the best version of ourselves."</p> <p>She usually begins her workouts "first thing in the morning", which usually means rolling out of bed before 5am. </p> <p>J-Lo regularly mixes up her workouts, explaining, "I'm always evolving and looking for things that keep me excited and motivated."</p> <p>"I'm not afraid to challenge myself, so I'm open to pushing myself, which is what I'm doing right now as I rehearse for my new album, [This Is Me… Now], coming out this summer."</p> <p>Even Jennifer's trainer Tracy Anderson said the pop star goes above and beyond for her health and fitness, pushing herself for maximum results. </p> <p>'"She shows up no matter what, she takes the time seriously, she knows her body, she wants to be herself," Anderson has said.  </p> <p>"She knows that putting in the hard work to get her workout in is part of it."</p> <p><em>Image credits: Getty Images</em></p>

Do emoji reflect our emotions, or are we just putting on a brave face?

<p>As if human communications weren’t complicated enough. Researchers are saying people also use emoji to reflect or mask their internal emotional state, depending on the context and accepted social rules <a href="https://emojipedia.org/confused-face/" target="_blank" rel="noreferrer noopener">😕</a></p> <p>As we all know, in face-to-face communication, a person may choose to “mask” their true internal feelings in certain situations, like receiving an unwanted gift. They might smile, or display positive facial expressions, contradicting their true feelings. And such displays are often governed by expected social norms and differ according to culture <a href="https://emojipedia.org/face-in-clouds/" target="_blank" rel="noreferrer noopener">😶‍🌫️</a>. </p> <p>Moyu Liu from the University of Tokyo was curious about whether emoji used in online communications – on social media, texting or email – work the same way <a href="https://emojipedia.org/thinking-face/" target="_blank" rel="noreferrer noopener">🤔</a>.</p> <p>Just like facial expressions, emoji can represent or mask emotions, <a href="https://www.frontiersin.org/articles/10.3389/fpsyg.2023.1035742/full" target="_blank" rel="noreferrer noopener">Liu’s study found</a>.  However negative emoji are more likely to represent a persons true feelings.</p> <p>Liu says “with online socializing becoming ever more prevalent, it is important to consider whether it is causing us to become more detached from our true emotions.” </p> <p>“Do people require a ‘shelter’ to express their genuine emotions, and is it possible to break free from pretence and share our true selves in online settings?”</p> <p>More than 1,200 Japanese participants were recruited for the study, aged 10 – 29 years, and all users of a popular downloaded emoji keyboard<a href="https://emojipedia.org/keyboard/" target="_blank" rel="noreferrer noopener">⌨️</a> in Japan called Simeji.</p> <p>Particpants were asked to respond as they normally would to a set of private and public messages, while also rating the intensity of their emotions.</p> <p>The study shows people use emoji in a similar way to facial expressions.</p> <p>Positive faces <a href="https://emojipedia.org/hugging-face/" target="_blank" rel="noreferrer noopener">🤗</a><a href="https://emojipedia.org/grinning-face-with-big-eyes/" target="_blank" rel="noreferrer noopener">😃</a><a href="https://emojipedia.org/smiling-face-with-smiling-eyes/" target="_blank" rel="noreferrer noopener">😊</a> were used in both positive contexts, and negative contexts where the user wanted to cover their true emotional state.</p> <p>However, negative emoji strongly reflected intense negative emotions <a href="https://emojipedia.org/pouting-face/" target="_blank" rel="noreferrer noopener">😡</a><a href="https://emojipedia.org/face-with-steam-from-nose/" target="_blank" rel="noreferrer noopener">😤</a>.</p> <p>People were more likely to truly express their emotions using emoji in private, rather than public communication.</p> <p>And they were more likely to use masking emoji – not matching their emotions – when dealing with high status individuals. And in general, were less likely to use emoji in these contexts <a href="https://emojipedia.org/zipper-mouth-face/" target="_blank" rel="noreferrer noopener">🤐</a>.</p> <p>Liu emphasized that the study should be expanded in the future to include a broader span of demographics and consider different cultural contexts. Prince Harry alleges physical fight with brother William

<p>Prince Harry has accused his brother, William, of physically assaulting him during an argument over his wife, Meghan, Duchess of Sussex in 2019.</p> <p>UK newspaper The Guardian claim to have seen an advance copy of Prince Harry's highly anticipated memoir, Spare, in which Harry reportedly alleges William knocked him to the floor during the altercation.</p> <p>The alleged incident took place after a conversation between the two brothers, during which William, the heir to the British throne, called Meghan Markle "difficult," "rude" and “abrasive”.</p> <p>"The confrontation escalated until William "grabbed me by the collar, ripping my necklace, and ... knocked me to the floor," The Guardian reported.</p> <p>The Guardian article focuses on the alleged physical altercation between the brothers but describes the entirety of the book as a "remarkable volume."</p> <p>The article reports Harry's version of events, in which William arrives at Harry and Meghan's then home, Nottingham Cottage on Kensington Palace grounds, to allegedly discuss "'the whole rolling catastrophe' of their relationship and struggles with the press.”</p> <p>Harry alleges that William attacked him after he had offered him water and attempted to cool a heated verbal exchange, according to The Guardian.</p> <p>The article quotes Harry, "He set down the water, called me another name, then came at me. It all happened so fast. So very fast. He grabbed me by the collar, ripping my necklace, and he knocked me to the floor. I landed on the dog's bowl, which cracked under my back, the pieces cutting into me. I lay there for a moment, dazed, then got to my feet and told him to get out.”</p> <p>The article says Harry states in the book that William urged him to hit back, but he refused to do so. What’s the connection between cosmetic procedures and mental health?

<p>Although we cannot be sure of the exact numbers of Australians undergoing cosmetic procedures, as there is no requirement for health professionals to report their statistics, there is a consensus demand is on the rise. </p> <p>In 2015, the <a href="https://cpca.net.au/wp-content/uploads/2016/05/31-05-2016_AUSTRALIAS_SPEND_ON_COSMETIC_TREATMENTS_TOPS_1_BILLION.pdf">Cosmetic Physicians College of Australasia</a> found Australians were spending more than $1 billion a year on non-invasive cosmetic procedures like Botox and fillers. This is more than 40% higher, per capita, than in the United States.</p> <p>In the US, where procedure <a href="https://cdn.theaestheticsociety.org/media/statistics/2021-TheAestheticSocietyStatistics.pdf">statistics are reported</a>, there was a 42% increase in the number of filler procedures and a 40% increase in Botox procedures performed in the last year alone. </p> <p>Rates of mental health issues in this group may be higher than the general population, but seemingly not enough is being done to ensure the psychological safety of people requesting cosmetic procedures.</p> <h2>Body dysmorphic disorder</h2> <p>Body image concerns are generally the main motivator for seeking cosmetic procedures of all kinds. These concerns are <a href="https://www.sciencedirect.com/science/article/pii/S1740144518305552">usually focused on the body part</a> where the cosmetic intervention is sought, such as the nose for a rhinoplasty. </p> <p>Severe body image concerns are a key feature of several mental health conditions. The most prevalent in people seeking cosmetic procedures is body dysmorphic disorder. In the general community, around 1-3% of people will experience body dysmorphic disorder, but in populations seeking cosmetic surgery, this <a href="https://pubmed.ncbi.nlm.nih.gov/35715310/">rises to 16-23%</a>.</p> <p><a href="https://bdd.iocdf.org/professionals/diagnosis/">Body dysmorphic disorder</a> involves a preoccupation or obsession with one or more perceived flaws in physical appearance which are not visible or seem minor to other people. In response to the distress regarding the flaw, the person with body dysmorphic disorder will perform repetitive behaviours (such as excessively checking body parts in the mirror) and mental acts (such as comparing their appearance with other people). </p> <p>These concerns can have a significant negative impact on the person’s daily life, with some people too distressed to leave their home or even eat dinner with family members out of fear of being seen by others. </p> <p>With the distress associated with body dysmorphic disorder seemingly stemming from physical appearance issues, it makes sense someone with body dysmorphic disorder is far more likely to turn up at a cosmetic clinic for treatment than a mental health clinic.</p> <p>The problem is, cosmetic intervention usually makes the person with body dysmorphic disorder feel the <a href="https://link.springer.com/article/10.1007/s00266-017-0819-x">same or worse</a> after the procedure. They may become even more preoccupied with the perceived flaw and seek further cosmetic procedures. </p> <p>Patients with body dysmorphic disorder are also <a href="https://academic.oup.com/asj/article/22/6/531/204131">more likely</a> to take legal action against their treating cosmetic practitioner after believing they have not received the result they wanted.</p> <p>For these reasons, body dysmorphic disorder is generally considered by health professionals to be a “red flag” or contraindication (a reason not to undergo a medical procedure) for cosmetic procedures.</p> <p>However, this is not entirely clear-cut. <a href="https://academic.oup.com/asj/advance-article/doi/10.1093/asj/sjac243/6678852?searchresult=1">Some studies</a> have shown people with body dysmorphic disorder can improve their symptoms after cosmetic intervention, but the obsession may just move to another body part and the body dysmorphic disorder diagnosis remain.</p> <h2>What about other mental health conditions?</h2> <p>Body dysmorphic disorder is by far the most well-studied disorder in this area, but is not the only mental health condition that may be associated with poorer outcomes from cosmetic procedures.</p> <p>According to a <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8034989/">recent systematic review</a>, the rates of depression (5-26%), anxiety (11-22%) and personality disorders (0-53%) in people seeking cosmetic surgery may be higher than the general population (which are estimated to be 10%, 16% and 12% respectively).</p> <p>However, these rates should be interpreted with some caution as they depend greatly on how the mental health diagnosis was made – clinician-led interview (higher rates) versus mental health questionnaire (lower rates). Some interview approaches can suggest higher rates of mental health issues as they may be quite unstructured and thus have questionable validity compared with highly structured questionnaires. </p> <p>Besides body dysmorphic disorder, the research investigating other mental health conditions is limited. This may just be due to the fact body image focus is at the core of body dysmorphic disorder, which makes it a logical focus for cosmetic surgery research compared with other types of psychiatric disorders.</p> <h2>So what should happen?</h2> <p>Ideally, all cosmetic surgeons and practitioners should receive sufficient training to enable them to conduct a brief routine assessment of all prospective patients. Those with signs indicating they are unlikely to derive psychological benefit from the procedure should undergo a further assessment by a mental health professional before undergoing the procedure. </p> <p>This could include an in-depth clinical interview about motivations for the procedure, and completing a range of standard mental health questionnaires. </p> <p>If a person was found to have a mental health issue in the assessment process, it does not necessarily mean the mental health professional would recommend against pursuing the procedure. They may suggest a course of psychological therapy to address the issue of concern and then undergo the cosmetic procedure. </p> <p>At the moment, assessments are only recommended rather than mandated for cosmetic surgery (and not at all for injectables like Botox and fillers). The <a href="https://plasticsurgery.org.au/wp-content/uploads/2020/08/2.5-AHPRA-MBA-Guidelines-for-Registered-Medical-Practitioners-Who-Perform-Cosmetic-Medical-and-Surgical-Procedures-2016.pdf">guidelines</a> say evaluation should be undertaken if there are signs the patient has “significant underlying psychological problems”.</p> <p>This means we are relying on the cosmetic medical practitioner being capable of detecting such issues when they may have received only basic psychological training at medical school, and when their business may possibly benefit from not attending to such diagnoses.</p> <p>An August 2022 <a href="https://www.ahpra.gov.au/Resources/Cosmetic-surgery-hub/Cosmetic-surgery-review.aspx">independent review</a> by the Australian Health Practitioner Regulation Agency and the Medical Board of Australia recommended the guidelines around mental health assessment should be “strengthened” and emphasised the importance of medical practitioners receiving more training in the detection of psychiatric disorders. </p> <p>Ultimately, as cosmetic practitioners are treating patients who are seeking treatment for psychological rather than medical reasons, they must have the wellbeing of the patient front-of-mind, both out of professional integrity and to protect themselves from legal action. Long COVID: How lost connections between nerve cells in the brain may explain cognitive symptoms

<p>For a portion of people who get COVID, symptoms continue for <a href="https://www.ons.gov.uk/peoplepopulationandcommunity/healthandsocialcare/conditionsanddiseases/bulletins/prevalenceofongoingsymptomsfollowingcoronaviruscovid19infectionintheuk/6october2022" target="_blank" rel="noopener">months or even years</a> after the initial infection. This is commonly referred to as “long COVID”.</p> <p>Some people with long COVID complain of “<a href="https://theconversation.com/what-is-and-what-isnt-brain-fog-190537" target="_blank" rel="noopener">brain fog</a>”, which includes a wide variety of cognitive symptoms affecting memory, concentration, sleep and speech. There’s also growing concern about findings that people who have had COVID are at <a href="https://www.thelancet.com/journals/lanpsy/article/PIIS2215-0366(22)00260-7/fulltext" target="_blank" rel="noopener">increased risk</a> of developing brain disorders, such as dementia.</p> <p>Scientists are working to understand how exactly a COVID infection affects the human brain. But this is difficult to study, because we can’t experiment on living people’s brains. One way around this is to create <a href="https://www.nature.com/articles/s41578-021-00279-y" target="_blank" rel="noopener">organoids</a>, which are miniature organs grown from stem cells.</p> <p>In a <a href="https://www.nature.com/articles/s41380-022-01786-2.pdf" target="_blank" rel="noopener">recent study</a>, we created brain organoids a little bigger than a pinhead and infected them with SARS-CoV-2, the virus that causes COVID-19.</p> <p>In these organoids, we found that an excessive number of synapses (the connections between brain cells) were eliminated – more than you would expect to see in a normal brain.</p> <p>Synapses are important because they allow neurons to communicate with each other. Still, the elimination of a certain amount of inactive synapses is part of normal brain function. The brain essentially gets rid of old connections when they’re no longer needed, and makes way for new connections, allowing for more efficient functioning.</p> <p>One of the crucial functions of the brain’s immune cells, or <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5768411/" target="_blank" rel="noopener">microglia</a>, is to prune these inactive synapses.</p> <p>The exaggerated elimination of synapses we saw in the COVID-infected models could explain why some people have cognitive symptoms as part of long COVID.</p> <p><strong>Parallels with neurodegenerative disorders</strong></p> <p>Interestingly, this pruning process is believed to go awry in several disorders affecting the brain. In particular, excessive elimination of synapses has recently been linked to <a href="https://www.nature.com/articles/s41593-018-0334-7" target="_blank" rel="noopener">neurodevelopmental disorders</a> such as <a href="https://www.nature.com/articles/s41593-018-0334-7" target="_blank" rel="noopener">schizophrenia</a>, as well as <a href="https://www.frontiersin.org/articles/10.3389/fncel.2019.00063/full" target="_blank" rel="noopener">neurodegenerative disorders</a> such as Alzheimer’s and Parkinson’s disease.</p> <p>By sequencing the RNA of single cells, we could study how different cell types in the organoid responded to the virus. You can see the microglial cells in red.</span> <span class="attribution"><span class="source">Sellgren lab</span>, <span class="license">Author provided</span></span></figcaption></figure> <p><strong>A possible target for treatment</strong></p> <p>One limitation of our research is that our organoid models closely resemble the foetal or early brain, rather than the adult brain. So we can’t say for sure whether the changes we noted in our study will necessarily be reflected in the adult brain.</p> <p>However, some <a href="https://pubmed.ncbi.nlm.nih.gov/33248159/" target="_blank" rel="noopener">post-mortem</a> and <a href="https://pubmed.ncbi.nlm.nih.gov/35255491/" target="_blank" rel="noopener">imaging studies</a> report neuronal death and reduction in grey matter thickness in COVID patients, which hints at similar instances of synapse loss caused by an infection in adults.</p> <p>If this proves to be a fruitful line of enquiry, we believe our findings could point to a mechanism contributing to persisting cognitive symptoms after COVID and other viral infections that affect the brain.</p> <p>SARS-CoV-2 is an RNA virus and similar <a href="https://pubmed.ncbi.nlm.nih.gov/27337340/" target="_blank" rel="noopener">processes</a> have been seen in mice infected with other RNA viruses that can also cause residual cognitive symptoms, such as the <a href="https://pubmed.ncbi.nlm.nih.gov/31235930/" target="_blank" rel="noopener">West Nile virus</a>.</p> <p>From here we want to study how different drugs could inhibit the changes we saw in the infected models, hopefully paving the way towards effective treatments. In <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6410571/">other research</a>, we’ve observed that an antibiotic called minocycline can reduce the degree to which microglia prune synapses in a dish. Why a renowned artist is burning his own masterpieces

<p dir="ltr">Artist Damien Hirst has begun burning hundreds of his own artworks after his latest collection prompted the buyer to either choose the physical work or the NFT representing it. </p> <p dir="ltr">Those who chose to buy the NFT, or virtual version, of the artwork were told their corresponding physical piece would be destroyed. </p> <p dir="ltr">Asked how he felt about burning the works, Hirst said, "It feels good, better than I expected."</p> <p dir="ltr">The artist himself burned each work individually, with the estimated cost of the works being burned equated to almost $18 million (AUD). </p> <p dir="ltr">Live-streaming the event, the Turner Prize winner and assistants used tongs to deposit individual pieces stacked in piles into fireplaces in the gallery as onlookers watched.</p> <p dir="ltr">"A lot of people think I'm burning millions of dollars of art but I'm not," Hirst said. "I'm completing the transformation of these physical artworks into NFTs by burning the physical versions.</p> <p dir="ltr">"The value of art, digital or physical, which is hard to define at the best of times will not be lost; it will be transferred to the NFT as soon as they are burnt."</p> <p dir="ltr">Hirst launched his first NFT collection last year, called <em>The Currency</em>, which was made up of 10,000 NFTs, corresponding to 10,000 original pieces of art, forcing buyers to choose what medium they would receive. </p> <p dir="ltr">According to London’s Newport Street Gallery, 5,149 buyers opted for the physical works while 4,851 chose the NFTs. </p> <p dir="ltr">Hirst’s works will continue to be burned until <em>The Currency</em> exhibition closes on October 30th. </p> <p dir="ltr"><em>Image credits: Getty Images</em></p>
