"Losing his speech": Home Alone star's major health update

<p><em>Home Alone</em> star Ken Hudson Campbell's daughter has shared a health update for fans after the actor underwent surgery to remove a tumour in his mouth. </p> <p>Speaking to <em>People Magazine</em>, Michaela revealed that her father is  "doing well" after his 10-hour surgery last week, but there are still a few challenges their family will have to face, as Campbell will need to have radiation treatment five times a week. </p> <p>"I think the hardest part of this is going to be him losing his speech and his ability to talk. So speech therapy is going to play a big role in it," she told the publication. </p> <p>"We're kind of playing it one step at a time, depending on how debilitated he is.</p> <p>"If he needs extra help, we might put him into a nursing home for a little while, but if not, we're going to take him back home and we still will be hiring post-op care help with that."</p> <p>This comes after Michaela and her family managed to raise $102,000 USD ($AU 152,000) for their father's treatment through crowd-funding, after the actor lost his SAG-AFTRA health insurance in January last year. </p> <p>Campbell, who played Santa Claus in <em>Home Alone</em>, "burst into tears" when he saw the messages and well wishes on the crowd-funding page.</p> <p>He was diagnosed with Squamous cell carcinoma - or oral cancer, on October 27. </p> <p>His family are hoping for a full recovery for the actor who they described as "a loving father of two children with an admirable lust for life, and a passion for acting, writing, &amp; sports." </p> <p><em>Images: Instagram</em></p>

Ken’s rights? Our research shows Barbie is surprisingly accurate on how ‘men’s rights activists’ are radicalised

<p><em><a href="https://theconversation.com/profiles/lucy-nicholas-145660">Lucy Nicholas</a>, <a href="https://theconversation.com/institutions/western-sydney-university-1092">Western Sydney University</a></em></p> <p>In the Barbie movie, we open with a picture of a perfect Barbieland where (almost) everyone is happy, diversity and sisterhood are embraced, and Barbies hold all positions of power.</p> <p>The Kens however, reflecting the popularity of the dolls in the real world, play a mainly decorative role.</p> <p><em>Spoilers for Barbie follow.</em></p> <p>In the film, we see a disgruntled Ken (played hilariously by Ryan Gosling) follow “Stereotypical” Barbie (Margot Robbie) to the real world where she has to find her human owner. This is all to fix an error that is allowing the real world to seep into Barbie land, with symptoms such as Barbie having an existential crisis.</p> <p>In the real world, Ken discovers the concept of the patriarchy. This sees him take a journey that is clearly influenced by, and pokes fun at, many aspects of contemporary anti-feminist men’s rights culture.</p> <figure><iframe src="https://www.youtube.com/embed/pBk4NYhWNMM?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <h2>Barbieland and the matriarchy</h2> <p>It has been proposed that <a href="https://theconversation.com/in-greta-gerwigs-barbie-land-the-matriarchy-can-be-just-as-bad-as-the-patriarchy-209317">Barbieland is a matriarchy</a>, but I would argue that their attitude to Kens is instead indifference.</p> <p>Ken was aggrieved that Barbie didn’t notice him and reciprocate his affections. This is not dissimilar to the grievances of some real-life men under contemporary feminism. Why don’t women’s lives revolve around them? And what can they do to address this perceived injustice?</p> <p>The movie cleverly parallels the emotions, narratives and logics that lead men to extreme antifeminist and misogynistic thinking, and in doing so exposes the flimsiness of their foundations.</p> <p>Having undertaken <a href="https://apo.org.au/sites/default/files/resource-files/2020-12/apo-nid307612.pdf">research</a> on online antifeminist discourses, Ken’s journey from aggrievement to masculine “enlightenment” parallels themes we found in Men’s Rights Activist spaces.</p> <p>Radicalisation into this world is often motivated by a feeling among boys and men of being left behind by a feminist world or system that doesn’t value them. This then leads them to long for an imagined natural order of patriarchy where women are back in their place and men regain their entitlements.</p> <p>These logics underpin <a href="https://theconversation.com/yes-the-incel-community-has-a-sexism-problem-but-we-can-do-something-about-it-207206">incel</a> culture, a movement that is increasingly understood as a terror threat, and has been associated with various acts of terror, such as the 2022 Aotearoa New Zealand <a href="https://www.nzherald.co.nz/nz/closest-to-an-incel-attack-nz-has-had-experts-concern-after-attempted-murder-of-auckland-schoolgirls/HYPEVZ6F4BFT3CV2O4SXS5FR7U/">Epsom crash case</a>.</p> <h2>The manosphere and MRAS</h2> <p>The “<a href="https://link.springer.com/chapter/10.1007/978-3-319-68360-7_2">manosphere</a>” can be understood as a loose coalition of antifeminist online subcultures.</p> <p>This includes MRAs (<a href="https://journals.sagepub.com/doi/full/10.1177/1097184X15574338">Men’s Rights Activists</a>) who claim reverse discrimination and that feminism has gone too far, and <a href="https://journals.sagepub.com/doi/full/10.1177/1097184X18816118">Redpillers</a> who claim to have swallowed the “red pill” to see the truth about feminism’s dominance. PUAs (<a href="https://repository.upenn.edu/entities/publication/d8971ded-1ec3-4939-8c0a-ab248dad62ba">Pick up Artists</a>) teach men how to manipulate the women they feel they are entitled to, to give them sex; and MGTOWs (<a href="https://www.tandfonline.com/doi/full/10.1080/1369118X.2020.1751867">Men Going Their Own Way</a>), who are antifeminist separatists (from women).</p> <p>Some of the most well known members of the manosphere are incels (involuntary celibates) a misogynistic community of self identified “beta-males” who want an end to women’s rights which prevent them from getting sex.</p> <h2>Ken’s grievances</h2> <p>Like many MRAs, Ken struggles with a sense of entitlement romantically (rather than sexually in genital-free Barbieland), and in attitudes to power and respect.</p> <p>Ken was being “<a href="https://www.merriam-webster.com/dictionary/friend%20zone">friend-zoned</a>” by Barbie, who despite being “boyfriend and girlfriend” wouldn’t let him stay over at the Dream House, because “every night is girl’s night”.</p> <p>This is coupled with a feeling of not being special, as Ken is essentially interchangeable with any other Ken. He is also “alpha’d” by other Kens: in the language of the manosphere, Barbie is a <a href="https://www.abc.net.au/news/2018-04-27/incels-inside-their-terrifying-online-world/9700932">“Stacy” and the other Ken is an alpha “Chad”</a> preventing him from getting what he wants.</p> <p>In <a href="https://apo.org.au/sites/default/files/resource-files/2020-12/apo-nid307612.pdf">our data</a> we found women are often described as “overlords”, man-haters, misandrists and “feminazis”. Among other concerns, men perceive economic loss due to women’s participation in the workplace, and crucially a lack of men’s sexual access to women brought about by the gains of feminism such as the awareness raised around consent by the #metoo movement.</p> <p>These men all share a starting point of grievance at women and their perceived indifference towards them.</p> <p>When Ken goes to the real world, he discovers patriarchy and he LOVES it. He has been “redpilled”. Patriarchy explains his aggrievement, and affirms his feelings. He takes patriarchy back to Barbieland and transforms it to Kendom, where the men change it to a society oriented around men and their power (and horses…).</p> <figure><iframe src="https://www.youtube.com/embed/Y1IgAEejvqM?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <h2>Redpilled ken</h2> <p>This redpilled Ken is a hilarious parody of the “neomasculinity” of the pick up artist (PUA) movement, that seeks to restore a masculine-centred world.</p> <p>Neomasculinity is about a belief in biological difference, traditional masculinity and heteronormative gender roles.</p> <p>The amusing depiction of the Kens trying to perform traditional <a href="https://www.britannica.com/topic/hypermasculinity">hypermasculinity</a> and needing their egos stroked – such as in a hilarious scene where the Kens are serenading the Barbies on the beach with an acoustic rendition of Matchbox Twenty’s <a href="https://youtu.be/HAkHqYlqops">Push</a> (“I wanna push you around … I wanna take you for granted”) – brilliantly shows the extent to which toxic masculinity is learned.</p> <figure><iframe src="https://www.youtube.com/embed/HAkHqYlqops?wmode=transparent&amp;start=0" width="440" height="260" frameborder="0" allowfullscreen="allowfullscreen"></iframe></figure> <p>Additionally, the competition among the Kens (that the Barbies ultimately stoke to overturn the Kentriarchy) is the perfect illustration of the damage toxic models of masculinity does to men. As Australian sociologist Raewyn Connell has long argued, almost no men can live up to <a href="https://www.jstor.org/stable/27640853">masculine ideals</a>, resulting in negative outcomes not just for women but also for men themselves.</p> <h2>Finding the real Ken</h2> <p>The movie ends with Barbie, her human (America Ferrera), “Weird Barbie” (Kate McKinnon) and Allan (Michael Cera) deprogramming the brainwashed Barbies and turning the Kens against each other.</p> <p>But what of Just Beach Ken? And what can we learn from this for preventing or managing radicalisation of this feeling of aggrievement in real men or boys?</p> <p>Well, Barbie and Ken reach a middle ground. Barbie encourages Ken to work out who he is outside of his relation to Barbie, and to learn being Just Ken is enough.</p> <p>This isn’t dissimilar to the methods of men’s behaviour change programs and counselling for men who use violence, which use <a href="https://www.tandfonline.com/doi/full/10.1080/10926771.2023.2189043">trauma-informed motivational interviewing</a>, reflect on challenging gender norms and breaking down rigid thought processes, and developing emotional literacy and communication strategies.</p> <p>But it also illustrates that men and boys need alternative narratives to make sense of themselves in the world, and alternative communities for affirmation, before it gets to this stage.</p> <p>In <a href="https://apo.org.au/sites/default/files/resource-files/2020-12/apo-nid307612.pdf">our report</a>, we recommended: "Providing alternative narratives and considering how far-right [or MRA] groups provide men with emotional support networks, with a view to providing better alternatives."</p> <p>Feminism has consistently been about separating attributes from their gendered associations, breaking down the Barbie/Ken binary. So if there is one thing we can take away from the Barbie movie, it is that hierarchy and rigid gender benefits nobody, and power and social roles have nothing to do with the genitals you are born with.<!-- Below is The Conversation's page counter tag. <p dir="ltr">Naomi Campbell surprised fans overnight by announcing that she had secretly welcomed a baby boy.</p> <p dir="ltr">The 53-year-old model took to Instagram to share an adorable snap of her cradling the newborn while her two-year-old daughter holds both her mum and baby brother’s hand.</p> <p dir="ltr">"My little darling, know that you are cherished beyond measure and surrounded by love from the moment you graced us with your presence. A True Gift from God 🙏🏾, blessed ! There is no greater love," she captioned the photo with her first born.</p> <p dir="ltr">Fans and celebrities alike have taken to the comment section to congratulate Campbell.</p> <p dir="ltr">“NAOMI!!! Congratulations!!” wrote American singer, Kelly Rowland.</p> <p dir="ltr">“Oh my goodness! Welcome!! Goldie Hawn celebrates her 77th birthday

Royal biographer hints at Queen's cause of death

<p dir="ltr">A royal expert claims Queen Elizabeth II was suffering from a “relatively painless” but “invariably fatal” condition before announcing her death 90 minutes before Buckingham Palace.</p> <p dir="ltr">In a video uploaded to YouTube at 5pm local time (4am NZST), controversial royal biographer Lady Colin Campbell claimed the Queen had passed away at 2.37pm (1.37am NZST).</p> <p dir="ltr">Buckingham Palace announced the monarch’s passing 90 minutes later, at around 6.30pm UK time.</p> <p dir="ltr">Her announcement came at the end of a lengthy clip where the royal expert spoke about a condition the Queen was allegedly suffering from.</p> <p dir="ltr">Lady Campbell, who is most well-known for her books about Princess Diana and the Queen Mother, claimed that Her Majesty was suffering from a serious bone condition, though she wouldn’t reveal “the word that accurately conveys her diagnosis” out of respect for her “dignity and privacy”.</p> <p dir="ltr">“If she wants to reveal that word, or her advisors wish to reveal it, that is up to them. I don’t think one needs to use the word to get across the point that I think most people will be able to pick up, that this is a really serious situation,” Lady Campbell said in the video, prior to announcing the monarch’s passing.</p> <p dir="ltr">“The condition has been induced, in part, according to people who know her well, has been created by the tremendous stress to which she has been subjected over the last three years.”</p> <p dir="ltr">The Palace has not confirmed the Queen’s cause of death.</p> <p dir="ltr">“Can you imagine an older woman, as her life is winding down, and she is hoping to enjoy the last few years of her life in good health being bombarded by the tremendous abuse to which she and the monarchy have been subjected,” Lady Campbell said.</p> <p dir="ltr">She went on to say she had tried to warn people that the Queen was “far more ill than they thought she was” over the past few months.</p> <p dir="ltr">“I have on several occasions in the last few weeks, if not months, made the point that she had been affected to her bones. I used that repeatedly to get across the point that what she was suffering from was a malady of the bones,” she continued.</p> <p dir="ltr">“There are two maladies of the bones, one is more painful than the other. Fortunately the Queen’s malady, although it falls in the same category and condition of the more painful one, has been the less painful one.</p> <p dir="ltr">“It has been restrictive, and I will not go into the medical treatments she has been receiving. I have previously indicated that her bruising was due to cannulas and I have left it at that.”</p> <p dir="ltr">After the video cuts away, Lady Campell continued filming, claiming she had found out about the Queen’s passing.</p> <p dir="ltr">“Having just made this video it is with great sadness that I have to inform you that events have yet again overtaken one’s plans, and I am reliably informed that the Queen died at 14.37pm this afternoon,” she said.</p> <p dir="ltr">“And that the reason why the announcement has not been made so far is that they are waiting for Harry and Meghan to arrive at Balmoral, after which the announcement will be made.”</p> <p dir="ltr">Buckingham Palace went on to announce the news of Her Majesty’s passing before Harry arrived at Balmoral, while Meghan remained in London.</p> <p dir="ltr">“I think we should be very grateful for having had such a wonderful monarch,” Lady Campbell continued.</p> <p dir="ltr">“And I think we can be also grateful for the fact that her death was relatively painless. Bone cancer is not fun.</p> <p dir="ltr">“But she was fortunate enough to have the lesser of the forms of bone cancer, and she kept her spirits and her vitality to practically the end.</p> <p dir="ltr">“And now, I would say, my sympathies to all her loved ones, all her family, and really, all her subjects many of whom love her.”</p> <p dir="ltr">However, the <em><a href="https://www.couriermail.com.au/news/world/queen-elizabeths-cause-of-death-may-never-be-released/news-story/47ceca6491d9ef44b1d9112061674cdb" target="_blank" rel="noopener">Courier Mail</a></em> reported that medical experts said frailty and “geriatric syndrome” - a term describing a group of common health conditions older people experience that don’t fit in distinct disease categories - could have been contributors to her passing.</p> <p dir="ltr">Her symptoms reportedly met five of seven criteria used by Britain’s NHS to classify people as frail, including being over 85, having ongoing health conditions, requiring regular help, being forced to cancel activities and using a walking stick.</p> <p><span id="docs-internal-guid-1630db36-7fff-3f8c-cae6-c2b858607b4b"></span></p> <p dir="ltr">In the hours before her death, a statement from Buckingham Palace said doctors were “concerned” for the Queen’s health and that she was “comfortable” at Balmoral.</p> <p dir="ltr"><em>Images: Getty Images / Lady Colin Campbell (YouTube)</em></p>

Now a year on, I have tripled my number of children and my resolve," he told the publication.</p> <p>David's rocker dad Jimmy Barnes has also openly spoken about his addictions in his memoir Working Class Man. During an interview with Now To Love, Mahalia Barnes revealed why her family is so open to sharing their stories.</p> <p>"I think that being in the public profile, you do have less privacy, obviously. That's just how it works; it sort of comes with the territory.</p> <p>"And obviously, there are things that you can choose to keep private, or you can speak openly. <p>Naomi Campbell is a mum.</p> <p>The 50-year-old British supermodel has surprised fans with the announcement that she welcomed her first child, a baby girl.</p> <p>“A beautiful little blessing has chosen me to be her mother, So honoured to have this gentle soul in my life there are no words to describe the lifelong bond that I now share with you my angel. There is no greater love,” she captioned a photo of herself holding the newborn’s feet, which she posted on Instagram.</p> <p>The father of the child and whether Campbell gave birth or hired a surrogate is unknown.</p> <p>Speaking to Page Six, an insider said: “Naomi has been wanting to be a mother for a long time and it finally happened. Renowned psychic believes David Campbell’s son is Princess Diana reincarnated

<p>Network Nine TV host David Campbell made headlines on Sunday when he said that his son Billy, 4, believes he is the “reincarnation” of Princess Diana.</p> <p>The convincing account is dividing public opinion, but Queensland-based psychic medium Peter Williams believes that it’s possible. You can't tell them what you're looking for. It is going to be so hard now to get this information without contaminating it, or without having a sense of bias or leading their son into this.”</p> <p>Peter also mentioned that there is a limited window into the previous lives of people.</p> <p>“Aged four through to six is the main bracket," he said. “Once they hit six or seven, a lot of children tend to lose those memories as they start school.</p> <p>“It is almost like we become tainted by this world … everything becomes black and white when you start school.”</p> <p>Peter concluded: “This four-year-old has no doubt in his mind he is Princess Diana, so the information that's going to come through will be absolutely bang on. Rock Hudson’s “true love” shares details about their forbidden romance

<p>Lee Garlington dated movie star Rock Hudson in the '60s, a time when men pursuing relationships with one another was deemed unacceptable. Now, years later, Garlington looks back fondly on the secret moments the two shared together.</p> <p>“We couldn’t take any pictures together, it was too dangerous,” said Garlington.</p> <p>“We could only take pictures of each other.”</p> <p>Hudson was the leading man of Hollywood. She told him to visit Puerto Vallarta, which back then, was a quiet town.</p> <p>“We walked on the beach and took pictures of each other with his camera and drove around in an open jeep,” Garlington revealed. “We just lived the life of two normal gay men that loved one another. There were no paparazzi, and no one knew we were there. We were just comfortable being us.”</p> <p>The couple only had one picture together, which was taken at a bar in New Orleans. “His agent told him that he was never to have one of his boyfriends in a photo because if anyone saw it, they would suspect he was gay.”</p> <p>According to Garlington, Hudson “did not have the opportunity to live his life the way he wanted to and he had to go around hiding".</p> <p>“I wish he had been born 30 or 40 years later. He’d be more relaxed and at ease and it would have been a happier life. I had no idea I meant that much to him.”</p> <p>Years later, Garlington is now happily married to Paul Garlington, a man who he has been committed to for the last 32 years. But despite it all, he still remembers the special moments he shared with Hudson.</p> <p>“I remember how handsome he was and what a great time we had together. He was the kindest man I ever met.”</p>

Why Jimmy Barnes abandoned son David Campbell as a baby

<p>Jimmy Barnes has opened up about the biggest regret of his life, abandoning his son David Campbell when he was young.</p> <p>Speaking to <a href="https://www.who.com.au/"><em><strong><span style="text-decoration: underline;">Who</span></strong></em></a> magazine, the Aussie musician revealed why he decided to run away from his responsibilities when his eldest son, now 45, was born.</p> <p>The 62-year-old Cold Chisel star said it wasn’t his “beautiful boy” he was running from, but rather the pain from his difficult childhood and the belief that his son would be better off without him.</p> <p>In the interview, Jimmy discussed the trauma he experienced growing up witnessing his father “bash” his mother.</p> <p>“I guess I ran away from the responsibility of [being a parent]. I just felt like I was dying [felt overwhelmed] I thought there were no prospects, no hope,” he said.</p> <p>“It was just a matter of time before I died. That's why I was trying hard to flee.”</p> <p>David, who is now the <em style="font-weight: inherit;">Today Extra</em> co-host, was left in the care of his maternal grandmother.</p> <p>Until the age of 10, David believed that Jimmy was just a “family friend” and his mother, Kim, was “his sister”.</p> <p>Jimmy candidly revealed that leaving his son was not the first time he ran away from a situation in his life.  </p> <p>The rock star had to flee his South Australian family home growing up to escape the domestic violence, abuse and alcoholism that ran rampant.</p> <p>The singer, whose family moved from Scotland to Australia when he was five years old, said his parents migrated to escape their own problems.</p> <p>"[My parents] were running away from stuff without dealing with it ... they came 12,000 miles and had the same problems [in Australia] they had in Glasgow. “The Crown” star Claire Foy’s sad announcement

<p>The family of legendary country music singer Glen Campbell have announced his passing at the age of 81. “It is with the heaviest of hearts that we announce the passing of our beloved husband, father, grandfather, and legendary singer and guitarist, Glen Travis Campbell ... following his long and courageous battle with Alzheimer’s disease,” they wrote on Facebook yesterday.</p> <p>His daughter, Ashley Campbell, took to Twitter to share a heartbreaking yet touching tribute to her beloved dad.</p> <blockquote class="twitter-tweet"> <p dir="ltr">Heartbroken. His best-loved songs include “Gentle on My Mind”, “Rhinestone Cowboy”, “By the Time I Get to Phoenix”, “Wichita Lineman”, “Galveston”, “Southern Nights” and “The Hand That Rocks the Cradle”.</p> <p>Campbell also found success on the small screen, hosting his very own music and comedy variety show, <em>The Glen Campbell Goodtime Hour</em>, from 1969 to 1972. A successful actor as well, he was lauded for his role in 1969’s <em>True Grit</em>, for which he received both Golden Globe and Academy Award nominations.</p> <p>He is survived by his wife, Kim Woollen, and his eight children, Debby, Kelli, Travis, Kane, Dillon, Cal, Shannon and Ashley.</p>

Mother confirms death of 15-year-old daughter in Manchester blast

<p>Charlotte Campbell had to wait by the phone for a day, unsure of her daughter’s wellbeing, after her daughter went missing after the Manchester Arena blast.</p> <p>The mother broke down in tears on<em> Good Morning Britain</em> hours before learning of her daughter’s death.</p> <p>"All I know is she was at the Manchester Arena with her friend watching Ariana Grande and she's not turned up yet," she said.</p> <p>"I can't get through to her. I've called the hospitals, I've called all the places, the hotels where people say that children have been taken. I've called the police. There's no news, I've just got to wait. I'm waiting at home just in case she turns up here."</p> <p>The mother rang police, hospitals and hospitals but was told to wait. She shared her daughter’s picture on Facebook and Twitter in the hope that someone would be able to find her.</p> <p>Charlotte told the<em> BBC</em> that she lost contact with her daughter just before Ariana Grande was about to perform.</p> <p>"She was at the concert, she'd just seen the support act and said she was having an amazing time and thanking me for letting her go," she said.</p> <p>"She was with her friend, Adam. Adam was found about half an hour ago – he's in hospital – but Olivia's not been found yet ... I'm just hearing nothing."</p> <p>"It's the most horrible feeling ever, to know that your daughter's there and you can't find her and you don't know if she's dead or alive," Charlotte told <em>CNN.</em></p> <p>"I don't know how people can do this to innocent children."</p> <p>However, on Tuesday night Charlotte identified her 15-year-old daughter, Olivia Campbell, as one of the victims of the suicide bombing at Manchester Arena where singer Ariana Grande had performed for nearly 20,000 fans.</p> <p>"RIP my darling precious gorgeous girl Olivia Campbell taken far far to soon go sing with the angels and keep smiling mummy loves you so much," Charlotte wrote on Facebook and then said on Twitter, "RIP my beautiful princess Olivia Campbell. Gone far too soon &amp; never will be forgotten. I love you.”</p> <p style="text-align: center;"><iframe src="https://www.facebook.com/plugins/post.php?href=https%3A%2F%2Fwww.facebook.com%2Fcharlotte.campbell.104%2Fposts%2F10156083791962004&amp;width=500" width="500" height="607" style="border: none; overflow: hidden;" scrolling="no" frameborder="0" allowtransparency="true"></iframe></p> <p>The post received an outpouring of grief and condolences.</p> <p>"So sorry for your loss, your daughter is absolutely beautiful. Prayers from the USA," Debra Sanders said.</p> <p>"Just know that there's an entire world out here that is grieving for your loss," Peggy Jones said.</p> <p>Victims who have been identified since the bombing include Martyn Hett, Kelly Brewster, Marcin and Angelika Klis and John Atkinson. Also among the 22 victims were eight-year-old Saffie Rose Roussos and eighteen-year-old Georgina Callander. </p>

What I’ve learnt about dying as an intensive care physician

<p><em><strong>Ken Hillman is Professor of Intensive Care at the University of New South Wales, the Director of the Simpson Centre for Health Services Research and is an actively practising clinician in Intensive Care, at Liverpool Hospital.</strong></em></p> <p>As an intensive care physician I’m increasingly confronted with managing patients who are at the end of their life. Australians need to be aware that the way that they will spend the last few days or weeks of their lives is largely predetermined, not by their own wishes but by a medical conveyor belt from the community into acute hospitals and from there into intensive care units.</p> <p>There’s no conspiracy behind this, it has just happened this way. The drivers include unreal societal expectations of what modern medicine can and, more importantly, cannot offer, fed by daily reports of the latest miracle cures; a medical profession that’s uncomfortable with discussing dying and death; medical specialisation that has resulted in amazing advances but focuses on specific single-organ problems and not the patient’s overall health status; and a lack of doctors who can stand back and recognise patients who are at the end of their lives. All this is reinforced by a society reluctant to openly discuss issues around ageing and dying. The perfect storm.</p> <p>Interestingly, nobody wants it this way. Almost 70 per cent of Australians want to die in their own homes. Yet, almost 70 per cent will die in acute care hospitals.</p> <p>People who suddenly become ill in their homes or in the community usually have an ambulance called. They are now on the conveyor belt. Ambulance personnel have no discretionary power – they have to take the patient to an acute hospital for further assessment.</p> <p>Acute illness or trauma is frightening and most of us have little knowledge of what is available in the acute hospital. So, the journey starts – and for many, it’s appropriate. Medicine can perform some miracles. But for others, the so-called illness state is a normal and expected part of the dying process. Differentiating can be difficult.</p> <p>The major challenge is to identify a potentially reversible component of a disease. Something that medicine can recognise and reverse – a patient who has fallen and fractured his hip can have it repaired, for instance.</p> <p>But for many older people, there’s often little that’s amenable to modern medicine. As people age, they collect chronic health conditions or co-morbidities – this is the medicalisation of the ageing process. These conditions can sometimes be controlled but they’re not usually reversible.</p> <p>Organ function declines markedly with age. Muscles become weaker, bones become more brittle, vital organ function deteriorates, brain function diminishes and wrinkles appear. The rate at which this occurs is encoded at conception and is called apoptosis – the programed death of cells and tissues.</p> <p>You can optimise your chances of reaching your apoptotic potential with the help of living healthily and modern medicine. Diabetes can be controlled, for instance, and coronary arteries unblocked. Nevertheless, ageing is unavoidable and dying inevitable. Eventually the combination of chronic conditions means that even a small acute problem such as a simple urinary tract infection can result in death. This presents the dilemma for medicine and patients – how far do we go to sustain life?</p> <p>Doctors are programed to cure. In an age of medical specialisation, they concentrate on incremental improvements in care of their own organ and refer to colleagues for advice about the other problems. As a result, elderly patients are often taking many medications with little or no benefit in the context of their chronic health status.</p> <p>Clinical trials showing the efficacy of medicines are conducted in selected patients, not 90-year-olds with many chronic health problems. And when the end is finally near, those at the end of their lives come to hospitals for their last few days or weeks. Many are placed on life support machines and can no longer relate to their relatives and friends. Those who are conscious often plead to be allowed to die.</p> <p>As an intensive care specialist I often become frustrated with my colleagues’ failure to recognise when patients are at the end of life. One of the worst phone calls an intensivist can receive from a colleague goes something like this, “I’ve had a chat to the relatives and they say they want everything done, can you help?”</p> <p>This puts people like me in a difficult position. First, there’s an inference that what we can do will make the patient better. Then there’s the difficult situation of having to explain for the first time that we believe the patient is at the end of her life and any further active management would be futile.</p> <p>The speciality of intensive care has a special responsibility to begin a frank and open discussion with our society about the limitations of modern medicine and the inevitability of ageing and dying.
